DESCRIPTION

Anovulation is a cycle in which a woman does not ovulate, or does not release an egg(s). Some women have many, or all, cycles in which they do not ovulate. Most of the time there is no menstrual bleeding, or if there is bleeding, it is because of the low estrogen and is called breakthrough bleeding. Many times a woman will see this bleeding and assume that she is ovulating.

There can be many reasons why a woman does not ovulate. And most of the time, a health care provider can prescribe drugs that will bring on ovulation. A woman who does have some bleeding once a month would never know she was anovulatory unless she was charting her basal body temperatures. A chart would look very erratic - with the temperature jumping all over the place - with no upward shift to identify ovulation.

CAUSES

Breastfeeding.

Pre-menopause (peri-menopause) and menopause.

Hormone therapy - such as, coming off oral contraceptives pill or other hormones such as Depo-Provera. Hormone imbalances are the most probable cause of anovulatory cycle. The long-term use of certain medications. Steroidal oral contraceptives (the Pill) are sometimes responsible. These drugs work by intentionally disrupting the hypothalamic-pituitary-ovarian axis, suppressing ovulation and thereby preventing pregnancy. For women using long-acting injectable steroid contraceptives (Depo-Provera), it appears likely that the longer the contraceptive is continued the more likely it is that amenorrhea will result.

Polycystic ovarian syndrome.

Illnesses - such as hypothalamic dysfunction, hyperprolactinemia, luteal phase defects, or tumors of the pituitary gland adrenal gland or ovaries. Other causes of anovulatory cycles are primary ovarian failure, resistant ovary syndrome and autoimmune oophoritis.

Stress and anxiety. Anxiety and other forms of emotional stress can also take their toll on normal ovulation.

Travel.

Strenuous exercise. A prolonged strenuous program of exercise, such as running, can interfere with the ovulatory cycle by suppressing the output of hormones called gonadotropins from the hypothalamus in the brain.

Excessive weight gain or loss. Eating disorders interfere with the ovulation cycle.

Infertility caused by chemotherapy and radiation therapy used as a cancer treatment. The overwhelming majority of women who receive total body irradiation will be sterile; however, some pre-pubertal and adolescent females do recover ovulation and menstruation. In patients who receive chemotherapy-only preparative regimens, the incidence of sterility is more variable and more age related, i.e., the older the woman is at the time of treatment the more likely chemotherapy will produce anovulation. These are important considerations because of the need for hormone replacement. All females should have frequent gynecologic follow-up. The overwhelming majority of men who receive total body irradiation will become sterile. Sterility is much more variable after chemotherapy-only regimens. Men should have sperm counts performed over time to determine whether or not sperm are present.

Once you know the probable causes of anovulation, you must take care to avoid any of these problems, especially if you have a history of fertility problems. Avoid any strenuous exercise without consultation and do not attempt to try out fad diets as these may lead to anovulatory cycles. Maintain a healthy nutritious diet and learn to manage stress and develop a healthy lifestyle to keep this disorder at bay.

TREATMENT

Anovulation can be difficult to detect. Some women have seemingly normal menstrual periods even though they are not ovulating. Most often, women who do not ovulate also do not menstruate, a disorder known as amenorrhea, or do not menstruate regularly, a condition called oligomenorrhea. Because of this, scant, erratic, short and/or painless menstrual cycles can sometimes alert a woman or her doctor about an anovulation problem.

If you experience bleeding between periods for more than 2-3 cycles, you should notify your health care provider immediately. Too many of these anovulatory cycles can contribute to irregular bleeding, or endometrial hyperplasia. A diagnostic test for anovulatory cycles may include the following depending on other factors like your age and medical history.

DIAGNOSTIC TESTS

• Serum assays.
• LH (on days 13 and 15 of menstrual cycle, to detect midcycle peak).
• FSH.
• Progesterone.
• Prolactin.
• DHEA-SO4.
• Testosterone and SHBG (for obtaining Free Androgen Index [FAI] or calculated free testosterone levels)
• CT scan or MRI scan of pituitary and hypothalamus.
• Endometrial biopsy.
• Ovarian biopsy.
• Specific antibody tests.

TREATMENT FOR ANOVULATORY CYCLES

Treatments for anovulatory cycles vary based on the underlying cause of the condition and other factors like age and medical history. For many infertile women with anovulation, treatment with one or another of fertility drugs can be remarkably successful. Clomiphene citrate (Clomid) is often a good first choice for an anovulatory woman who is producing estrogen.

If clomiphene alone is unsuccessful, Pergonal is added to bolster the attempts to ripen a follicle. Pergonal bypasses the natural hormone stimulation of the pituitary on the ovary. It applies stimulation directly to the ovary, and then, once a follicle grows to sufficient size, HCG is used as the final step to release the egg.

Some other treatments that can used in tandem with medication are:

• Medications for specific underlying conditions.
• Nutritional modifications.
• Stress reduction.
• Surgery (in the case of tumors).

MEDICATION

If you are trying to conceive, anovulation can be frustrating. Treatment is possible, if you choose to remedy the situation. If you suspect anovulation, you need to contact your health care provider for a series of tests to check hormone levels. Usually fertility drugs are prescribed which bring on ovulation. The most widely prescribed drugs are Clomid or Pergonal. These are usually prescribed in small doses to give the body a chance to ovulate, and then the dosage is gradually increased until ovulation occurs. Some women will ovulate only with the help of these drugs. Of the two, Clomid is the least invasive and is usually prescribed when the root of infertility cannot be determined.

If there is no menstrual bleeding, a drug called Provera, or a similar drug, is given to induce menstruation. Once ovulation has been established your health care provider can better determine if additional treatment is needed.

Women are happy that in this day and age anovulation is not a reason for infertility. With the advances in technology, medicine, and the deeper understanding of natural alternative therapies there are choices available that can be utilized to help nature along.
