These forms are to be used for design ideas. I believe each midwife should design her own forms to reflect her own practice. However, it is nice to have some basic ideas to begin with and then allow individual creativity to influence the final result.
If you want a free copy of any one of these forms, please send a self-addressed, stamped envelope and I will print out a copy and send it to you. The forms vary in length from 1 page to several pages in length. Click on the specific form you would like to receive and check on the number of pages it contains before making a request.
Be sure to check with your postal service to ensure proper postage is on the appropriate sized envelope for the particular form you would like sent to you. New postal changes need to be kept in mind.

If you want copies of all my forms in Microsoft Word 2002 (.doc format) and/or Microsoft Works 8.5 2005 (.wps format) on a CD, please enclose a check or money order (in US dollars only) for $20.00 with your request. This price includes shipping and handling costs. These can then be downloaded on your computer and altered using your computer program for your own use. I always include several client handout articles (.html format) and other helpful information on the CD as well as the forms.
I apologize for having to raise the price of the CD. The rising costs of materials and the recent postage changes (2007) have forced me to do this.
I can custom-make the CD to your specifications and include any number of articles or information available on my website at no extra cost. Submit this information with your order and the specific URLs for the articles you wish included.
