DESCRIPTION
Examination of the vulva and vagina is an integral and important part of every gynecological examination. The appearance of the vulva and vagina varies normally in relation to age and hormone status and changes in the skin of the vulva and vagina must take into account the expected texture of the epithelium for any given age. Good lighting and, where indicated, a magnifying glass or a colposcope, are needed for a systematic examination.
Both benign and malignant tumors of the vulva and vagina are relatively uncommon.
COMMON VAGINAL INFLAMMATIONS
This page will diagnostically compare four of the most common inflammatory problems that frequently effect the vagina. I will not discuss the treatment here. These links will provide more indepth discussion, treatment and prevention recommendations as well as nutritional support recommendations to help healing.
It is very important that diagnosis and treatment should be decided in consultation with the health care practitioner. Treatment depends on many factors and the final decision should be made after a discussion between health care provider and patient.
TRICHOMONAS VAGINITIS
Vulva: Often inflamed.
Urethritis: Usually absent, but a Trichomonas cystitis occurs occasionally.
Bartholin's Gland Infection: Absent.
Vaginal Mucosa: Red, inflamed, with red granular or petechial spots in the fornix.
Cervix: May show red spots ("strawberry spots").
***Many patients present less typical signs. Definitive diagnosis depends on identification of the causative organism.

MONILIA (CANDIDA) VAGINITIS
Vulva: Often inflamed.
Urethritis: Absent.
Bartholin's Gland Infection: Absent.
Vaginal Mucosa: Red, inflamed, with white or grayish often tenacious patches of discharge. May bleed when plaques of discharge are scraped off.
Cervix: May show patches of discharge.
***Many patients present less typical signs. Definitive diagnosis depends on identification of the causative organism.
EARLY GONORRHEA
Vulva: Often inflamed.
Urethritis: Often present. The urethral orifice is red and swollen. Pus may be milked from the urethra and sometimes from Skene's glands.
Bartholin's Gland Infection: May be present.
Vaginal Mucosa: Usually normal in adult.
Cervix: May be inflamed; pus exudes from os.
***Many patients present less typical signs. Definitive diagnosis depends on identification of the causative organism.
SENILE VAGINITIS
Vulva: Atrophic.
Urethritis: Absent.
Bartholin's Gland Infection: Absent.
Vaginal Mucosa: Atrophic, dry, with superficial erosions and filmy adhesions. Bleeds easily.
Cervix: Small.
