DESCRIPTION
Sebaceous cysts (a form of trichilemmal cyst) are skin growths that contain a mixture of sebum (oil) and skin proteins, appearing as a white, semi-solid material. A sebaceous cyst is a closed sac (cyst) below the surface of the skin that has a lining that resembles the uppermost part (infundibulum) of a hair follicle. Sebum is produced by sebaceous glands of the epidermis. A whitehead is actually a tiny sebaceous cyst.
It is sometimes (but not always) considered to be equivalent to epidermoid cyst, or similar enough to be addressed as a single entity. Some sources state that a "sebaceous cyst" is defined not by the contents of the cyst (sebum) but by the origin (sebaceous glands). Because an "epidermoid cyst" originates in the epidermis, and a "pilar cyst" originates from hair follicles, neither type of cyst would be considered a sebaceous cyst by this definition. However, in practice, the terms are often used interchangeably. "True" sebaceous cysts are relatively rare.
SIGNS & SYMPTOMS
A sebaceous cyst generally appear as small, slowly growing swellings commonly on the face, scalp, back, ears, and upper arm, although they may occur anywhere on the body except the palms of the hands and the soles of the feet.
In males, a common place for them to develop is the scrotum and chest. They are more common in hairier areas, where in cases of long duration they could result in hair loss on the skin surface immediately above the cyst. They are smooth to the touch, vary in size, and are generally round in shape.
In females, small lumps or bumps that occur just under the skin of the vagina, genitalia, breast, abdomen, face, neck, or elsewhere on the body are the most common symptom of sebaceous cysts.
Occasionally infections may occur. Signs or symptoms that may indicate infection of sebaceous cysts include:
- Redness.
- Tenderness.
- Increased temperature of the skin over the bumps or lumps.
- Grayish white, cheesy, foul smelling material draining from the bump or lump.
These nodules feel firm but movable and rarely hurt unless they become infected. If infection sets in, there may be redness and swelling, and the area may become sensitive to the touch. Sebaceous cysts are benign (non-cancerous), but they can become a site of chronic, usually bacterial, infection. With chronic infection, an abscess may form.
The generally mobile masses consist of:
- Fibrous tissues and fluids.
- A fatty or keratinous substance, consisting of fibrous structural proteins, that resembles cottage cheese, in which case the cyst may be called a "keratin cyst".
- A somewhat viscous, serosanguineous fluid containing purulent (pus can appear as a grayish-white, whitish-yellow, yellow, or yellow-brown substance produced during inflammatory bacterial infections) and bloody material.
The nature of the contents of a sebaceous cyst, and of its surrounding capsule, will be determined by whether the cyst has ever been infected.
With surgery, a cyst can usually be excised in its entirety. Poor surgical technique or previous infection leading to scarring and tethering of the cyst to the surrounding tissue may lead to rupture during excision and removal. A completely removed cyst will not recur, though if the patient has a predisposition to cyst formation, further cysts may develop in the same general area.
CAUSES
The surface of your skin (epidermis) is made up of an extremely thin, protective layer of cells that your body continuously sheds. Most epidermoid cysts form when these surface cells, instead of exfoliating normally, move deeper into your skin and multiply. Most often, this occurs in areas where there are small hair follicles and larger sebaceous glands, such as your face, neck, upper back and groin.
The epidermal cells form the walls of the cyst, and then secrete the protein keratin into the interior. The keratin is the thick yellow substance that sometimes drains from the cyst.
Several factors can lead to this abnormal proliferation of cells, including:
- Damage to a hair follicle. Each hair grows from a follicle, a small pocket of modified skin in the dermis, the layer of skin just below the epidermis. Follicles damaged by injuries such as abrasions or surgical wounds can become blocked by surface cells.
- A ruptured sebaceous gland. Located just above the hair follicles, sebaceous glands produce sebum, the oil that lubricates your skin and coats each hair shaft. These glands are easily ruptured by inflammatory skin conditions, especially acne, making them a likely site for epidermoid cysts.
- Developmental defect. Epidermoid cysts can begin in a developing fetus when stem cells intended to form skin, hair or nails become trapped in cells forming other tissues.
- Heredity. Epidermoid cysts can develop in people with Gardner's syndrome, a rare genetic disorder that causes growths in the colon, or basal cell nevus syndrome, an inherited disease that leads to several serious defects.
Blocked sebaceous glands, swollen hair follicles, and excessive testosterone production will cause such cysts.
Skin trauma can also induce a cyst to form.
A case has been reported of sebaceous cyst being caused by dermatobia hominis. The genus Dermatobia contains only one species, D. hominis, the only species of botfly that attacks humans (in addition to a wide range of other animals, including other primates). It is also known as the torsalo or American warble fly, even though the Warble fly is in the genus Hypoderma and not Dermatobia and is a parasite on cattle and deer instead of humans. Dermatobia fly eggs are vectored by mosquitoes and muscoid flies; the female captures the mosquito and attaches its eggs to the body of it, then releases it. Either the eggs hatch while the mosquito is feeding and the larvae use the mosquito bite area as the entry point, or the eggs simply drop off the muscoid fly when it lands on the skin. The larvae develop inside the subcutaneous layers, and after approximately eight weeks, they drop out to pupate for at least a week, typically in the soil. The adults are small gray flies resembling a blowfly. This species is native to Mexico and Central America.
RISK FACTORS
Just about anyone can develop one or more epidermoid cysts, but these factors make you more susceptible:
- Age. Though they can occur at any age, you are most likely to develop an epidermoid cyst in your 30s or 40s. People with Gardner's syndrome develop them much earlier - around puberty.
- Your sex. Men are about twice as likely to have epidermoid cysts as women are.
- A history of acne. Epidermoid cysts are especially common in people who have had acne. Milia often occur in men and women with a long history of sun exposure.
- Skin injuries. Any traumatic or crushing injury to your skin - slamming your hand in a car door, for instance - increases your risk of an epidermoid cyst.
PREVENTION
There's no real way to stop epidermoid cysts from forming, but avoiding excessive sun exposure and using oil-free skin care products can help prevent milia. To prevent scarring and infection, do not try to squeeze cysts yourself.
CONVENTIONAL MEDICAL TREATMENT
DIAGNOSIS
Sebaceous cysts are usually easily diagnosed by their appearance to the trained eye. In some cases, a biopsy may be necessary to rule out other conditions with a similar appearance. You may be referred to a dermatologist for treatment.
TREATMENTS
Sebaceous cysts generally do not require medical treatment. Sebaceous cysts most often disappear on their own and are not dangerous. Occasionally, if they continue to grow, they may become unsightly, inflamed (infected) and tender (painful). Sometimes sebaceous cysts grow large that they may interfere with your everyday life. When this happens, surgical removal in your health care provider's office may be necessary. Small inflamed cysts can often be treated by injection of steroid medications or with antibiotics.
COMPLICATIONS
Sebaceous cysts may occasionally become infected and form into painful abscesses. It is important when sebaceous cysts are surgically removed that the entire sac is excised to help prevent a recurrence. However, it's important to note that sebaceous cyst recurrence in not unusual.
In rare cases, epidermoid cysts can give rise to basal and squamous cell skin cancers. Because this occurs so seldom, epidermoid cysts usually are not biopsied unless they are solid, immobile, infected or have other unusual characteristics that suggest a more serious problem. Besides cancer, other complications include:
- Inflammation. Epidermoid cysts can become tender and swollen, even if they are not infected. Inflamed cysts are difficult to remove, and your health care provider is likely to postpone treating them until the inflammation subsides.
- Rupture. A ruptured cyst often leads to a boil-like abscess that requires prompt treatment.
- Infection. An epidermoid cyst can become infected spontaneously or after a rupture.
- Genital discomfort. Genital epidermoid cysts can lead to painful intercourse and urination.
SURGICAL TREATMENT
Surgical excision of a sebaceous cyst is a simple procedure to completely remove the sac and its contents. These are the general approaches used:
- Traditional wide excision or total incision. This surgical technique removes the entire cyst and so prevents recurrence. The downside is that total incision requires sutures and is more likely to leave a scar than other treatments.
- Minimal excision. Some health care providers prefer this technique because it removes the whole cyst wall but causes minimal if any scarring. During the procedure, your health care provider makes a tiny incision in the cyst, expresses the contents, and then removes the cyst wall through the incision. The small wound is usually left to heal naturally.
- A punch biopsy excision is similar to a minimal excision. Punch biopsy excision appears to be superior to traditional wide elliptical excision for the treatment of sebaceous cysts when intervention is necessary (strength of recommendation is based on 1 small randomized study). They found punch biopsy to be less time-consuming and to offer superior cosmetic results. However, cysts larger than 2 cm took longer with the punch biopsy technique. No rigorous methodological studies have compared punch biopsy excision of sebaceous cysts with the minimal excision technique.
Minimal excision and punch biopsy techniques are purported to produce minimal bleeding, have faster healing times, and produce less scarring. (2) Though both techniques offer a shorter procedural time, they appear to have a slightly higher rates of recurrence.
- Lasers. To minimize scarring, your health care provider may use a carbon dioxide laser to vaporize an epidermal cyst on your face or other sensitive area.
What's The Best Treatment For Sebaceous Cysts?
TYPICAL SURGICAL PROCEDURE
The typical outpatient surgical procedure for cyst removal is to numb the area around the cyst with a local anaesthetic, then to use a scalpel to open the lesion with either a single cut down the center of the swelling, or an oval cut on both sides of the center-point. If the cyst is small, it may be lanced instead. The person performing the surgery will squeeze out the keratin (the semi-solid material consisting principally of sebum and dead skin cells) surrounding the cyst, then use blunt-headed scissors or another instrument to hold the incision wide open while using fingers or forceps to try to remove the cyst intact. If the cyst can be removed in one piece, the "cure rate" is 100%. If, however, it is fragmented and cannot be entirely recovered, the operator may use curettage (scraping) to remove the remaining exposed fragments, then burn them with an electro-cauterization tool, in an effort to destroy them in place. In such cases the cyst may or may not recur. In either case, the incision is then disinfected and, if necessary, the skin is stitched back together over it. A scar will most likely result. In some cases where "cure rate" is not 100% the resulting hole is filled with an antiseptic ribbon after washing it with an iodine based solution. This is then covered with a field dressing. The ribbon and the dressing are to be changed once or twice daily for 7-10 days after which the incision is sewed up.
An infected cyst may require oral antibiotics or other treatment before and/or after excision.
An approach involving incision, rather than excision, has also been proposed. In this procedure, your health care provider makes a small cut in the cyst and expresses the contents. Although incision and drainage is relatively quick and easy, cysts often recur after this treatment.
NON-SURGICAL TREATMENT
If you have a small inflamed cyst, your health care provider may inject it with a corticosteroid medicine that reduces swelling in an inflamed but uninfected cyst.
Another common and effective method of treatment involves placement of a warm, moist cloth (compress) or a heat-pad over the area directly on the cyst for about fifteen minutes, twice daily, for about 10 days (depending on size and location of the cyst). This can help the cyst drain and heal. There is some anecdotal evidence however that this home remedy can lead to infection in a previously uninfected cyst. This may be caused by an over-heated or non-sterile heat pad.
This method works by bringing the temperature of the wax-like material inside of the cyst to a temperature at which it melts, and can be reabsorbed and processed by the body, as a small amount of oily fluid. This method is preferred over surgery both for reasons of associated costs and risks of surgery. This methodology is not applicable for non-sebaceous cysts, however, as other varieties of cysts do not contain the same hardened sebum deposits, and therefore do not melt to be reabsorbed by the body.
Under no circumstances must one try to pop the cyst, as it can lead to infection of the surrounding tissue. The neck is a particularly dangerous region, due to the glands and blood vessels.

CONSIDERATIONS
Drugs often prescribed for acne, including topical and oral antibiotics such as tetracycline and erythromycin, and tretinoin (Retin-A), a derivative of vitamin A that comes in cream, gel, or liquid form, may be prescribed for severe and/or extensive infection. Another acne drug, isotretinoin (Accutane), also derived from vitamin A, has serious side effects.
If an antibiotic is prescribed, be sure to take an acidophilus supplement to replace necessary "friendly" bacteria. Do not take acidophilus at the same time of day as the antibiotic.
If you develop an inflamed or infected sebaceous cyst, your health care provider may suggest surgical drainage, a procedure that is performed in the practitioner's office.
NUTRITION RECOMMENDATIONS
Drink plenty of quality water to keep the skin hydrated and flush out toxins.
A nutrient-rich diet is essential for healthy skin. Eat a diet consisting of 50 to 75 percent raw vegetables and fresh fruits, in particular, leafy green vegetables, carrots and salads. Eat soured products such as yogurt.
Optimize your body and health by using nutritional supplements such as vitamins, minerals (including iron if you are anemic), essential fatty acids and other recommended nutrients.
Avoid fats in your diet. Consume no fried foods, animal fats, or heat-processed vegetable oils such as those sold in supermarkets. Do not cook with oil and do not eat any oils that have been subjected to heat, whether in processing or cooking. If a little oil is necessary, such as a salad dressing, use cold-pressed canola or extra virgin olive oil only.
Do not drink soft drinks, caffeine, cocoa, and alcoholic beverages. Avoid sugar, chocolate, dairy products, eggs, fish, meat, salt, and all junk or fast food.
Follow a fasting program once a month.
