Prolonged rupture of membranes (PROROM) is defined as rupture of the membranes (ROM) greater than 24 hours.
If diagnosed, the midwife should discuss potential risks and options of management.
The midwife may recommend doing a Beta Strep culture if not previously done.
Expectant management:
a. Monitor fetus for reactivity, do these tests:
Non-Stress Testing (NST)
Fetal Activity
Fetal Assessment and Fetal Assessment Chartb. Take maternal temperature and pulse every 2-4 hours.
c. Monitor fetal heart tones (FHT) as in normal labor.
d. Do CBC with diff. every 12 hours.
e. Minimize sterile vaginal exams.
The midwife may initiate and maintain phone contact with client's health care provider.
The midwife may prepare for hospital transfer at any signs of maternal or fetal distress or infection.
