DESCRIPTION
Anxiety disorder is a far more common problem than was once thought. It can affect people in their teenage years through middle age and later. More than 65 million Americans suffer annually from the worry, panic, stress, exaggerated concern and chronic insomnia characteristic of anxiety. For many, these feelings are triggered by high-pressure jobs, relationships, financial problems, loneliness, crowds, traffic jams, travel, high places, closed in places and a myriad of other motivations. Anxiety and its related disorders appears to affect twice as many women as men, though there may not actually be that wide a disparity between the sexes; it may effect the numbers of men and women equally. Psychologists believe that men are far less prone to report or even acknowledge having a problem with anxiety, but women are much more apt to seek help than men. Anxiety disorder can be either acute or chronic.
Acute anxiety disorder can manifests itself in episodes commonly known as panic attacks. Anxiety is an unsettling, fearful feeling about something that is going to happen or that a person thinks is going to happen. Anxiety disorders are a group of mental illnesses that result in severe anxiety that interferes with a sufferer's day-to-day life. Worry, panic, stress, exaggerated concern and chronic insomnia are characteristics of anxiety. A panic attack is an instance in which the body's natural "fight or flight" reaction occurs at the wrong time. This is a complex, involuntary physiological response in which the body prepares itself to deal with an emergency situation. Anxiety causes the body to produce more adrenal hormones, especially adrenaline. The increased production of adrenaline causes the body to step up its metabolism of proteins, fats, and carbohydrates to quickly produce energy for the body to use. In addition, the muscles tense, and heartbeat and breathing becomes more rapid. Even the composition of the blood changes slightly to make it more prone to clotting.

In the face of a threat such as an assault, an accident, or a natural disaster, this type of anxiety reaction is perfectly normal and helpful for survival. A certain amount of anxiety is normal and helps improve performance and allows people to avoid dangerous situations. At other times, the symptoms caused by a surge in adrenaline can be distressing and frightening. Panic attacks are usually abrupt and intense. They can occur at any time of the day or night, and can last from a few seconds up to half an hour. To the panic sufferer, it often feels as though they are much longer. For some it may be a one time episode; other persons become constantly anxious about everything. A person having a panic attack often believes that he or she is experiencing a heart attack or stroke. The attacks themselves are very unpredictable; some people experience one every few weeks, while others may have several a day. They are often triggered by stress (conscious or unconscious) or certain emotions, but may also occur in response to certain foods, drugs, or illness. Food allergies and hypoglycemia are both common among people with this disorder, and can promote panic attacks. An attack may follow ingestion or overindulgence in caffeine-based stimulants such as tea or coffee. Some attacks occur with no apparent cause. the unpredictability of the attacks makes them even more distressing.

Several types of anxiety are recognized including acute situational anxiety (which is usually short-term), chronic generalized anxiety disorder, and adjustment disorder with anxious mood.
Many people with acute anxiety disorder become fearful of being alone and of visiting public places because they fear having a panic attack. Of course, this only adds to the level of anxiety and leads to their lives being abnormally restricted. Many psychologists believe that at least in some cases, panic attacks are self-induced; that is, the fear of panic attack is the very thing that brings one about.
Chronic anxiety is a milder, more generalized form of anxiety. It is defined as unrealistic or excessive anxiety for 6 months or longer. Many people feel a vague sense of anxiety much of the time, but the intensity of the feeling does not reach the levels of those in an actual panic attack. They may feel chronically uneasy, especially in the presence of other people, and tend to startle easily. Headaches and chronic fatigue are common among people with this form of the disorder. Generalized anxiety disorder can begin at any age, but the onset typically occurs in the person's twenties or thirties. Some people with chronic anxiety disorder also suffer from occasional panic attacks.
Anxiety disorders can be acute or chronic and include the following conditions:
- Generalized Anxiety Disorder (GAD): A condition that causes a person to experience frequent, persistent worry over a period of at least six months. GAD is characterized by restlessness, irritability, muscle tension, fatigue, sleep disturbance and/or concentration difficulties.
- Panic Attack / Anxiety Attack: A sudden experience of apprehension or terror accompanied by a fear of going crazy, losing control or impending doom. Sufferers experience heart palpitations, shortness of breath, choking, smothering sensations, chest pain and/or light-headedness.
- Phobias: Persistent, unreasonable fears triggered by intense, unfounded and enduring concerns of a specific object or activity.
- Obsessive Compulsive Disorder (OCD): A type of anxiety disorder characterized by distressing, repetitive thoughts, images or impulses that are intense, absurd, unusual or frightening.
- Post Traumatic Stress Disorder: A type of anxiety disorder that is triggered by a very traumatic or life-altering event.
- Hypochondriasis: A medical condition wherein a person believes his real or imagined symptoms are signs of a serious disease, even though medical diagnosis proves this is not the case.
- Dysmorphophobia: A type of anxiety disorder that results in the persistent fear of being ugly, causing the person to become overly fixated on physical appearance.
For years, panic attacks were dismissed as a psychosomatic phenomenon. However, repeated studies have shown that this disorder has a real, physical basis. Many experts believe that panic attacks are caused principally by a malfunction in brain chemistry, wherein the brain sends and receives false "emergency signals". Hyperactivity in certain areas of the brain causes the release of norepinephrine, which causes the pulse, blood pressure, and breathing to become more rapid - the classic symptoms of panic attack.
Anxiety disorder may be hereditary to some extent, as it seems to run in families. Some cases may be linked to a relatively harmless abnormality of heart function called mitral valve prolapse. Anxiety disorder manifests itself in different ways, but health care providers agree that conflict, whether internal or interpersonal, promotes a state of anxiety. Anxiety is the most common mental health problem in the U.S.
FREQUENT SIGNS & SYMPTOMS
The symptoms of an anxiety or panic attack come on suddenly, with little to no warning, and last from 10 minutes to 30 minutes or more. These attacks are unpredictable and can occur from once every two weeks to several times a day.

Feeling that something undesirable or harmful is about to happen. A person having a panic attack experiences a vague, uncomfortable feeling of fear, dread or danger from an unknown source, and may be overwhelmed by a sense of impending disaster or death.
Dry mouth; swallowing difficulty or hoarseness.
Rapid breathing and heartbeat. Other feelings that can accompany a panic attack include shortness of breath, smothering, claustrophobic sensation, heart palpitations, and chest pain.
Hot flashes and/or chills.

CAUSES
The exact cause of anxiety and anxiety disorder is not known, but experts believe it may result from a chemical imbalance in the brain that affects how the brain transmits information. Anxiety disorder can be passed from parent to child; a child of someone with anxiety disorder, depression or bipolar disorder is eight times more likely to develop anxiety-related problems than a child with no family history of the disease. Anxiety can also be tied to stressors, such as the sudden loss of a relationship, being fired or other traumatic event. Overeating, alcohol abuse, pre-menstrual syndrome, irritable bowel syndrome may contribute to anxiety and panic attacks.
A panic attack results in the activation of the body's defense mechanisms for "fight or flight" response. Excess adrenaline is discharged from the adrenal glands, and adrenaline breakdown products (catecholamines) eventually affect various parts of the body. There is no way to prevent the development of anxiety and panic disorders, but there are methods to reduce the frequency of attacks.

Consider lifestyle changes to reduce stress. This includes avoiding alcohol and caffeine, getting regular exercise, eating a healthy diet and establishing a good, trusting relationship with a medical professional who specializes in anxiety problems.

Some laboratory studies may be done to rule out medical conditions that produce anxiety, such as hyperthyroidism. Laboratory tests are usually normal.
People with anxiety disorder, especially those who experience acute attacks, often seek medical assistance in hospital emergency rooms, only to be told they are just suffering from stress and that everything will be fine with rest. In one study, up to 70 percent of people who had panic attacks were found to have seen ten or more different health care providers before being correctly diagnosed.
Obtain therapy to understand the specific but unconscious threat or source of stress.
Learn techniques, including biofeedback and relaxation therapy, to reduce muscle tension. Music can be effective in reducing anxiety. Color also can be used to induce relaxation and calm.
Follow a regular, energetic fitness routine using aerobic exercise. Any type of regular exercise will work - brisk walking, bicycle riding, swimming, or whatever fits your individual lifestyle. After a few weeks of regular exercise, most people notice an improvement in anxiety symptoms.
Be sure to get adequate rest. If sleep is a problem, consult information about insomnia for suggestions.
To help manage an acute attack, use breathing techniques. Inhale slowly to a count of four, hold your breath for a count of four, exhale slowly to a count of four, and then do nothing for a count of four. Repeat this sequence until the attack subsides. Remind yourself that panic attacks last for a limited amount of time, and that the attack will pass.
Call a trusted friend or family member. Talking things over can diffuse anxiety.

MEDICATION
Anti-anxiety drugs may be prescribed for a short-term basis. Antidepressants may be prescribed for panic disorders. A number of different drugs are used to block panic attacks. Their use must be carefully monitored by a health care provider. The effectiveness of any given drug varies from individual to individual, and all drugs used for this disorder can cause unpleasant side effects. Alprazolan (Xanax), one of the drugs most commonly used for this illness, has varying effectiveness and can cause drowsiness and light headedness. It can also be very addictive. The risk of dependence and its severity seem to be higher if this drug is taken in relatively high doses (more than 4 milligrams per day) and for more than eight weeks.
