ALOPECIA AREATA (HAIR LOSS)
Alopecia Totalis; Alopecia Universalis; Androgenetic Alopecia (AGA)

BASIC INFORMATION

DESCRIPTION

hair HAIR LIFE CYCLE: Each hair follicle (the small cavity through which hair grows) is always somewhere in its ongoing life cycle that's characterized by three distinct stages: growth, rest and loss. The first stage, in which hair is grown, is called the anagen phase. This typically lasts two to five years. Next, the follicle goes into a transitional resting stage (catagen) for several days, and finally, in the telogen phase, which lasts three months, hair is shed -- normally about 50 to 100 hairs a day. The cycle starts all over again when the follicles begin to sprout new hair. Thinning and baldness occur when follicles do not replace shed hair. Alopecia is baldness or loss of hair.

The accepted belief among health care providers is that most hair loss is due to a combination of genetics and hormonal changes, such as menopause. A less common form of hair loss is alopecia areata, believed to be an immune response gone awry in which hair falls out in patches that may not grow back.

TYPES OF ALOPECIA:

  • Alopecia totalis means the loss of all the scalp hair.

  • Alopecia universalis means loss of all body hair, including eyebrows and eyelashes.

  • Alopecia areata is the sudden hair loss in circular patches on the scalp. The hair loss is not accompanied by other visible evidence of scalp disease. It can involve hair on the scalp, eyebrows, eyelashes, genital area and underarms (sometimes). Factors that are involved in hair loss include heredity, hormones, and aging. Researchers have yet to determine the exact cause of hair loss, but some scientists believe the body's immune system mistakes hair follicles for foreign tissue and attacks them. Many suspect a genetic component.

  • Androgenetic alopecia (AGA) is a less dramatic but more prevalent type of hair loss. It is often referred to as male pattern baldness. AGA is common in men. As the name implies, a genetic or hereditary predisposition to the disorder and the presence of androgens (male sex hormones) are involved in this condition. Research indicates that the hair follicles of individuals susceptible to AGA may have receptors programmed to slow down or shut off hair production under the influence of androgens.

  • Women sometimes have the same type of hair loss, but it is not usually as extensive and most often does not occur until after menopause. All women experience some hair thinning as they grow older, especially after menopause, but in some cases it begins as early as puberty. In addition, most women lose some hair two or three months after having a baby because hormonal changes prevent normal hair loss during pregnancy.

    FREQUENT SIGNS & SYMPTOMS

  • Sudden hair loss in sharply defined circular patches. In rare cases, body hair loss may be total (alopecia universalis).

  • No pain.

  • No itch.

  • CAUSES

  • Unknown, but heredity and emotional factors, such as stress and anxiety, may contribute to hair loss. The autoimmune system (misdirected immune response in which the body's defenses become self destructive) may also be involved.

  • Occasionally due to thyroiditis or pernicious anemia. An imbalance is the thyroid, such as found in hypothyroidism or an imbalance in the adrenal glands is often a cause of hair loss.

  • Hormonal changes such as those seen during menopause. A healthy balance of male and female sex hormones is integral to hair growth, but it's the male hormone testosterone (produced by both men and women) that is associated with hair loss. The hair follicle responds to hormonal levels. Testosterone seems to cause hair loss on the scalp while encouraging hair growth in the pubic and underarm areas. At the onset of menopause, when the body makes fewer female hormones to counter the body's existing levels of testosterone, many women experience hair loss. Supplemental estrogen can help correct the imbalance and promote hair growth.

  • PREVENTIVE MEASURES

  • Cannot be prevented at present.

  • The strands you do have should be handled with care. Avoid shampoos with detergents such as sodium lauryl sulfate, ammonium laureth sulfate and cocamide DEA, which dry the scalp and hair. Instead, choose products with gentler cleansers, such as castile (made from olive oil) and decyl polyglucose (derived from corn). Other good ingredients to look for in products include the nourishing botanicals jojoba, rosemary, nettle, horsetail, sage and chamomile.

To keep from doing further damage, opt for an easy-to-maintain hairstyle (but no ponytails), limit shampooing to two or three times a week, blot hair dry with a super-absorbent towel, detangle with a wide-tooth wooden comb and avoid blow-drying. If you need to use styling products, go for sprays or gels with natural ingredients, such as flaxseed or guar gum, instead of with high isopropyl and ethyl alcohol contents. Consider installing a shower filter to reduce chlorine, which dries the hair and scalp. And definitely forgo curling irons, hot rollers, dyes and perms.

In the meantime, rather than obsessing about getting your hair to grow back, maintain a healthy lifestyle and keep your spirits up. That's the strategy that many hair loss sufferers have adopted, though it can be challenging. With hair loss, you must be patient. Hair doesn't grow fast to start with, so when you're correcting a problem, you have to realize it will take months, or maybe years, to correct the problem.

    Recommended Shampoos and Conditioners:
    Recommended Styling Products:

    EXPECTED OUTCOME

  • Usually curable with spontaneous new growth, in 18 months to three years. Persons with a few small patches are generally cured completely. The disorder recurs in 25% of cases.

  • POSSIBLE COMPLICATIONS

  • Loss of all hair.

  • Slow or incomplete regrowth.

  • TREATMENT

    GENERAL MEASURES

  • It is normal to lose 50 to 100 hairs a day.

  • Taking large doses of Vitamin A (100,000 IU or more daily) for a long period of time can trigger hair loss, but stopping the vitamin A will often reverse the problem. Often the hair grows back when the cause is corrected.

  • Consider wearing a hair piece or wig during the acute phase.

  • In a simple, self-limited case where the alopecia is not noticeable, no treatment may be necessary.

  • Continue to bathe and shampoo as usual. The disorder is not contagious.

  • Don't tug on normal hair close to area of hair loss. Avoid rough treatment. Do not use a brush or fine toothed comb, or towel-dry your hair. Also, do not use a blow dryer or other heated appliances on your hair; let it dry naturally. Do not comb your hair until it is dry, as wet hair tends to break off. Use a pick to put wet hair in place. If you have long hair, start combing it from the ends and work up. if you start to detangle by combing straight from the crown down you stand to cause more breakage to your hair. if you have REALLY long hair, do yourself a favor, comb your conditioner thru your hair while in the shower, starting from the ends up. rinse, then when you get out of the shower do your hair turban backwards or you loose all that detangling you did in the first place! Get your hair trimmed every 4-6 weeks to keep the ends healthy.

  • Lie head down on a slant board fifteen minutes a day to allow the blood to reach your scalp. Massage your scalp daily to improve blood circulation.

  • Be careful of using products that are not natural on the hair. Allergic reactions to chemicals in these products occur frequently. Alternate among several different hair products, using only all-natural and pH-balanced formulas.

  • Additional information available from:

    MEDICATION

  • Topical steroids may be prescribed. Apply topical steroid once or twice a day unless directed otherwise. Apply immediately after bathing or shampooing for better spreading and penetration. For scalp and groin, use only low-potency steroid products without fluorine. In special cases, you may have injections of steroids into affected areas and be prescribed oral cortisone drugs to take on alternate days.

  • Injections of triamcinolone into the scalp may be used for some patients.

  • Photohemotherapy with PUVA may be recommended. This treatment combines the use of a drug that sensitizes the skin along with a controlled dose of ultraviolet light.

  • Rogaine, a topical solution developed by the Upjohn Company and containing 2 percent minoxidil, has been approved by the FDA for the treatment of male pattern baldness. This product is now available over the counter in drugstores. However, this drug may cause heart changes if used for a long period of time, researchers at the University of Toronto report. Also, although using minoxidil does result in hair growth, the quality of hair is usually poor and hair growth ceases when the use is discontinued.

  • HERBAL & HOLISTIC RECOMMENDATIONS

    HERBAL THERAPY:

Herbal remedies increase circulation, disinfect the scalp and stimulate hair growth.

  • Use apple cider vinegar and sage tea as a rinse to help hair grow. Catmint or chamomile infusions used as a final rinse can help with itchiness, a source of hair loss. Ginger can either taken as a tea or applied to the scalp.

  • Licorice extract and may help prevent hair loss. It can be taken orally and/or used topically. Licorice can increase blood pressure, so if you have high blood pressure be sure to consult with your health care provider before using licorice internally.

  • Horsetail is a good source of silica, necessary for strong, sleek, and shiny hair. Silica is a component in healthy hair. To shampoo hair, steep 2 tablespoons horsetail in 4 ounces of hot water and add to a baby shampoo.

  • Kombucha tea has been reported by some to help halt hair loss.

  • Green tea (Camellia sinensis) may be a safe remedy for women to take for hair loss. Green tea is simple the dried leaves of the tea plant. Black tea are dried tea plant leaves that have been through a fermentation process. The key ingredients of green tea are molecules called catechins and have a variety of actions including vasorelaxation (arteries relaxing) which has aided cardiovascular activity. Catechins of green tea are selectively bactericidal. They do not affect lactic acid bacteria but will reduce the proliferation of other bacteria types particularly those that use alpha-amylase activity in their growth and cell division. Green tea also contains antioxidants and may help to lower cholesterol. Of greatest interest to the research community green tea apparently has a protective effect against a range of cancers, including the reduction in the frequency of smoke-induced mutations and is particularly useful in protection from stomach and colon cancer. Some studies have shown green tea may influence blood concentrations of hormones linked to at least one form of genetic hair loss known as androgenic alopecia. In one study published in the journal Nutrition and Cancer in 1998, Japanese researchers found that drinking green tea could increase levels of the sex hormone binding globulin (SHBG) in women. Since this biochemical binds testosterone, the more SHBG present in a woman's body, at least theoretically, the less testosterone will be in her bloodstream. And that, say some experts, may block the chain of hormonal activity that initiates some forms of hair loss. Since green tea is certainly a good source of healthful antioxidants and may have some anti-cancer properties, so it certainly would not hurt a woman to include it in her diet. One cup of green tea roughly equals 100 mg of tea catechins. Typically research investigations have involved individuals drinking 3 cups of green tea (300 mg catechins) each day. Alternatively, capsules of green tea are widely available. Most catechins when ingested in green tea are passed through the gut and excreted intact. Very little is actually absorbed into the blood stream. Research indicates that ingestion of 3 grams of green tea solids will yield plasma levels of catechins in the nanograms per milliliter of plasma range which is very low. The researchers also noted that increasing green tea intake to 4.5 grams did not significantly increase the amount absorbed into the blood plasma so there seems to be an upper limit to the amount of catechins that can be absorbed and carried in the blood stream. Taking larger amounts of green tea will not necessarily equate to an increased level of blood plasma catechins or improved treatment effect! Green tea is well tolerated by most individuals with no significant side effects reported.

  • Saw Palmetto is a small shrub sometimes known as the dwarf American palm, Sabal serrulata, or Serenoa repens. This plant can be found in its natural habitat of the South Western United States. It is among the herbs making medical headlines. Saw palmetto has been under investigation for many years. The saw palmetto extract is known as the drug "Permixon" in medical literature. Permixon is a saw palmetto berry extract. The key important ingredient is an n-hexane lipidosterolic agent. This might also be described as a phytosterol (plant sterol). Saw palmetto is shown in clinical trials to have some impact on benign prostatic hyperplasia -- or enlarged prostate. This is prostate enlargement not due to cancerous cells. The prostate is particularly sensitive and responsive to estrogen and testosterone hormone concentrations. To treat human prostatic hyperplasia, a oral dose of 160 mg saw palmetto extract is given twice a day and has shown significant improvement for individuals with prostatic hyperplasia problems. Because this condition has been linked to an increase in the same hormones related to hair loss (androgens, including testosterone) at least some experts believe that saw palmetto may work much like the hair loss drug finasteride (Propecia or Proscar), which was originally developed to treat prostate enlargement. While there are no clinical trials attesting to saw palmetto's usefulness as a treatment for hair loss, there is at least some reliable research showing that it can stop or at least reduce androgen activity within the hair follicle. Whether or not this can stop hair loss in women is completely unknown. If the herb is strong enough to effect androgen pathways, then women should not be taking it without their health care provider's approval. This is particularly important if they are using any form of female hormones, including HRT (menopausal hormone replacement therapy), birth control pills, or even phytoestrogens (such as soy). Adverse events reported when taking saw palmetto are largely gastrointestinal such as nausea and abdominal pain. These side effects are most likely to occur when taking saw palmetto without food. If taken after a meal, the side effects were greatly reduced. Saw palmetto is well tolerated by the majority of people.

  • Radix urticae (stinging nettle root) is another plant investigated for treating benign prostate hyperplasia and has been used as an alternative treatment for androgenic alopecia.

  • Pumpkin seed extracted oil has also been suggested to affect the activity of testosterone in the body in similar fashion to saw palmetto. Pumpkin seed oil has been shown to contain antioxidants and has been used to reduce inflammation in disease.

  • Aloe, nettles, stinging nettle, birch leaves and watercress are excellent blood and skin cleansers. Drink 1 tablespoon of juice of stinging nettle, birch leaves and watercress, mixed in a glass of water to cleanse the blood.

  • Parsley used as a tea strengthens the kidneys. A tea made from raspberry and blackberry leaves is also good for the kidneys.

  • Massage the scalp regularly with tinctures of stinging nettle, birch, horsetail and rosemary for hair growth stimulation, increase blood circulation, and to disinfect the scalp.

  • Rosemary or stinging nettle may help combat dandruff, a source of hair loss. Put a large handful of rosemary into a pan and cover with water. Simmer for 10 to 15 minutes and leave to cool. Strain the mixture through a filter or a sieve into a jug containing a few drops of wheatgerm or any other type of natural oil. Use this rinse after you have washed your hair with a very mild shampoo. Finally rinse hair thoroughly with warm (not hot) water.

  • Marshmallow and burdock tea is good for dry hair. To make the tea, add 1 teaspoon of marshmallow and 1 teaspoon of burdock to a cup of hot water. Use as a rinse and/or massage into the scalp.

  • Arnica, marigold, rosemary, or southernwood are also useful for hair loss. For best results, massage an infused oil into the scalp.

  • Make a tea of rosemary and spritz on after a perm. Removes the "permy" smell. Use cool water after washing and conditioning hair. It will lay down the cuticle. Rosemary will stain gray. it's extremely difficult to put color molecules back into the cortex (middle layer) of the hair, without the use of peroxide to open said layer. Gray or white hair is wiry and course. This is because the cortex layer is full of air instead of color molecules. the rosemary actually will stain and coat the cuticle layer of the hair.

  • An Ayurvedic Antistress Tea can be made by mixing equal portions of brahmi and jatamamsi. Steep 1 teaspoon of this mixture in 1 cup of hot water. Drink 2 or 3 times a day to relieve stress, which is a cause of hair loss.

  • TRADITIONAL CHINESE MEDICINE (TCM):

Oriental medicine practitioners believe that the condition of the hair is a direct reflection of the blood, the organs responsible for cleansing the blood (especially the kidneys), the sex organs, and the adrenal glands. The condition of the hair follicles are directly affected by the toxicity in the blood. The hair follicles are responsible for the nourishment of the hair and they also support the growth of hair.

Stress can tax the adrenal glands significantly and can cause hair loss. Some oriental health care practitioners suggest that hair that is frizzy and has split ends reflects poor condition of the sex organs, as well.

So, the first step in treating hair loss is to improve the quality of the blood. This can be done by strengthening the kidneys. Excessive amounts of animal protein, and sweet foods can cause the hair at the top of the head to fall out. According TCM, the sweets, acidic blood, and oils create excessive energy (excess fire); this excess energy escapes from the body through the top of the head. This causes the hair follicles to expand. As a result, the follicles lose their grip on the hair and allow them to fall out.

The solution is to draw the energy back into the body, especially to the body's vital center, known as hara. This restores vitality to the hair follicles and allows them to regain their ability to contract tightly around the roots of the hair, holding it in place.

Many foods, herbs, and certain physiotherapies cause the energy within the body to move downward into the vital center, or hara.

  • Psoralea seeds, eaten 3 to 9 grams daily. A study examining the effects of psoralea extracts and exposure to ultraviolet light in 45 bald men found that, within 6 months, hair was completely restored in 36 percent of the cases and there was a significant restoration in another 30 percent.

  • The Chinese use crushed oyster and clam shells in a decocted tea to increase kidney energy (qi).

  • Oriental formulae are Bupleurum and Dragon Bone Combination (effective for round alopecia and good for those of strong constitution and nervousness), Cinnamon and Dragon Bone Combination (for round alopecia and good for weak persons with vertigo), and Siler and Platycodon Formula (for delicate physique and alcoholics or the constipated). Traditional oriental tonics tone liver and kidneys and promote new hair growth. They include such herbs as: Polygonum, Lycium fruit, Chinese foxglove root, Chinese yam, and Cornus.

Beneficial Essential Oils for Hair Loss:
    • Bay
    • Cedarwood
    • Grapefruit
    • Jojoba oil
    • Lavender
    • Lemon
    • Rosemary
    • Roman Chamomile

How To Use Essential Oils for Hair Loss: Add 2 drops of the essential oil either to the rinse water or to a good vegetable oil. Massage it well into the scalp. Wrap the hair in plastic wrap; place a warm towel around it. Leave it for 2 to 3 hours, or overnight. Shampoo the hair as usual, using a mild shampoo (avoid medicated shampoo), so that you do not damage the sebum balance.

Essential Oils for Stimulating Circulation and Improving the Hair Health: A blend of bay and lavender essential oils stimulates the scalp and helps hair health. Add six drops of each oil to four ounces of a warm carrier oil such as almond, soybean or sesame oil. Massage the mixture into the scalp and allow it to absorb for 20 minutes. Add three drops of bay essential oil to the shampoo you regularly uses and wash the hair with this shampoo.

Essential Oil Blend for Hair Growth
      1. Thyme essential oil - 2 drops
      2. Atlas cedarwood essential oil - 2 drops
      3. Lavender essential oil - 3 drops
      4. Rosemary essential oil - 3 drops
      5. Jojoba oil - 1/2 teaspoon
      6. Grapeseed oil - 4 teaspoons
Add the first four ingredients into a small glass jar. Mix well. Now add the remaining two ingredients. Mix thoroughly. Massage this mixture into your scalp for at least two minutes every night. Wrap your head in a warm towel after treatment.

Dermatologists at Aberdeen Royal Infirmary in Scotland gave 43 people, suffering from a rare autoimmune disorder that causes patchy hair loss, the essential oil blend described above and asked them to rub it into their scalps daily. After seven months, 19 regrew as much as half of their hair, while a few regrew almost all of their hair back. The study was reported in the November/December 1999 issue of Health magazine. The researchers speculated that this remedy may also help other types of balding, including midlife thinning for men.

Oils of arnica, lavender, rosemary or calamus stimulate healthy hair growth. Rub into hair before washing.

Caution: Avoid rosemary oil if you have high blood pressure. Keep lemon and grapefruit doses low if you have sensitive skin, as they may be slightly irritating.
