DESCRIPTION
Meniere's disease is an abnormality of the inner ear causing a host of symptoms. It is a disorder that can affect hearing and balance. It is characterized by episodes of dizziness and tinnitus or a roaring sound in the ears, fluctuating and progressive hearing loss, usually in one ear and the sensation of pain in the affected ear. It is caused by an increase in volume and pressure of the endolymph of the inner ear. The disorder usually affects only one ear and is a common cause of hearing loss. It is named after the French physician Prosper Meniere, who first reported that vertigo was caused by inner ear disorders and who first described the syndrome in an article published in 1861.
HISTORY
Ménière's disease had been recognized prior to 1972, but it was still relatively vague and broad at the time. The American Academy of Otolaryngology-Head and Neck Surgery Committee on Hearing and Equilibrium (AAO HNS CHE) made set criteria for diagnosing Meniere's, as well as defining two sub categories of Meniere's: cochlear (without vertigo) and vestibular (without deafness).
In 1972, the academy defined criteria for diagnosing Meniere's disease as:
- Fluctuating, progressive, sensorineural deafness.
- Episodic, characteristic definitive spells of vertigo lasting 20 minutes to 24 hours with no unconsciousness, vestibular nystagmus always present.
- Usually tinnitus.
- Attacks are characterized by periods of remission and exacerbation.
In 1985, this list changed to alter wording, such as changing "deafness" to "hearing loss associated with tinnitus, characteristically of low frequencies" and requiring more than one attack of vertigo to diagnose. Finally in 1995, the list was again altered to allow for degrees of the disease:
- Certain: Definite disease with histopathological confirmation.
- Definite: Requires two or more definitive episodes of vertigo with hearing loss plus tinnitus and/or aural fullness.
- Probable: Only one definitive episode of vertigo and the other symptoms and signs.
- Possible: Definitive vertigo with no associated hearing loss.
CAUSES
Meniere's disease is rare and the exact cause or causes of Meniere's disease is not known, but many experts believe it to be related to or results from endolymphatic hydrops or excessive swelling of the small, fluid-filled chambers of the inner ear and a change in fluid volume within a portion of the inner ear known as the labyrinth.
The labyrinth has two parts: the bony labyrinth and the membranous labyrinth. The membranous labyrinth, which is encased by bone, is necessary for hearing and balance and is filled with a fluid called endolymph. When your head moves, endolymph moves, causing nerve receptors in the membranous labyrinth to send signals to the brain about the body's motion. An increase in endolymph, however, can cause the membranous labyrinth to balloon or dilate, a condition known as endolymphatic hydrops.
It is thought that endolymphatic fluid bursts from its normal channels in the ear (a rupture of the membranous labyrinth allowing the endolymph to mix with perilymph, another inner ear fluid that occupies the space between the membranous labyrinth and the bony inner ear) and flows into other areas causing damage. This mixing, scientists believe, may be related to swelling of the endolymphatic sac or other tissues in the vestibular system of the inner ear with eventual pressure and disruptions within the inner ear, which is responsible for the body's sense of balance causing periodic attacks of vertigo which can cause the symptoms of Meniere's disease. The attacks can last from 10 minutes to several hours.
The symptoms may occur in the presence of a middle ear infection, head trauma or an upper respiratory tract and viral infections, temporomandibular joint (TMJ) syndrome, or drug use (by using aspirin), smoking cigarettes or excessive alcohol consumption. Stress, barometric changes, pregnancy, visual stimuli, experiencing orgasm, exposure to loud noises (noise pollution) may trigger this disorder. They may be further exacerbated by excessive consumption of caffeine, sugar and salt in some patients. Other factors, such as obesity and high blood cholesterol, may contribute to this syndrome. Allergies, spasms of the blood vessels that supply the inner ear, and, for women, fluid retention during the premenstrual period may also be related to Meniere's disease. Impaired blood flow to the brain from clogged arteries and poor circulation may also be involved.
It has also been proposed that Meniere's symptoms in some patients may be caused by the deleterious effects of a herpes virus. Herpesviridae are present in a majority of the population in a dormant state. It is suggested that the virus is reactivated when the immune system is depressed due to a stressor such as trauma, infection or surgery (under general anesthesia). Symptoms then develop as the virus degrades the structure of the inner ear.
SYMPTOMS
The symptoms of Meniere's are variable; not all sufferers experience the same symptoms. The symptoms of Meniere's disease occur suddenly and can arise daily or as infrequently as once a year. Vertigo, often the most debilitating symptoms of Meniere's disease, typically involves a whirling dizziness that forces the sufferer to lie down. Vertigo attacks can lead to severe nausea, vomiting and sweating and often come with little or no warning. However, so-called "classic Meniere's" is considered to comprise the following four symptoms:
- Periodic episodes of rotary vertigo (the abnormal sensation of movement) or dizziness.
- Fluctuating, progressive, unilateral (in one ear) or bilateral (in both ears) hearing loss, often initially in the lower frequency ranges.
- Unilateral or bilateral tinnitus (the perception of noises, often ringing, roaring, or whooshing), sometimes variable.
- A sensation of fullness or pressure in one or both ears.
Meniere's often begins with one symptom, and gradually progresses. However, not all symptoms must be present for a health care provider to make a diagnosis of the disease. Several symptoms at once is more conclusive than different symptoms at separate times. Some individuals with Meniere's disease have attacks that start with tinnitus (ear noises), a loss of hearing, or a full feeling or pressure in the affected ear. It is important to remember that all of these symptoms are unpredictable. Typically, the attack is characterized by a combination of vertigo, tinnitus, and hearing loss lasting several hours. People experience these discomforts at varying frequencies, durations, and intensities. Some may feel slight vertigo a few times a year. Loss of balance or unsteadiness may last for several days after the attack. Others may be occasionally disturbed by intense, uncontrollable tinnitus while sleeping. Meniere's disease sufferers may also notice a hearing loss and feel unsteady all day long for prolonged periods. Other occasional symptoms of Meniere's disease include headaches, abdominal discomfort, and diarrhea. A person's hearing tends to recover between attacks but over time becomes worse.
Attacks of vertigo can be severe, incapacitating, and unpredictable. Some patients experience vertigo for hours or days, and this combines with an increase in volume of tinnitus and temporary, albeit significant, hearing loss. Hearing may improve after an attack, but often becomes progressively worse. Nausea, vomiting, and sweating sometimes accompany vertigo.
Some sufferers experience what are informally known as "drop attacks" - a sudden, severe attack of dizziness or vertigo that causes the sufferer, if not seated, to fall. Patients may also experience the feeling of being pushed or pulled (Pulsion). Some patients may find it impossible to get up for some time, until the attack passes or medication takes effect.
In addition to hearing loss, sounds can seem tinny or distorted, and patients can experience unusual sensitivity to noises (hyperacusis). Some sufferers also experience nystagmus, or uncontrollable rhythmical and jerky eye movements, usually in the horizontal plane, reflecting the essential role of non-visual balance in coordinating eye movements.
Studies done on both right and left ear sufferers show that patients with their right ear tend to do significantly worse in cognitive performance. General intelligence was not hindered, and it was concluded that declining performance was related to how long the patient had been suffering from the disease.
DIAGNOSIS & TREATMENT
DIAGNOSIS
Many disorders have symptoms similar to Meniere's disease. Health care providers establish it with complaints and medical history. Proper diagnosis of Meniere's disease entails several procedures, including a medical history interview and a physical examination with a detailed otolaryngological examination by a health care provider, hearing (audiometery) and balance tests, and medical imaging with magnetic resonance imaging (MRI). An MRI is a useful test to determine whether a tumor is causing the patient's vertigo and hearing loss and should be performed to exclude a tumor of the eighth cranial nerve (vestibulocochlear nerve) or superior canal dehiscence which could cause similar symptoms. Accurate measurement and characterization of hearing loss are of critical importance in the diagnosis of Meniere's disease. Because there is no definitive test for Meniere's, it is only diagnosed when all other causes have been ruled out.
Through the use of several types of hearing tests, health care providers can characterize hearing loss as being sensory, arising from the inner ear, or neural, arising from the hearing nerve. Recording the auditory brain stem response, which measures electrical activity in the hearing nerve and brain stem, is useful in differentiating between these two types of hearing loss. Electrocochleography, recording the electrical activity of the inner ear in response to sound, helps confirm the diagnosis.
To test the vestibular or balance system, health care providers irrigate the ears with warm and cool water or air. This procedure, known as caloric testing, results in nystagmus, rapid eye movements that can help a health care provider analyze a balance disorder. Since tumor growth can produce symptoms similar to Meniere's disease,
Meniere's typically begins between the ages of 30 and 60 and affects men slightly more than women. Based on a recent study, it is estimated that there are currently approximately 615,000 individuals with diagnosed Meniere's disease in the United States and 45,500 newly diagnosed cases each year.
CONVENTIONAL MEDICAL TREATMENT
PURPOSE OF TREATMENT
There is no cure for Meniere's disease. Initial treatment is aimed at both dealing with immediate symptoms and preventing recurrence of symptoms, and so will vary from patient to patient. Health care providers may recommend vestibular training, methods for dealing with tinnitus, stress reduction, hearing aids to deal with hearing loss, and medication to alleviate nausea and symptoms of vertigo.
DIETARY & ENVIRONMENTAL CHANGES
Several environmental and dietary changes are thought to reduce the frequency or severity of symptom outbreaks. The symptoms of the disease are often controlled successfully by reducing the body's retention of fluids through dietary changes (such as a low-salt or salt-free diet and no caffeine or alcohol) or medication.
Most patients are advised to adopt a low-sodium diet, typically one to two grams (1000-2000 mg daily) at first, but diets as low as 400 mg are not uncommon. Patients are advised to avoid caffeine, alcohol and tobacco, all of which can aggravate symptoms of Meniere's. Some recommend avoiding Aspartame.
Patients are often prescribed a mild diuretic (sometimes vitamin B-6). Women may experience increased symptoms during pregnancy or shortly before menstruation, probably due to increased fluid retention. Some Health care providers recommend Lipoflavonoids. Many patients will have allergy testing done to see if they are candidate for allergy desensitization as allergies have been shown to aggravate Meniere's symptoms. Changes in medications that either control allergies or improve blood circulation in the inner ear may help. Eliminating tobacco use and reducing stress levels are more ways some people can lessen the severity of their symptoms.
Treatments aimed at lowering the pressure within the inner ear include antihistamines, anticholinergics, steroids, and diuretics. Devices that provide transtympanic micropressure pulses (such as the Meniett) are now showing some promise and is becoming more widely used as a treatment for Meniere's. The Meniett, specifically, is proven to be a safe method for reducing vertigo frequency for a majority of users.
The anti-herpes virus drug Acyclovir has also been used with some success to treat Meniere's Disease. The likelihood of the effectiveness of the treatment was found to decrease with increasing duration of the disease probably because viral suppression does not reverse damage. Morphological changes to the inner ear of Meniere's sufferers have also been found which it was considered likely to have resulted from attack by a herpes simplex virus. It was considered possible that long term treatment with acyclovir (greater than six months) would be required to produce an appreciable effect on symptoms. Herpes viruses have the ability to remain dormant in nerve cells by a process known as HHV Latency Associated Transcript. Continued administration of the drug should prevent reactivation of the virus and allow for the possibility of an improvement in symptoms. Another consideration is that different strains of a herpes virus can have different characteristics which may result in differences in the precise effects of the virus. Further confirmation that acyclovir can have a positive effect on Meniere's symptoms has been reported.
Many patients consider fluorescent lighting to be a trigger for symptoms. The plausibility of this can be explained by how important a part vision plays in the overall mechanism of human balance.
SURGERY FOR VERTIGO CONTROL
Surgery may be recommended if medical management does not control vertigo. Different surgical procedures have been advocated for patients with persistent, debilitating vertigo from Meniere's disease. Injection of steroid medication behind the eardrum, or surgery to decompress the endolymphatic sac may relieve symptoms. Permanent surgical destruction of the balance part of the affected ear can be performed for severe cases if only one ear is affected.
Chemical Labyrinthectomy: Recently, the administration of the ototoxic antibiotic drug (such as gentamicin), injected directly into the middle ear space has gained popularity worldwide for the control of the vertigo of Meniere's disease. The drug "kills" the vestibular apparatus.
Surgical Labyrinthectomy: Alternatively, surgeons can cut the nerve to the balance portion of the inner ear in a vestibular neurectomy, or the inner ear itself can be surgically removed (labyrinthectomy). These treatments eliminate vertigo, but because they are destructive, they are a last resort. Typically balance returns to normal after these procedures, but hearing loss may continue to progress. It is usually reserved for patients with non-functional hearing in the affected ear.
Vestibular Neurectomy: This surgery involves selectively severing a nerve from the affected inner ear organ, usually controls the vertigo while preserving hearing, but carries surgical risks.
PROGRESSION OF THE DISEASE
Progression of Meniere's is unpredictable: symptoms may worsen, disappear altogether, or remain the same.
Sufferers whose Meniere's began with one or two of the classic symptoms may develop others with time. Attacks of vertigo can become worse and more frequent over time, resulting in loss of employment, loss of the ability to drive, and inability to travel. Some patients become largely housebound. Hearing loss can become more profound and may become permanent. Some patients become deaf in the affected ear. Tinnitus can also worsen over time. Some patients with unilateral symptoms, as many as fifty percent by some estimates, will develop symptoms in both ears. Some of these will become totally deaf.
Yet the disease may end spontaneously and never repeat again. Some sufferers find that after eight to ten years their vertigo attacks gradually become less frequent and less severe; in some patients they disappear completely. In some patients, symptoms of tinnitus will also disappear, and hearing will stabilize (though usually with some permanent loss).
RESEARCH
Scientists are investigating environmental and biological factors that may cause Meniere's disease or induce an attack. They are also studying how fluid composition and movement in the labyrinth affect hearing and balance. By studying hair cells in the inner ear, which are responsible for proper hearing and balance, scientists are learning how the ear converts the mechanical energy of sound waves and motion into nerve impulses. Insights into the mechanisms of Meniere's disease will enable scientists to develop preventive strategies and more effective treatment.

CONSIDERATIONS
Some health care providers recommend a high-protein, low-refined carbohydrate diet because they have found that people with this disorder have high blood insulin levels. High insulin levels impair circulation.

Health care providers prescribe a variety of medications to combat the symptoms of vertigo and nausea. Promethazine (Phenergan) is usually prescribed to treat the nausea, vomiting, and vertigo symptoms. To reduce fluids in the system, diuretics may be prescribed. A low-salt diet may accompany this plan. Diazepam (Valium) is frequently prescribed to lessen anxiety and to sedate the vestibular system. For some people, health care providers prescribe antihistamines, and in some cases, steroids.
There may be a correlation between high calcium levels and the incidence of Meniere's disease. Health care providers in Europe have prescribed the calcium channel blocker medication flunarizine with positive results in combating vertigo. Flunarizine is not currently available in the United States.
If symptoms are severe and interfere drastically with daily life, surgery may be recommended. Surgical procedure options for Meniere's disease include the insertion of a small tube into the eardrum (a tympanostomy tube), enlarging the endolymphatic sac by drilling the bone surrounding it or draining the fluid to reduce pressure (endolymphatic sac decompression), placing a small tube into the endolymphatic sac to drain excess fluid (endolyphatic sac shunt), removal of the labyrinth or a portion of the labyrinth (surgical labyrinthectomy), cutting the vestibular nerve to destroy the balance function of the ear (vestibular nerve section surgery), placing ototoxic drugs (toxic antibiotics) in the ear to destroy the balance portion of the ear (chemical labyrinthectomy), or inserting drugs into the eardrum with a syringe by laser.
