DESCRIPTION
A sprain is not the same thing as a strain. If a muscle is stressed beyond its capability, it becomes strained. Putting undue weight on the muscles and using the muscles for prolonged periods without rest can create muscle strain. A strained muscle may go into spasms or knot up instead of relaxing normally. Localized pain (during movement), swelling, and loss of mobility occur.
If one of the ligaments, the tissues that connect bones to muscles, is wrenched or stretched excessively, the ligament may tear, causing a sprain. There is likely to be a brief sharp pain followed by rapid swelling. Soft tissue surrounding the joint may be sore and bruised. Sprains can result from unexpected movement or twisting of the affected area, or from a hard fall. The joints most often sprained are the ankle, back, finger, knee, and wrist.
Chronic (long-lasting) strains are injuries that gradually build up from overuse or repetitive stress, resulting in tendinitis (inflammation of a tendon). For example, a tennis player may get tendinitis in his or her shoulder as the result of constant stress from repeated serves.
SEVERITY OF SPRAINS AND STRAINS
A health care provider categorizes sprains and strains according to severity.
- A Grade I (mild) sprain or strain involves some stretching or minor tearing of a ligament or muscle. Grade I injuries usually heal quickly with rest, ice, compression, and elevation (RICE). Therapeutic exercise can also help restore strength and flexibility.
- A Grade II (moderate) sprain or strain is a ligament or muscle that is partially torn but still intact. Grade II injuries are treated similarly but may require immobilization of the injured area to permit healing.
- A Grade III (severe) sprain or strain means that the ligament or muscle is completely torn, resulting in joint instability. Grade III sprains and strains usually require immobilization and possibly surgery to restore function.
The key to recovery is an early evaluation by a medical professional. Once the injury has been determined, a treatment plan can be developed. With proper care, most sprains and strains will heal without long-term side effects.

KEEP YOUR KNEES HEALTHY

Three years ago Linda Morse tossed a tennis ball gracefully overhead, reached for it with her racket, planted her left leg on the hard tennis court for leverage, and felt something snap. "It was one of those 'Oh, no' moments," says Morse, now 73. "Most people I know who play tennis have some kind of knee trouble, and now, I just knew, I did, too."
Eleven million men and women over 45 will suffer from knee pain this year. Here's how to avoid becoming one of them.
In a typical year more than 6 million men and women 62 and older - and another 5 million between their mid-40s and early 60s - will visit their health care provider because of knee pain. Knees are one of the most commonly injured joints and the most likely to be afflicted with arthritis. Strange twinges or clicking noises are familiar complaints as well, along with - more dramatically - pain, swelling, and a tendency for the joint to seize up.
Serious knee problems are not inevitable, though. The human knee, most experts agree, can outlast us, provided it is not abused and receives some basic preventive maintenance. The right lifestyle and activity choices can help make your knees stronger, healthier, and pliant - and less likely to end your tennis game or any other sports.
WHEN ALL IS WELL...
When all is well inside your knee, it easily withstands loads equal to more than 4 times your body weight. It can also gyroscope in three dimensions, allowing your leg not just to bend, but also to pivot and twist. "It's a remarkable construction," says Charles Kenny, M.D., and orthopedist in Stockbridge Massachusetts.
The largest joint in your body, the knee is a complex interweaving of ligaments (which attach bone to bone), tendons (attaching bone to muscle), bones (primarily the tibia, or shin-bone; the femur, or thighbone; and the patella, or kneecap), and cartilage, a specialized tissue that provides cushioning. You have two types of cartilage in your knees. The first is the meniscus, a small, crescent-shaped pillow of squishy material that sits between the tibia and femur and acts as a shock absorber; there are two menisci, one on each side of your knee. The second type of knee cartilage is articular cartilage, a grand-sounding name for the thin layer of cells that covers the ends of the bones, like socks, and serves about the same purpose: keeping the bone ends safe and giving them a smooth, frictionless surface to move against.
But the knee's intricacy also makes it vulnerable. An injury, even one you suffered years ago and forgot about, can render your knee slightly unstable, like a car that's rattled itself out of alignment. The tissues holding the knee together shorten, pulling the bones out of their ideal positioning. To imagine what happens next, think of your misaligned car's balding, frayed tire tread. That's what the cartilage looks like inside an unstable knee. If it wears away completely, you have bone-on-bone arthritis, a painful condition.
Aging doesn't help. "By your 40s or 50s, the meniscus is becoming dehydrated," says Frederic Nicola, M.D., an orthopedic surgeon in Marina del Rey, California. This makes it more prone to a tear. In addition, as you get older, cartilage cells do not divide and reproduce as well as they once did, which causes the cartilage around your knees to grow thinner and less resilient. If you are not careful, the same movements you have always made - walking, serving a tennis ball, going up and down stairs - can tear or bruise the knee's tissues.
SO HOW CAN YOU KEEP YOUR KNEES AGILE AND PAIN-FREE?
Move More: "I know it sounds counterintuitive, especially if your knee is sore, but the most important thing for knee health is to be active," says Leigh Callahan, Ph.D., an associate professor with the Thurston Arthritis Research Center at the University of North Carolina. A knee that is not used stiffens; the muscles around it start to atrophy, and because these muscles would otherwise absorb some of the shock that moves up the leg with every step, a stiff knee has to take on more of the body's weight than a supple one.
Strengthen Your Thigh Muscles: In a 2009 study of 265 men and women with knee osteoarthritis, Mayo Clinic researchers found that those with the strongest quadriceps (thigh muscles) had less knee pain and better physical function than those with the weakest. But there is a right way and a wrong way to build those muscles. If you are an avid gym-goer, avoid the leg-extension machines, Kenny suggests. "They put far too much stress on the knee joint," he says.
Stretch Regularly: Maintaining flexibility is important, especially in the muscles and tendons that connect directly to the knee, such as the hamstrings and the quadriceps. If these tissues become overly taut, they can pull the knee out of alignment.
Lose The Weight, Finally: Losing as little as 5 percent of your body weight can dramatically reduce your chances of developing knee arthritis, the most common cause of knee pain, according to the authors of a study presented last November at a meeting of the American College of Rheumatology.
Take Glucosamine / Chondroitin: Many health care providers recommend taking 1,500 milligrams (mg) of glucosamine and 1,200 mg of chondroitin daily to strengthen aging cartilage. Their use is controversial because no studies have proven they work. But no studies have found a downside, either. "I firmly believe that this helps," Nicola says.
Get To The Doctor - Pronto - If Your Knee Is Swollen: Likewise, a rattling, grinding noise, or a feeling of gravel moving around inside your knee, should prompt a visit to the health care provider. These can be early warning signs of damaged cartilage. Fortunately, several techniques have been developed in the past few years to repair worn cartilage, including autologous chondrocyte implantation: a surgeon suctions out some of your living cartilage cells, grows millions of copies of them in the lab, then reinjects them into your knees. The technique is new - and is best used on tiny areas of worn cartilage. But researchers believe so-called tissue engineering could cone day offer relief even for those with full bone-on-bone arthritis.
If Your Doctor Recommend Surgery Right Away, Seek A Second Opinion: "I personally think there is more knee surgery being done these days than is warranted," says Kenny, himself a retired surgeon. Several major studies of arthroscopic surgery to remedy sore knees have found little or no benefit. Physical therapy, as well as anti-inflammatory medicines, can be just as effective - and less invasive.
KNEE FRIENDLY EXERCISES
Charles Kenny, M.D., a Massachusetts orthopedist, recommends the following three exercises to strengthen the muscles that attach to the knee. First, be sure to complete a short, five-minute warm-up (such as walking or jogging in place), to get blood moving to the muscles.

STRAIGHT-LEG LIFT
Sit against a wall, with one leg bent and the other straight. Gently lift the straight leg off the floor. (It won't go far.) Hold for a beat. Lower the leg gently. Repeat a few times, then switch legs.![]()
WALL SQUAT
Imagine you are sitting in a chair with your back against the wall, only there is no chair there. Stay in that position for 10 seconds, then slowly straighten up.![]()
SIDE SHUFFLE
Just like it sounds: shuffle quickly to one side, then stop and shuffle to the other. This will strengthen the muscles on either side of your knees.
After that fateful day on the tennis court, Linda Morse visited an orthopedic surgeon, who diagnosed a tear in her meniscus and scheduled her for surgery. But when he went into her knee, he found no tear - just a little arthritis. Since then, Morse has dedicated herself to protecting her knees. She exercises regularly, keeps her build lean, and does not play on hard tennis courts anymore. "It's made me diligent," she says of her injury scare. "Today I feel like I have the knees of a 40- or 50-year old."

Planning a walking trip? These six simple exercises will keep your feet in tiptop shape.
Americans walk an average of 100,000 miles during their lifetimes. That is like hiking from New York to California - more than 30 times. And that pounding can take a toll on your tootsies: more than 300 ailments can affect your feet, according to the American Podiatric Medical Association (apma.org), from bunions to heel pain. Foot-friendly accessories can help - acrylic socks that wick away moisture to help you avoid blisters, well-fitting shoes that do not pinch or put pressure on your feet - but the key to wellness is exercise, which can strengthen muscles, soothe achy joints, and improve range of motion, says Paul Langer, D.P.M., author of "Great Feet For Life". To work on specific issues, go barefoot and try these six targeted moves.
MARBLE PICKUPS
What They Do: Strengthen muscles in the bottom of the foot and ease arthritic joints in the mid-foot. Can also minimize the discomfort of hammertoes and bunions.
How To Do Them: Sit on a chair and use your toes to pick up marbles from the floor. Drop marbles into a bowl on the floor; repeat pickups with your other foot. Do for 5 minutes in total; work up to 10 minutes.
ARCH ROLLS
What They Do: Ease heel pain and loosen tight arches.
How To Do Them: Sit on a chair and roll a tennis ball or water bottle back and forth under one foot. Do for 2 to 5 minutes; repeat with your other foot.
TOWEL SCRUNCHES
What They Do: Strengthen mid-foot muscles and ease discomfort from fallen arches and plantar fasciitis (an inflammation of the connective tissue on the bottom of the foot that causes arch and heel pain).
How To Do Them: Lay a hand towel on the floor. While sitting or standing, use your toes to pull the towel toward you. Do 5 to 10 times with one foot; repeat with your other foot.
TOE STRETCHES
What They Do: Improve flexibility of the bottom of the foot, which can help ease the pain of plantar fasciitis.
How To Do Them: Sit on a chair. Cross one leg over the other so your ankle rests on your thigh. Grasp the toes on the crossed leg and gently pull them toward your shin; hold for 10 seconds. Repeat 3 times with each foot.
TOE WRITING
What They Do: Maintains the rang of motion in your toes, feet and ankles.
How To Do Them: Sit on a chair and raise one foot off the ground. Next, write the alphabet in the air with your toes, making big, bold letters. Repeat with your other foot.
CALF STRETCHES
What They Do: Loosen a tight Achilles tendon and tight calf muscles, which can cause pain in the forefoot or heel.
How To Do Them: Stand with your palms flat against a wall and your arms straight. Move one foot so it's 12 to 24 inches behind the other. Bend your knee that is closest to the wall, and keep your rear leg straight. Both feet should stay flat on the floor. Hold for 10 to 30 seconds. Then also bend the knee of your rear leg and hold for another 10 to 30 seconds. Repeat once with the opposite leg.

RECOMMENDATIONS
Immediately after the injury, raise the affected area above the heart level, if possible. Apply ice for no longer than 20 minutes. Do not use heat immediately following an injury. Apply ice intermittently every 4 hours for the first 1 to 2 days. Then, after the inflammation has subsided, apply heat for 20 minute periods 2 to 3 times a day. If possible, use a splint or sling to immobilize and protect the injured area.
If there is no significant swelling, call your health care provider right away or go to a hospital emergency room to have the injury evaluated. Especially with injuries to the wrists and ankles, it is wise to have x-rays taken to make sure no bones have been broken.
Consume plenty of juices made from fresh raw vegetables, including beets, garlic, and radishes. Raw vegetables are high in valuable vitamins and enzymes.

Make your home safer by taking measures such as the following:
- Use non-slippery floor coverings. If you have carpets that are not fitted, make sure the edges are taped down.
- Make sure you keep your path clear. If you have pets, they should be trained to keep out of your way. Do not leave things on the floor.
- Make sure you have plenty of light so that you can see properly.
- Wipe up any spilled liquids immediately.
- Wear shoes or slippers with rubber soles.

CONSIDERATIONS
Minor sprains can usually be treated at home. However, anytime an injury becomes painful and swollen, you should see a medical professional - especially if you hear a popping sound and find that you cannot use the injured joint normally.
Aromatherapy can be helpful. Cold compresses made with essential oils of Camphor, Chamomile, Eucalyptus, Lavender, and/or Rosemary are good. Add 10 drops or so of essential oil to 1 quart of cool water and use the mixture to make the compresses.
Clay poultices can be used to treat sprains and fractures.
