The midwife should alert and/or consult with the client's health care provider back-up. She should notify and arrange for emergency transport, and prepare for transfer, if needed.
The mother should be informed of risks involved in a breech delivery at home. If the mother refuses transport after being informed about the pros and cons, a client signed "refusal of transport" should be added to her file.
Upon refusal to transport or if the birth is imminent, than proceed with the birth.
