DESCRIPTION
Pneumonia is a serious infection of one or both lungs that can be caused by any number of different infectious agents including viruses, bacteria, fungi, protozoa, and mycoplasma. The infection causes tiny air sacs in the lung area to become inflamed and filled with mucus and pus, inhibiting oxygen from reaching the blood.
Lobar pneumonia affects only a section, or lobe, of one lung.
Bronchial pneumonia affects portions of both lungs.
Prior to the discovery of antibiotics, one-third of all people who developed pneumonia subsequently died from the infection.
Young children (especially infants), older adults, and people with chronic illnesses or a compromised immune systems are very vulnerable to the potentially life-threatening effects of this illness. Currently in the U.S., over 3 million people develop pneumonia each year and it is now the fifth to sixth (depending on resources) leading cause of death in the United States with more than 60,000 Americans dying each year of pneumonia. Over a half a million of these people are admitted to a hospital for treatment. Although most of these people recover, approximately 5 percent will die from pneumonia. Worldwide, it is a leading cause of death in children.
There are many kinds of pneumonia ranging in seriousness from mild to life-threatening. Pneumonia acquired while in the hospital can be particularly virulent and deadly. Although the signs and symptoms vary, many cases of pneumonia develop suddenly with chest pain, fever, chills, cough and shortness of breath. Infection often follows a cold or the flu, but it can also be associated with other illnesses or occur on its own. No matter what the cause, pneumonia usually leaves the sufferer with weakness that persists for 4 to 8 weeks after the acute phase of the infection has resolved.
CAUSES
Your lungs are two spongy organs surrounded by a moist membrane (the pleura). Each lung is divided into lobes with three on the right and two on the left. When you inhale, air is carried through the windpipe (trachea) to your lungs. Inside your lungs, there are major airways called bronchi. The bronchi repeatedly subdivide into many smaller airways (bronchioles), which finally end in clusters of tiny air sacs called alveoli.
Your body has mechanisms to protect your lungs from infection. In fact, you are frequently exposed to bacteria and viruses that can cause pneumonia, but your body normally prevents most of these organisms from invading and overwhelming your airways. For example, the nasal cilia screen out a lot of organisms but can not stop all from getting into your airways. Sometimes these microorganisms can get past your body's defenses, finally finding their way into your lungs' air sacs.
There, white blood cells (leukocytes), a key part of your immune system, begin to attack the invading organisms. The accumulating pathogens, white cells and immune proteins cause the air sacs to become inflamed and filled with fluid, leading to the difficult breathing that characterizes many types of pneumonia. If both lungs are involved, it is called double pneumonia.
CLASSIFICATIONS OF PNEUMONIA
Pneumonia is sometimes classified according to the cause of pneumonia:
- Community-Acquired Pneumonia: This refers to pneumonia you acquire in the course of your daily life, such as at school, work or the gym, for instance.
- Hospital-Acquired (Nosocomial) Pneumonia: If you are hospitalized, you are at a higher risk of pneumonia, especially if you are on a mechanical ventilator, are in the intensive care unit or have a compromised immune system. This type of pneumonia can be extremely serious, especially for older adults, young children and people with chronic obstructive pulmonary diseases (COPD) or HIV/AIDS. A common predisposing factor for this type of pneumonia is gastroesophageal reflux. This occurs when some of the contents of your stomach back up into the upper esophagus. From there, the gastroesophageal contents can be aspirated into the trachea and then into your airways. Even very small amounts of gastroesophageal reflux can lead to pneumonia in people who are hospitalized.
- Aspiration Pneumonia: This type of pneumonia occurs when foreign matter is inhaled (aspirated) into your lungs. Most often this can occur when the contents of your stomach enter your lungs after you vomit. This commonly happens when a brain injury or other condition affects your normal gag reflex. Another common cause of aspiration pneumonia is consuming too much alcohol. This happens when the inebriated person passes out, and then vomits due to the effects of alcohol on the stomach. If someone is unconscious, it is possible to aspirate the liquid contents and possibly solid food from the stomach into the lungs, causing aspiration pneumonia.
- Pneumonia Caused By Opportunistic Organisms: This type of pneumonia strikes people with compromised immune systems. Organisms that are not harmful for healthy people can be extremely dangerous for people with AIDS, organ transplantation and other conditions that impair the immune system. For example, P. carinii pneumonia almost never occurs in otherwise healthy people. Medications that suppress your immune system, such as corticosteroids or chemotherapy also can put you at risk of opportunistic pneumonia.
- Emerging Pathogens: Outbreaks of the H5N1 influenza (bird flu) virus and severe acute respiratory syndrome (SARS) have caused serious, sometimes deadly pneumonia infections, even in otherwise healthy people.
Pneumonia is typically preceded by an upper respiratory infection such as a cold, influenza, or measles.
BACTERIAL PNEUMONIA
Bacterial pneumonia can be very dangerous and may come on either suddenly or gradually, usually as a complication of some other health problem such as respiratory disease, a weakened immune system, or viral infection. Older adults, young children, alcoholics, and people who have just undergone surgery are also at risk. Streptococcus pneumoniae is the most common cause of bacterial pneumonia. This type of pneumonia is unlikely to spread from one person to another.
Streptococcus pneumoniae infection usually an abrupt onset of the illness with shaking chills, fever, and production of a rust-colored sputum. The infection spreads into the blood in 20 to 30 percent of cases, and if this occurs, 20 to 30 percent of these patients die.
Although antibiotics can treat some of the most common forms of bacterial pneumonias, antibiotic-resistant strains are a growing problem. For that reason, and because the disease can be very serious, it is best to try to prevent infection in the first place.
VIRAL PNEUMONIA
Viral pneumonia is more variable in course and severity. It can come on suddenly or gradually and symptoms are similar to those of a bacterial pneumonia and can be mild, severe, or anywhere in between. It is less serious than bacterial pneumonia, but if not cared for properly, a second, bacterial pneumonia infection can set in.
FUNGAL PNEUMONIA
Fungal pneumonia, especially Pneumocystis carinii pneumonia (PCP), is much less common than either the bacterial or viral variety, and is often associated with a weakened or suppressed immune system. People with HIV, AIDS, or certain types of cancer, or who are taking immunosuppressive drugs following organ transplantation, are most likely to be affected.
MYCOPLASMA PNEUMONIA
Mycoplasma pneumonia, or "walking pneumonia", is caused by an agent that is unclassified but appears to be both bacterium and virus. This form of pneumonia usually affects people under 40.
CHILDHOOD PNEUMONIA
Infants can contract pneumonia due to a Chlamydia trachomatis infection transferred to the child during birth.
Childhood pneumonia can also be caused by the same bacteria that cause whooping cough.
SYMPTOMS
Most people who develop pneumonia initially have symptoms of a cold which becomes worse as it settles into the lungs or it mimics a cold or the flu so many people may not realize they have pneumonia, a more serious condition. In other cases of pneumonia there can be a slow onset of symptoms. A worsening cough, headaches, and muscle aches may be the only symptoms. In some people with pneumonia, coughing is not a major symptoms because the infection is located in areas of the lung away from the larger airways.
Children and babies who develop pneumonia often do not have any specific signs of a chest infection but develop a fever, appear quite ill, and can become lethargic. Elderly people may also have few symptoms with pneumonia.
Although pneumonia symptoms may vary in intensity, depending on any underlying conditions the person may have and the type of organism causing the infection, they usually include:
- Fever up to 104°F.
- Chills and shaking.
- Cough with sputum production.
- Bloody or discolored sputum.
- Muscle aches.
- Fatigue.
- Sore throat.
- Headaches.
- Enlarged lymph glands in the neck.
- Cyanosis (a bluish, dusky or purplish cast to the skin and nails) from poor oxygenation of the blood.
- Rapid, difficult respiration. Short of breath.
- Pain in the chest if the outer pleural aspects of the lung are involved. The only pain fibers in the lung are on the surface of the lung (the pleura). This pain is usually sharp and worsens when taking a deep breath, known as pleuritic pain.
BACTERIAL PNEUMONIA
Many types of bacteria can cause pneumonia. Bacterial pneumonia can occur on its own, at the same time as viral pneumonia, or it may develop after a viral upper respiratory infection such as influenza. Signs and symptoms of bacterial pneumonia are likely to come on suddenly and include:
- Shaking and chills.
- A high temperature (fever).
- Sweating.
- Initially the cough is may be dry.
- Then a thick, greenish or yellow phlegm or rust-colored sputum is produced.
- Breathing becomes rapid and labored, shortness of breath.
- Chest pain worsens upon inhalation.
- Abdominal pain and fatigue are also common.
Ironically, high-risk groups such as older adults and people with a chronic illness or compromised immune system may have fewer or milder symptoms than less vulnerable people do. And instead of the high fever that often characterizes pneumonia, older adults may even have a lower than normal temperature.
Bacterial pneumonia is often confined to just one area (lobe) of the lung. This is called lobar pneumonia.
VIRAL PNEUMONIA
About half of pneumonias are caused by viruses. Vital pneumonia tends to begin with flu-like signs and symptoms. It usually begins with a dry (non-productive) cough, headache fever, muscle pain and fatigue. as the disease progresses, a person may become breathless and develop a cough that produces just small quantities of phlegm that may be clear or white. Symptoms of viral pneumonia are similar to bacterial pneumonia but usually considered less serious unless the person does not take proper care of the infection and the risk of developing a more serious secondary bacterial pneumonia infection.
MYCOPLASMA PNEUMONIA
This tiny organism causes signs and symptoms of similar to other bacterial and viral infections. Mycoplasma pneumonia appear more gradually and are often mild (less severe than those of viral or bacterial pneumonia) and flu-like. The symptoms may include a cough that is spasmodic, along with chills and a fever. You may not be sick enough to stay in bed or seek medical care and may never even know you have had pneumonia. For this reason, this type of pneumonia is often called "walking pneumonia".
Mycoplasma pneumonia spreads easily in situations where people congregate and is common among school children and young adults. Mycoplasma pneumonia responds well to treatment with the appropriate antibiotics, although you may continue to have a dry, nagging cough and continue to feel weak during your convalescence.
FUNGAL PNEUMONIA
Certain types of fungus also can cause pneumonia, although these types of pneumonia are much less common. Most people experience few if any symptoms after inhaling these fungi, but some develop symptoms of acute pneumonia, and still others may develop a chronic pneumonia that persists for months.
PHEUMOCYSTIS CARNINII
Pneumonia caused by P. carinii (Pneumocystis carinii) is an opportunistic infection that affects people living with AIDS. People whose immune systems are compromised by organ transplants, chemotherapy, or treatment with corticosteroids or other immune-suppressing drugs such as tumor necrosis factor (TNF) inhibitors also are at risk. The signs and symptoms of Pneumocystis carinii pneumonia include a cough that does not go away, fever and shortness of breath.
INFECTION WITH THE PNEUMONIA ORGANISM
Some cases of pneumonia are contracted by breathing in small droplets that contain the organisms that can cause pneumonia. These droplets get into the air when a person infected with these germs coughs or sneezes. In other cases, pneumonia is caused when bacteria or viruses that are normally present in the mouth, throat, or nose inadvertently enter the lung. During sleep, it is quite common for people to aspirate secretions from the mouth, throat, or nose. Normally, the body's reflex response (coughing back up the secretions) and immune system will prevent the aspirated organisms from causing pneumonia. However, if a person is in a weakened condition from another illness, a severe pneumonia can develop. People with recent viral infections, lung disease, heart disease, and swallowing problems, as well as alcoholics, drug users, and those who have suffered a stroke or seizure are at higher risk for developing pneumonia than the general population.
Once organisms enter the lungs, they usually settle in the air sacs of the lung where they rapidly grow in number. This area of the lung then becomes filled with fluid and pus as the body attempts to fight off the infection.
RISK FACTORS
Factors that increase the risk of pneumonia include:
- Being either under 1 year of age or over 60-65 years of age.
- A weakened immune system or an immune system not fully developed as with very young children.
- Cardiovascular disease.
- Diabetes.
- HIV infection.
- Seizure or stroke.
- Aspiration under anesthesia.
- Alcoholism.
- Smoking.
- Kidney failure.
- Sickle cell anemia disease.
- Malnutrition.
- Foreign bodies in the respiratory passages.
- Exposure to chemical irritants.
- Allergies or chronic lung disease.
CHRONIC ILLNESS & IMMUNE DEFICIENCIES
Having certain diseases which may increase your risk include immune deficiency diseases such as HIV/AIDS and chronic illnesses such as cardiovascular disease, emphysema and diabetes. You are also at increased risk if you have had your spleen removed, or your immune system has been impaired by chemotherapy or long-term use of immunosuppressant drugs.
SMOKING, ALCOHOL ABUSE
Smoking or alcohol abuse increase risk of pneumonia. Millions of microscopic hairs (cilia) cover the surface of the cells lining your bronchial tubes. The hairs beat in a wave-like fashion to clear your airways of normal secretions, but irritants such as tobacco smoke paralyze the cilia, causing secretions to accumulate. If these secretions contain bacteria, they can develop into pneumonia. Alcohol interferes with your normal gag reflex as well as with the action of the white blood cells that fight infection.
HOSPITALIZATION IN ICU
If you are hospitalized in an intensive care unit you are at greater risk of developing pneumonia. Pneumonia acquired in the hospital tends to be more serious than other types of pneumonia. People who require mechanical ventilation are particularly at risk because the breathing tube (endotracheal tube) bypasses the normal defenses of the respiratory tract, prevents coughing, may allow the stomach's contents to back up into the esophagus where they can be aspirated, and can harbor bacteria and other harmful organisms.
SURGERY OR TRAUMATIC INJURY
People who have had surgery or who are immobilized from a traumatic injury have a higher risk of pneumonia because surgery or serious injuries may make coughing (which helps clear the lungs) more difficult and lying flat can allow mucous to collect in your lungs, providing a breeding ground for bacteria.
CHEMICAL & POLLUTANT EXPOSURE
Exposure to certain chemicals or pollutants increases your risk of developing some uncommon types of pneumonia. This may be increased if you work in agriculture, construction or around certain industrial chemicals or animals. Exposure to air pollution or toxic fumes can also contribute to lung inflammation, which makes it harder for the lungs to clear themselves.
PREVENTION
You usually do not "catch" pneumonia from someone else. Instead, you develop the disease because your immune system is temporarily weakened, often for no known reason. The following suggestions can help keep you healthy:
- Conventional medical approach is to recommend getting vaccinated. Because pneumonia can be a complication of the flu, getting a yearly flu shot may be a good way to prevent viral influenza pneumonia, which can lead to bacterial pneumonia. In addition, get a vaccination against pneumococcal pneumonia at least once after age 55, and if you have any risk factors, every five years thereafter. Your health care provider may recommend a pneumonia vaccine if you are a smoker, if you are younger but have a lung or cardiovascular disease, certain types of cancer, diabetes or sickle cell anemia, if your immune system is compromised or if you have had your spleen removed for any reason.
- A vaccine known as pneumococcal conjugate vaccine may help protect young children against pneumonia. It is recommended for all children younger than age 2 and for children 2 years and older who are at particular risk of pneumococcal disease, such as those with an immune system deficiency, cancer, cardiovascular disease or sickle cell anemia. Side effects of the pneumococcal vaccine are generally minor and include mild soreness or swelling at the injection site.
- Wash your hands often and thoroughly often can help reduce your risk, especially during winter months when colds and flu are most common. Your hands are in almost constant contact with germs that can cause pneumonia. These germs enter your body when you touch your eyes or rub the inside of your nose. Good hand washing techniques and hygiene is one of the best things you can do for yourself to minimize threat of being infected. If you are unable to wash hands, use an alcohol-based hand sanitizer, which can be more effective than soap and water in destroying the bacteria and viruses that cause disease. Most hand sanitizers contain ingredients that keep your skin moist. Carry disposable hand wipes in your purse or in your wallet to clean hands. These have been specially treated to be anti-viral and anti-bacterial.

- Protect others from infection. If you have pneumonia, try to stay away from anyone with a compromised immune system. When that isn't possible, you can help protect others by wearing a face mask and always coughing into a tissue. Avoid crowds and sick people. Avoid touching areas possibly contaminated with bacteria and viruses, such as public restrooms, door knobs, elevator buttons, public telephones, commuter trains and other public transportation methods and areas, etc. Reducing exposure risk helps to prevent illness.
- Keep your hands away from your nose, eyes, and mouth. bacteria and viruses are most likely to enter your body through these areas.
- Stop smoking. Smoking damages your lungs' natural defenses against respiratory infections. It irritates the lining of your nose, sinuses, and lungs, which may make you susceptible to complications of bacterial and/or viral infections.
TAKE CARE OR YOURSELF
- Proper rest and sufficient sleep helps keep your immune system strong.
- Eat a balanced diet with plenty of fruits, whole grains and vegetables. Build and maintain a strong immune system. (See Nutrition below for more information.)
- Get regular, moderate exercise and plenty of fresh, clean air to help keep your immune system strong.
- Reduce stress. Chronic stress and anxiety can weaken your immune system making you more susceptible to infections.
Things to remember:
- Antibiotics. Taking antibiotics will not treat viral infections or prevent complications associated with a viral infection. If a secondary bacterial infection develops after a viral infection, antibiotics may be helpful.
COMPLICATIONS OF PNEUMONIA
How serious pneumonia is for you depends on your overall health and the type and extent of pneumonia you have. If you are young and healthy, your pneumonia can usually be treated successfully. But if you have heart failure or lung ailments, especially from smoking, or if you are older, your pneumonia may be harder to cure. You are also more likely to develop complications, some of which can be life-threatening.
- Bacteria in the bloodstream. Pneumonia can turn deadly when inflammation from the disease fills the air sacs in your lungs and interferes with your ability to breathe. In some cases the infection may invade your bloodstream (bacteremia). It can then spread quickly to other organs. This is detected by doing cultures of your blood.
- Fluid accumulation and infection around the lungs. Sometimes fluid accumulates between the thin, transparent membrane (pleura) covering your lungs and the membrane that lines the inner surface of your chest wall. This is a condition known as pleural effusion. Normally, the pleurae are silky smooth, allowing your lungs to slide easily along your chest wall when you breathe in and out. But when the pleurae around your lungs become inflamed (pleurisy), often as a result of pneumonia, fluid can accumulate and may become infected (empyema). In that case, you may have a tube placed between your ribs to drain the fluid or, occasionally, a surgical procedure to clear out some of the infected material.
- Lung abscess. A cavity containing pus (abscess) that forms within the area affected by pneumonia is another potential complication. Abscesses usually are treated with antibiotics, but in rare cases they may need to be removed surgically.
If you have emphysema or pneumonia in both lungs (double pneumonia), you may need a mechanical respirator to help you breathe.
TREATMENT
DIAGNOSIS & HEALTH EXAM
Your health care provider may first suspect pneumonia based on your medical history and a physical exam.
LISTENING TO LUNG NOISES
During the exam, your health care provider will listen to your lungs with a stethoscope to check for course breathing and abnormal bubbling or crackling sounds (rales) and for rumblings (rhonchi) that signal the presence of thick liquid. There may be wheezing, or the sounds of breathing may be faint in a particular area of the chest.
CHEST X-RAY
A chest x-ray is usually ordered to confirm the diagnosis of pneumonia and to determine the extent and location of the infection. The lungs have several segments referred to as lobes, usually two on the left and three on the right. When the pneumonia affects one of these lobes it is often referred to as lobar pneumonia. Some pneumonias have a more patchy distribution that does not involve specific lobes. In the past, when both lungs where involved in the infection, the term "double pneumonia" was used. This term is rarely used today.
SPUTUM-PHLEGM SAMPLES
Sputum or phlegm samples can be collected and examined under the microscope. If the pneumonia is caused by bacteria or fungi, the organisms can often be detected by this examination. A sample of the sputum can be grown in special incubators, and the offending organism can be subsequently identified. It is important to understand that the sputum specimen must contain little saliva from the mouth and be delivered to the laboratory fairly quickly. Otherwise, overgrowth of non-infecting bacteria may predominate.
BLOOD TESTS
A blood test that measures white blood cell count (WBC) may be performed. An individual's white blood cell count can often give a hint as to the severity of the pneumonia and whether it is caused by bacteria or a virus. An increased number of neutrophils, one type of WBC, is seen in bacterial infections, whereas an increase in lymphocytes, another type of WBC, is seen in viral infections.
BRONCHOSCOPY
Bronchoscopy is a procedure in which a thin, flexible, lighted viewing tube is inserted into the nose or mouth after a local anesthetic is administered. The breathing passages can then be directly examined by the health care provider, and specimens from the infected part of the lung can be obtained.
NEEDLE ASPIRATION OF FLUID
Sometimes, fluid collects in the pleural space around the lung as a result of the inflammation from pneumonia. This fluid is called a pleural effusion. If the amount of this fluid that develops is large enough, it can be removed by inserting a needle into the chest cavity and withdrawing the fluid with a syringe in a procedure called a thoracentesis. In some cases, this fluid can become severely inflamed (parapneumonic effusion) or infected (empyema) and may need to be removed by more aggressive surgical procedures.
The extent of testing depends on how ill you are and your underlying risk factors and whether or not you are responding to therapy.
CONVENTIONAL MEDICAL TREATMENT
Pneumonia can be a serious and life-threatening infection. This is true especially in the elderly, children, and those that have other serious medical problems, such as COPD, heart disease, diabetes, and certain cancers. Fortunately, with the discovery of many potent antibiotics, most cases of pneumonia can be successfully treated. In fact, pneumonia can usually be treated with oral antibiotics without the need for hospitalization.
BACTERIAL PNEUMONIA TREATMENT OVERVIEW
Health care providers usually treat bacterial pneumonia with antibiotics. Although you may start to feel better shortly after beginning your medication, be sure to complete your entire course of antibiotics. Stopping medication too soon may cause your pneumonia to return. It also helps create strains of bacteria that are resistant to antibiotics - an increasingly serious problem in the United States.
PNEUMOCOCCAL BACTERIAL PNEUMONIA
Two vaccines are available to prevent pneumococcal disease; the pneumococcal conjugate vaccine (PCV7; Prevnar) and the pneumococcal polysaccharide vaccine (PPV23; Pneumovax). The pneumococcal conjugate vaccine is part of the routine infant immunization schedule in the U.S. and is recommended for all children under the age of 2 years and children 2-4 years of age who have certain medical conditions. The pneumococcal polysaccharide vaccine is recommended for adults at increased risk for developing pneumococcal pneumonia including the elderly, people who have diabetes, chronic heart, lung, or kidney disease, those with alcoholism, cigarette smokers, and in those people who have had their spleen removed.
Antibiotics often used in the treatment of this type of pneumonia include penicillin, amoxicillin and clavulanic acid (Augmentin, Augmentin XR), and macrolide antibiotics including erythromycin, azithromycin (Zithromax, Zmax), and clarithromycin (Biaxin). Penicillin was formerly the antibiotic of choice in treating this infection. With the advent and widespread use of broader-spectrum antibiotics, significant drug resistance has developed. Penicillin may still be effective in treatment of pneumococcal pneumonia, but it should only be used after cultures of the bacteria confirm their sensitivity to this antibiotic.
KLEBSIELLA BACTERIAL PNEUMONIA
Klebsiella pneumoniae and Hemophilus influenzae are bacteria that often cause pneumonia in people suffering from chronic obstructive pulmonary disease (COPD) or alcoholism. Useful antibiotics in this case are the second- and third-generation cephalosporins, amoxicillin and clavulanic acid, fluoroquinolones (levofloxacin [Levaquin], moxifloxacin-oral [Avelox], gatifloxacin-oral [Tequin], and sulfamethoxazole and trimethoprim [Bactrim, Septra]).
MYCOPLASMA BACTERIAL PNEUMONIA
Mycoplasma pneumonia are treated with antibiotics. Even so, recovery may not be immediate. In some cases fatigue may continue long after the infection itself has cleared. Many cases of mycoplasma pneumonia go undiagnosed and untreated. The signs and symptoms mimic those of a bad chest cold, so some people never seek medical attention. Mycoplasma pneumoniae is a type of bacteria that often causes a slowly developing infection. Symptoms include fever, chills, muscle aches, diarrhea, and rash. This bacterium is the principal cause of many pneumonias in the summer and fall months, and the condition often referred to as "atypical pneumonia." Macrolides (erythromycin, clarithromycin, azithromycin, and fluoroquinolones) are antibiotics commonly prescribed to treat Mycoplasma pneumonia.
LEGIONELLA BACTERIAL PNEUMONIA
Legionnaire's disease is caused by the bacterium Legionella pneumoniae that is most often found in contaminated water supplies and air conditioners. It is a potentially fatal infection if not accurately diagnosed. Pneumonia is part of the overall infection, and symptoms include high fever, a relatively slow heart rate, diarrhea, nausea, vomiting, and chest pain. Older men, smokers, and people whose immune systems are suppressed are at higher risk of developing Legionnaire's disease. Fluoroquinolones are the treatment of choice in this infection. This infection is often diagnosed by a special urine test looking for specific antibodies to the specific organism.
Mycoplasma, Legionnaire's, and another infection, Chlamydia pneumoniae, all cause a syndrome known as "atypical pneumonia." In this syndrome, the chest x-ray shows diffuse abnormalities, yet the patient does not appear severely ill. These infections are very difficult to distinguish clinically and often require laboratory evidence for confirmation.
P. CARINII PNEUMONIA
Pneumocystis carinii pneumonia is another form of pneumonia that usually involves both lungs. It is seen in patients with a compromised immune system, either from chemotherapy for cancer, HIV/AIDS, and those treated with TNF (tumor necrosis factor), such as for rheumatoid arthritis. Once diagnosed, it usually responds well to sulfa-containing antibiotics. Steroids are often additionally used in more severe cases.
VIRAL PNEUMONIA
Viral pneumonias do not typically respond to antibiotic treatment, although a few viral pneumonias may be treated with antiviral medications. The recommended treatment is generally the same as for the flu which consists of rest and plenty of fluids. These infections can be caused by adenoviruses, rhinovirus, influenza virus (flu), respiratory syncytial virus (RSV), and parainfluenza virus (that also causes croup). These pneumonias usually resolve over time with the body's immune system fighting off the infection. It is important to make sure that a bacterial pneumonia does not secondarily develop. If it does, then the bacterial pneumonia is treated with appropriate antibiotics. In some situations, antiviral therapy is helpful in treating these conditions.
FUNGAL PNEUMONIA
If your pneumonia is caused by a fungus, you will likely be treated with anti-fungal medications. Fungal infections that can lead to pneumonia include histoplasmosis, coccidiomycosis, blastomycosis, aspergillosis, and cryptococcosis. These are responsible for a relatively small percentage of pneumonias in the United States. Each fungus has specific antibiotic treatments, among which are amphotericin B, fluconazole (Diflucan), penicillin, and sulfonamides.
ANTIBIOTIC CONCERNS
Major concerns have developed in the medical community regarding the overuse of antibiotics. Most sore throats and upper respiratory infections are caused by viruses rather than bacteria. Though antibiotics are ineffective against viruses, they are often prescribed. This excessive use has resulted in a variety of bacteria that have become resistant to many antibiotics. These resistant organisms are commonly seen in hospitals and nursing homes. In fact, health care providers must consider the location when prescribing antibiotics (community-acquired pneumonia, or CAP, versus hospital-acquired pneumonia, or HAP).
The more virulent organisms often come from the health-care environment, either the hospital or nursing homes. These organisms have been exposed to a variety of the strongest antibiotics that are available. They tend to develop resistance to some of these antibiotics. These organisms are referred to as nosocomial bacteria and can cause what is known as nosocomial pneumonia when the lungs become infected.
Recently, one of these resistant organisms from the hospital has become quite common in the community. In some communities, up to 50 percent of Staph aureus infections are due to organisms resistant to the antibiotic methicillin. This organism is referred to as MRSA (methicillin-resistant Staph aureus) and requires special antibiotics when it causes infection. It can cause pneumonia but also frequently causes skin infections. In many hospitals, patients with this infection are placed in contact isolation. Their visitors are often asked to wear gloves, masks, and gowns. This is done to help prevent the spread of this bacteria to other surfaces where they can inadvertently contaminate whatever touches that surface. It is therefore very important to wash your hands thoroughly and frequently to limit further spread of this resistant organism.
OTHER TREATMENTS
In addition to these treatments, your health care provider may recommend over-the-counter (OTC) medications to reduce fever, treat your aches and pains, and soothe the cough associated with pneumonia. You do not want to suppress your cough completely, though, since coughing helps clear your lungs. If you must use a cough suppressant, use the lowest dose that helps you get some rest.
If you have severe pneumonia, you will be hospitalized and treated with intravenous antibiotics and put on oxygen. If you do not need oxygen, you may recover as quickly at home with oral antibiotics as in the hospital, especially if you have access to qualified home health care. Sometimes you may spend three or four days in the hospital receiving intravenous antibiotics and then continue to recover at home with oral medication.
FOLLOW-UP CARE
Your health care provider will most likely schedule a follow-up X-ray and an office visit after your initial diagnosis and treatment. By that time your infection should have cleared, but it is important for your health care provider to see you, even if you are feeling better. Follow-up appointments and X-rays are especially important in smokers. If you are not feeling better, the follow-up visit is an opportunity for your health care provider to schedule tests to determine more specifically what is causing your symptoms.
HOME TREATMENT
If you have pneumonia, the following measures can help you recover more quickly and decrease your risk of complications:
- Fever, headaches, and body aches are typical pneumonia symptoms. If fever is uncomfortable, take acetaminophen or ibuprofen to lower it. You may also sponge your body with lukewarm water to reduce fever. Do not use cold water or ice. Lowering the fever will not make your symptoms go away faster, but it may make you more comfortable. If the fever is not causing discomfort, you may just want to watch for signs that it is going higher. A slight fever may help the body fight the virus. To relieve body aches and headache, take aspirin, acetaminophen, or ibuprofen. To avoid the risk of Reye's syndrome, do not give aspirin to anyone under age 20. Also, be aware that aspirin and ibuprofen can cause stomach upset, while acetaminophen does not.
- Get plenty of rest. Even when you start to feel better, be careful not to overdo it.
- Drink lots of fluids, especially water. Liquids keep you from becoming dehydrated and help loosen mucus in your lungs. Hot tea herbal teas with lemon, water, fruit juices, and broth-like soups are all good choices.
- Coughing is a common symptom associated with pneumonia. Some coughing is beneficial to break loose and expel mucus and fluids in the lungs. To help ease cough discomfort, try sucking on cough drops or plain, hard candy. Taking a non-prescription cough medicine that contains dextromethorphan may help you get some sleep. Ask your health care provider about taking cough remedies. Some products contain a high percentage of alcohol. Use them with caution, especially when giving them to children. Elevating your head at night with an extra pillow(s) may help if coughing keeps you awake and it eases breathing difficulties.
- Take the entire course of any prescribed medications. Stopping medication too soon can cause your pneumonia to come back and contributes to the development of antibiotic-resistant bacteria.
- Avoid smoking and breathing other people's smoke. This is good advice anytime, but it is especially important when you have a respiratory infection like a cold, the flu, or pneumonia.
- Keep all of your follow-up appointments. Even though you feel better, your lungs may still be infected. It is important to have your health care provider monitor your progress.

THE MAJOR IMPORTANT NUTRIENTS FOR FIGHTING INFECTION
Pneumonia is rarely a problem in individuals with healthy immune systems. In these cases, the body's normal defence mechanisms can either shrug it off or weaken its impact. So healthy eating and perhaps nutritional supplements, if needed, should help to prevent pneumonia infections. There are three specific micronutrients that are especially effective for preventing pneumonia:
- Vitamin A is necessary for maintaining the health of the lining of the respiratory passages. It promotes healing of lung tissue, helps the immune system and reduces the duration of illness. A deficiency of this vitamin increases susceptibility to respiratory infections, which in turn can lead to pneumonia. (See Nutritional Supplements on this page for supplement links.)
- Vitamin C, taken with bioflavonoids, is another powerful anti-pneumonia weapon. Bioflavonoids help the body absorb Vitamin C. Vitamin C is the most important supplement to help fight an infectious pneumonia and other inflammations. High dosages of vitamin C to bowel tolerance are recommended. A recommended dosage of Vitamin C, with bioflavonoids, is 1,000 mg every few hours. A major review of over 60 separate studies by Professor Harri Hemila, of the University of Helsinki, concluded that there is a substantial and consistent benefit from vitamin C. Intakes of more than 1 g/day (which is around 20 times the recommended daily allowance) have been proved to cut pneumonia rates almost in half, especially among the elderly - even in serious cases requiring hospitalization. - Zinc deficiency is known to be associated with weakened immune function and, thus, vulnerability to serious infectious disease. Zinc fortifies the immune system and also helps the body maintain adequate amounts of vitamin E, which does not stay in the body for long on its own. Many studies have shown that, in developing countries, giving children 20-30 mg/day of zinc will almost halve pneumonia rates. However, even in the developed world, children are often zinc-deficient. Agriculture practices, and food refining and processing have all conspired to reduce the average zinc intake in developed countries to about a third below the recommended daily minimum. In cases of pneumonia, zinc appears to work by reducing lung inflammation and obstruction to the airways. Signs of zinc deficiency include poor wound-healing, hair loss, skin rashes, night-blindness, and impaired taste and smell. A recommended dosage of zinc is 30-80 mg daily. OTHER IMPORTANT NUTRIENTS
Deficient in some lung diseases, the antioxidant vitamin E prevents free-radical damage at the cellular level. A recommended dosage of vitamin E (d-alpha-tocopherol) is 200-400 IU daily. Vitamin E with mixed tocopherols dosage recommendation is 400-800 IU twice daily.
Since deficiencies of vitamin B are common in pneumonia, a B-complex should also be added. Recommended dosage of a quality B-Complex is 100 mg daily.
If antibiotics are taken, an acidophilus combination should be supplemented to replenish healthy intestinal bacteria destroyed by antibiotics. These natural intestinal bacteria are important for the normal intestinal balance and for overall health. Take a Acidophilus combination, 3 capsules or 1 teaspoon daily.
Pneumonococcal vaccines provides protection against more than 20 different strains of microorganisms that can cause pneumonia. It is recommended by conventional practitioners for anyone without a spleen, anyone with chronic disease (especially diseases that affect the lungs), and everyone over the age of 65.
A urine test for Streptococcus Pneumoniae takes 15 minutes to perform. It is more efficient than diagnosis from mucus, blood, or saliva.
The use of antibiotics for minor infections may lead to the development of antibiotic-resistant bacteria in the upper airway, which can cause pneumonia.

See your health care provider if you suspect pneumonia. Children with pneumonia have to be carefully monitored. If you suspect that your child might have pneumonia, seek medical advice immediately. This is a potentially dangerous disease. Do not use any home remedy as a substitute for proper medical care.
Eat a diet consisting of raw fruits and vegetables. Eat plenty of yellow vegetables for vitamin A, which supports the health of respiratory passages. Vitamin C, found in citrus fruits and leafy green vegetables, promotes the healing of infection. Fresh fruits and vegetables also provide enzymes, which help build a strong immune system. Eat whole grains and sprinkle wheat germ on your morning cereal to ensure adequate vitamins B and E. These are often deficient, leaving the body susceptible to disease.
Consume plenty of fluids (at least 3 to 4 quarts of fluid and water a day while resting in bed), especially fresh juices, herbal teas, soups, and quality water, to prevent dehydration (fluid loss during fever) and help flush out the body. Drink several cups of echinacea tea daily. This helps to build up your immune system. To shorten the length of respiratory infection, go on a liquid diet emphasizing hot herbal teas and hot broth for one or two days. Fast on pure juices, herbal teas, fresh lemon juice and quality water. Fasting during acute congestive illnesses as pneumonia is a wise choice since it assists the body in decongesting the lungs and promotes elimination of wastes. Take hot chicken or turkey soup. This is grandmother's old remedy and it is still good today. Add some cayenne pepper and fresh crushed or sliced garlic to your soup (the more the better). Cayenne pepper helps to prevent and break up congestion and the fresh garlic helps to fight viral infections. Sometimes I make a fresh onion, fresh garlic, and cayenne soup in a chicken or turkey broth. I use lots of onion and several cloves of fresh garlic in my soup. Exclude dairy products, sugar, and white flour products from your diet.
Do not take zinc at the same time you eat or drink citrus fruits or juices. It will diminish the effectiveness of the zinc. Do consume a lot of other types of fruit.
Do not take iron supplements while a fever exists.
Do not give aspirin to a child who has a fever. The combination of aspirin and a viral illness has been linked to the development of Reye's Syndrome, a potentially dangerous complication.
If you consume alcoholic beverages even occasionally, or if you have liver or kidney disease, be cautious about using the pain killer acetaminophen (Tylenol, Datril, and others). The combination of alcohol and acetaminophen has been associated with serious liver problems.
If you are over 65, see your health care provider. Bacterial and viral respiratory infections can cause serious complications for people in this age group.
Since bacterial and viral microorganisms are often transmitted by way of the hands, wash your hands frequently with an antibacterial soap. Rinse and dry your hands thoroughly and keep them away from your eyes, nose, and mouth. If you sneeze or cough, cover your nose and mouth and use a disposable tissue (and then wash your hands again). Teach your children not to touch their eyes or noses, or to put things in their mouths and wash their hands frequently.
Use a cool mist humidifier or vaporizer to help ease breathing. You can keep air moisture high by placing a container filled with water near a radiator or heat vent or by simmering a pot of water on your stove. The bedrooms should be aired frequently and the patient kept warm in bed. Whenever possible and weather allows, the patient should sit out in the fresh air while dressing warmly.
If there is no fever, place moist, hot towels on the chest and cover them with a dry towel, then a wool shawl. Place a heating pad or hot water bottle on your chest to relieve pain.
Consider using a device called Air Supply from Wein Products. This is a personal air purifier that is worn around the neck. It kills and deactivates airborne viruses, bacteria, molds, and spores.
Get plenty of rest and sleep. The pneumonia should be healed completely before resuming a regular routine. A partial recuperation could result in more serious, chronic lung ailments.
For fever, wash the body with a terry cloth dipped in apple cider vinegar. Apply cool chest compresses for high fever.
Gently massage extremities to prevent thrombosis (blood clots) from forming and aids in increasing circulation to the arms and legs). A back massage helps relax muscles in the back that may be sore from coughing and aids in breathing efforts.
HOMEOPATHIC PREVENTION & REMEDIES
Constitutional treatment is recommended during and after a pneumonia infection to boost the immune system. The sample of remedies below should be used in mild cases of viral pneumonia. If no improvement occurs within twenty-four hours, see a health care provider. Take every two hours for up to ten doses.

Cayenne (Capsicum) is an antimicrobial that can help control infections such as pneumonia. Cayenne helps to keep the mucus flowing, aiding in preventing congestion and headaches. Simply add a bit of cayenne powder to soups and other foods. My favorite is a "Cayenne Cocktail" made from tomato or vegetable-tomato (V8) juice, heated using a microwave or in a stove top pan, with about 1/2 to 1 teaspoon of cayenne pepper added to it (this is very hot to the taste). Sip it down as quickly as possible while the juice is warm. I usually make this using a coffee mug as a container before adding the cayenne pepper. I find this is soothing on a sore throat and it helps break up mucus and clears the sinuses.

Combine equal parts of fresh or dried Plantain and Thyme. Bring to boil 1 cup of water with 1 slice of lemon and 1 teaspoon of demerrara sugar (a type of raw sugar) or honey. Remove from the stove and add the herb mix. Steep for half a minute, strain and sip tea as hot as possible. In the acute stage of pneumonia, prepare and drink this tea hourly.
Equal parts of Lungwort, Plantain and Marshmallow is an excellent combination for pneumonia. Pour 1 cup of boiling water over 1 teaspoon of this mix; steep for ten minutes; strain and sip slowly three times daily. Prepare fresh each time.
The Silica in Horsetail helps to strengthen and rebuild lung tissue. Take 15-25 drops of Horsetail tincture or 3-4 capsules of Silica extract three times daily.

Pour 1 cup of boiling water over 1 tablespoon of the herbal blend. Steep for 10 minutes, then strain. Sweeten with honey to taste and drink as much of the tea as possible throughout the day.
ANTIMICROBIAL TEA
1/4 teaspoon dried Goldenseal Root
Pinch of dried Licorice Root
Pneumonia requires aggressive therapy with strong antimicrobials. Goldenseal and licorice root are natural antibiotics that can be used as effective treatment. Pour 1 cup of boiling water over the dried, chopped Goldenseal root and dried Licorice root. Cover and steep for 10 minutes, then strain. Drink 1/3 cup of the tea 3 times daily. Caution: do not take Goldenseal internally on a daily basis form more than 7 days at a time. Women who are pregnant should not use it at all.
INHALATION THERAPY
Certain essential oils are infection fighting and fever reducing and can be particularly useful in combating pneumonia. These oils include Tea Tree Essential Oil, Bergamot Essential Oil, and Peppermint Essential Oil. Place a few drops in a vaporizer or a simmer-pot, or add them to a dish of steaming water, and inhale deeply for 3-10 minutes. Tea Tree Essential Oil and Eucalyptus Essential Oil are immune-system stimulants, as well.
For excess mucus and congestion, add a few drops of Rosemary Essential Oil to a tissue and inhale it deeply throughout the day for relief.
Thyme is a potent antibiotic that, when used in an inhalation, can help reduce coughs and congestion. Pour 2 quarts of boiling water over 3 tablespoons of dried thyme leaves. Place a towel over your head, bend towards the bowl and inhale deeply. Thyme essential oil may be used in place of thyme leaves, if desired.
Steam inhalation with Eucalyptus, Chamomile or Peppermint will soothe and heal the affected lung tissue. Add 10 drops of essential oils of each to a bowl of hot water. Inhale for fifteen minutes.

- Signs of a bacterial infection develop, such as an ear infection, bronchitis, or pneumonia.
- Influenza develops in a person over age 65 who has other health problems (lung, heart, or kidney disease or diabetes) or in someone who is being treated with chemotherapy for cancer.
- Influenza develops in a person of any age who has a long-term respiratory illness, such as asthma or chronic obstructive pulmonary disease (COPD).
- Symptoms include fever over 101°F (38.33°C), shaking chills, and a cough that produces mucus from the lungs (productive cough).
- Fever is high or prolonged. See Fevers: The guidelines for ages 3 and under or for ages 4 and older for more information. However, it is common for adults with the flu to have high fevers [up to 103°F(39.4°C)] for 3 to 4 days. If many people in your community have similar symptoms and it is flu season, it is likely you have the flu and do not need to see a health care provider. If you have any concerns, contact your health care provider.
In most healthy people who have the flu, it will go away in 5 to 7 days, though fatigue can last much longer. Although you may feel very sick, home treatment is usually all that is needed. If it is flu season, you may just want to treat your symptoms at home. Watch closely for signs of a bacterial infection and developing pneumonia.
Early treatment (within 48 hours of your first symptoms) with antiviral medications may reduce the severity of influenza. However, it is not certain whether antiviral medications can prevent serious flu-related complications. Complications, which occur especially in infants, older adults, and people who have chronic health problems, may require a health care provider visit and care beyond home treatment.
Call your health care provider if you have any concerns that your symptoms are caused by something other than a cold or flu.
A health care provider who specializes in treating infectious diseases may be needed if the diagnosis is unclear or severe complications develop.
