DESCRIPTION

The three major forms of male sexual dysfunction are erectile dysfunction, ejaculatory dysfunction and decreased libido. Erectile dysfunction is a common problem in men over the age of 40 due to hormonal abnormalities, medications, psychological problems, neurologic disease, or vascular insufficiency. Men with this disorder experience significant psychological distress which improves if treatment successfully restores erectile function. Premature ejaculation (or rapid ejaculation) may be an even more common form of sexual dysfunction.

While research suggests that sexual dysfunction is common (43% of women and 31% of men report some degree of difficulty), it is a topic that many people are hesitant to discuss. Fortunately, most cases of sexual dysfunction are treatable, so it is important to share your concerns with your partner and health care provider.

There are different types of ejaculation disorders, including:
  • Premature ejaculation: This refers to ejaculation that occurs before or soon after penetration.
  • Inhibited or retarded ejaculation: This is when ejaculation is slow to occur.
  • Retrograde ejaculation: This occurs when, at orgasm, the ejaculate is forced back into the bladder rather than through the urethra and out the end of the penis.

Male sexual dysfunction involving Premature Ejaculation (PE), also known as rapid ejaculation, early ejaculation, or by the Latin term ejaculatio praecox. It is described as male orgasm and ejaculation following brief sexual stimulation, and prior to satisfactory arousal and orgasm in the sexual partner in more than 50 percent of his sexual encounters (Masters & Johnson). Other sex researchers have defined premature ejaculation as occurring if the man ejaculates within 2 minutes of penetration, however, a survey by Alfred Kinsey in the 1950s demonstrated that 3/4 of men ejaculated within 2 minutes of penetration in over half of their sexual encounters. Today, most sex therapists understand premature ejaculation as occurring when a lack of ejaculatory control interferes with sexual or emotional well-being in one or both partners. Definition criteria vary, e.g., inability to maintain an erection of sufficient duration to satisfy a partner, or ejaculation that occurs before individual wants it to.

This is a common sexual disorder affecting all age groups of men affecting 20% to 30% of men. It is characterized by a constant inability or lack of voluntary control over the ejaculatory reflex. In some cases, premature and inhibited ejaculation are caused by psychological factors, including a strict religious background that causes the person to view sex as sinful, a lack of attraction for a partner and past traumatic events. Premature ejaculation, the most common form of sexual dysfunction in men, often is due to nervousness over how well he will perform during sex. Certain drugs, including some anti-depressants, may affect ejaculation, as can nerve damage to the spinal cord or back.

Retrograde ejaculation is common in males with diabetes who suffer from diabetic neuropathy (nerve damage). This is due to problems with the nerves in the bladder and the bladder neck that allow the ejaculate to flow backward. In other men, retrograde ejaculation occurs after operations on the bladder neck or prostate, or after certain abdominal operations. In addition, certain medications, particularly those used to treat mood disorders, may cause problems with ejaculation.

    GENERAL MEASURES

  • Laboratory test results are usually normal, since most males with this problem are healthy individuals.


  • Work to improve communications with your partner. Sexual and/or marriage-partner counseling may be necessary to improve communication skills between partners.


  • Try to reduce your performance anxiety.


  • The following methods are recommended by sex researchers and therapists Masters and Johnson. These methods will usually lead to ejaculatory control for 5 to 10 minutes or longer:
      1. Sensate-focus exercises, in which each partner caresses the other's body without intercourse to learn the relaxed, pleasurable aspects of touching.

      2. Mutual physical examination of each other's bodies to acquaint both partners thoroughly with anatomy. This helps reduce shameful feelings about sex.

      3. Stop-and-start technique, in which the man is stimulated through controlled intercourse or masturbation until he feels an impending ejaculation. Stimulation is stopped, then resumed in 20 to 30 seconds.

      4. Squeeze technique, in which the woman squeezes her partner's penis with her thumb and forefinger when he feels an impending ejaculation. When ejaculatory feelings pass, intercourse is resumed. This is repeated as often as necessary until the man can control ejaculation to the satisfaction of both partners.

      5. Individual psychotherapy may be recommended for some men, and marital therapy for couples may be helpful.

      6. Counseling from a qualified sex therapist if other methods are not successful.

      Drug Treatment For Premature Ejaculation Is Effective

      University of Minnesota researchers have shown that the drug dapoxetine is a safe and effective drug treatment for premature ejaculation.

      The drug, dapoxetine, is a selective serotonin re-uptake inhibitor (SSRI) that was developed specifically to treat premature ejaculation. SSRIs, normally used to treat depression, have been prescribed "off-label" for treatment of premature ejaculation, but those drugs need to be taken regularly in order to be effective. Dapoxetine, however, is a short-acting drug that has an effect within about one hour.

      This multicenter trial took place at 121 sites across the United States. Men in stable, heterosexual relationships took part in the trial, which divided the men into three groups. One group received a placebo, the second received 30 milligrams of the drug, and the third group received 60 mg of the drug. Both the study participants and medical personnel overseeing the trial were unaware who was assigned to each group. Both groups who received the drug increased their time to ejaculation. Jon Pryor, M.D., professor and chair of urological surgery, was the primary investigator of this clinical trial. The research will be published in the Sept. 9, 2006, issue of The Lancet.

      At the start of the study, the men ejaculated less than one minute after intercourse began. After the 12-week study, the time to ejaculation increased to 1.75 minutes for the placebo group, 2.78 minutes for the group on the lower dose, and 3.32 minutes for the higher dose. The time to ejaculation was measured by the use of a stopwatch by the men's partners. "Dapoxetine also improved patients' perceptions of control over ejaculation, satisfaction with sexual intercourse, and overall impression of the change in their condition," Pryor said. "The patients' partners also benefited through improved satisfaction with sexual intercourse."

      Premature ejaculation is thought to be the most common male sexual dysfunction, effecting between 21 and 33 percent of men. The underlying physical reason for PE is not well understood.

      The study was funded by ALZA Corporation, the company that developed the drug dapoxetine. Pryor has served on advisory boards for ALZA advisory boards in the past.

      DAPOXETINE FOR THE TREATMENT OF PREMATURE EJACULATION: LACK OF INTERACTION WITH PDE5 INHIBITORS

      INTRODUCTION AND OBJECTIVE: Dapoxetine is a medication currently in development for the treatment of premature ejaculation (PE), the most common male sexual dysfunction. The pharmacokinetics (PK) of dapoxetine make it well suited for on-demand treatment of PE. Dapoxetine is quickly absorbed, with peak concentrations occurring ~1.5 h after dosing, followed by a rapid decline in plasma concentrations, which decrease to ~5% of peak concentrations by 24 h post dosing. The objective of this study was to determine if dapoxetine has drug interactions with PDE5 inhibitors.

      INTRODUCTION AND OBJECTIVE: Dapoxetine is a medication currently in development for the treatment of premature ejaculation (PE), the most common male sexual dysfunction. The pharmacokinetics (PK) of dapoxetine make it well suited for on-demand treatment of PE. Dapoxetine is quickly absorbed, with peak concentrations occurring ~1.5 h after dosing, followed by a rapid decline in plasma concentrations, which decrease to ~5% of peak concentrations by 24 h post dosing. The objective of this study was to determine if dapoxetine has drug interactions with PDE5 inhibitors.

      METHODS: This study was a randomized, open-label, 3-period crossover study in which healthy adult male subjects (n = 24; 18-45 years of age) received a single administration of dapoxetine 60 mg, dapoxetine 60 mg + tadalafil 20 mg, and dapoxetine 60 mg + 100 mg sildenafil, each being administered in a separate treatment period. Each treatment was separated by a 6-14-day washout period. Blood samples were collected pre dose and at specified times after dosing for measurement of dapoxetine and its metabolites and tadalafil and sildenafil plasma concentrations. Safety was assessed by adverse event (AE) recording throughout the study and by laboratory tests. Heart rate, blood pressure, and respiratory rate were also measured pre dose and at various time points after treatment, and ECGs were performed at screening, post-dose at every treatment, and at termination.

      RESULTS: Tadalafil and sildenafil did not have a clinically significant effect on the PK of dapoxetine. With regard to the effect of tadalafil, the 90% confidence intervals (CIs) for the ratios of Cmaxand of AUCinf for dapoxetine + tadalafil compared with dapoxetine alone were (89.55, 108.23) and (96.21, 114.85), respectively. With sildenafil, the mean AUC of dapoxetine was 22% higher and the 90% CIs for the ratios of Cmaxand of AUCinf for dapoxetine + sildenafil compared with dapoxetine alone were (94.63, 114.37) and (106.90, 132.57), respectively. The magnitude of the change in dapoxetine AUCinf suggests that it is not likely to be clinically significant. Plasma concentrations of tadalafil and sildenafil were comparable to published reports, suggesting that dapoxetine does not affect the PK of these 2 PDE5 inhibitors. No clinically significant changes were observed in ECGs, clinical laboratory test results, or vital signs, and no serious AEs were reported.

      CONCLUSIONS: Dapoxetine has no significant PK interactions with tadalafil or sildenafil, and the combinations were well tolerated.

      EFFICACY AND TOLERABILITY OF DAPOXETINE IN THE TREATMENT OF PREMATURE EJACULATION

      INTRODUCTION AND OBJECTIVE: Premature ejaculation (PE) is the most common male sexual dysfunction; however, currently no medications are specifically indicated for PE. The objective of this analysis was to examine the efficacy and tolerability of on-demand administration of dapoxetine, the first medication developed specifically for the treatment of PE.

      METHODS: Men with PE (N = 2614), by DSM-IV-TR criteria, were enrolled in 2 randomized, double-blind, placebo-controlled, multicenter, phase III clinical trials of identical design. Each had a 2-week baseline period and a 12-week treatment period. At the end of the baseline period, subjects with mean intravaginal ejaculatory latency time (IELT) = 2 minutes were randomized 1:1:1 to placebo, dapoxetine 30 mg, or 60 mg, and instructed to take study drug 1-3 hours prior to anticipated intercourse. The primary efficacy endpoint was IELT adjusted for baseline, as measured by a stopwatch held by the female partner. Secondary endpoints (subject perception of Control Over Ejaculation and Satisfaction With Sexual Intercourse) were assessed at baseline, 4, 8, and 12 weeks, and rated on 5-point scales of 0=very poor to 4=very good. Tolerability was assessed. Groups were compared using ANOVA analysis.

      RESULTS: Preliminary results showed significant (all P<0.0001) differences between placebo and each active treatment group in mean IELT, Control Over Ejaculation, and Satisfaction With Sexual Intercourse. Changes from baseline to study endpoint for mean IELT were 0.90 to 1.75 min (placebo), 0.92 to 2.78 min (30 mg), and 0.91 to 3.32 min (60 mg). Changes from baseline to study endpoint in percentage of subjects giving ratings of fair, good, or very good for Control Over Ejaculation were 3.5% to 26.4% (placebo), 2.5% to 51.8% (30 mg), and 3.3% to 58.4% (60 mg), and for Satisfaction With Sexual Intercourse, 51.8% to 55.2% (placebo), 52.4% to 70.9% (30 mg), and 56.7% to 79.2% (60 mg). Both dapoxetine 30 mg (P=0.0006) and 60 mg (P<0.0001) were more effective than placebo in increasing IELT on first dose. Treatment-related adverse events (AEs) (incidence = 5%) with both 30 mg and 60 mg were nausea (8.7%, 20.1%) and headache (5.9%, 6.8%), and with 60 mg only, diarrhea (6.8%) and dizziness (6.2%). Discontinuation rates due to AEs for placebo, 30 mg, and 60 mg were 0.9%, 4%, and 10%.

      CONCLUSIONS: Dapoxetine 30 mg or 60 mg is effective for the on-demand treatment of PE, with increased IELT, improved Control Over Ejaculation, and increased Satisfaction With Sexual Intercourse. Dapoxetine was effective on first dose. Both doses were well tolerated.

      TREATMENT OF PREMATURE EJACULATION: A COMPARATIVE VARDENAFIL AND SSRI CROSSOVER STUDY

      INTRODUCTION AND OBJECTIVE: Premature ejaculation (PE) is one of the most common male sexual disorders. This condition may be primary or secondary to underlying disease. Control of PE has been focused on behavioral therapy, topical anaesthetics, tricyclic antidepressants and selective serotonin re-uptake inhibitors; however, an approved treatment does not exist. There are several reasons why Phosphodiesterase (PDE)5-inhibitors could have an potential effectiveness in the treatment of PE.

      METHODS: Healthy men evaluated for primary and secondary PE graded their ejaculation on a scale of 0 to 8 (0 = almost never, 8 = almost always). Baseline International-Index-of-Erectile-Function (IIEF) scores were obtained. The intravaginal ejaculatory latency time (IVELT) was measured by stop watch. 37 heterosexual men (age 38 ± 5,7, range 26-56) with a stable sexual relationship for at least one year who scored their PE as 4 or greater and IVELT ≤ 1,5 minutes were included in the study. They were randomized to 6 weeks vardenafil (10mg, 30 minutes pre-intercourse) or sertraline, a selective serotonin re-uptake inhibitor (SSRI) (50 mg 4 h pre-intercourse). After a 1-week wash-out period, medication was changed in a cross-over design. During the study PE was graded using the same scales after the initiation of each treatment. Partner sexual satisfaction, IIEF-scores and patient preference were also assessed.

      RESULTS: The mean initial PE grade was 6.14 ± 1.6 and that for IVELT was 0.54 minutes (range 0.1 to 1.5) for all participants. With vardenafil PE grade improved 2.9 ± 2.1 (P <0.01) and IVELT 5.23 ± 3.71, (P <0.001). Of the 34 men treated with sertraline improvement was reported (PE grade 1.86 ± 1.31, P <0.01; IVELT 2.87 ± 1.86, P <0.001). Partner sexual satisfaction and IIEF rate were greater with 10 mg vardenafil.

      CONCLUSIONS: There is a potential usefulness of vardenafil as a promising line of therapy in PE.
