DESCRIPTION

Memory is as natural to us as breathing. It is an ability we all have, yet rarely ever think of - unless we perceive that we are losing that ability. Aging is an inevitable process, and as we grow older, most people become somewhat forgetful, and more absent-minded. Memory lapses are an annoyance in themselves, but worse is the anxiety that often comes along with them. Some deterioration of cognitive functioning, including memory health, is considered to be a normal part of aging and can begin from as young as the mid-30s.

The degree to which this happens, however, is greatly affected by genetic composition, stress, and other health-related factors such as diet, exercise, and lifestyle. As we age, the brain may find it more difficult to make connections along its neural pathways. This is often part of the natural aging process which affects everyone to a greater or lesser extent. Information is misplaced as it is processed through the brain. As years pass, it is vital that we maintain a healthy nervous system to help bring about optimal brain function.

We begin asking ourselves if our memory lapses are a symptom of some other problem, such as midlife depression or arteriosclerosis. Probably the greatest fear provoked by lapses in memory is that of Alzheimer's disease, a progressive and debilitating disease that usually starts in midlife with minor defects in memory and behavior. Although this is a fairly common disorder among older people, it is important to realize that most memory lapses have nothing to do with Alzheimer's disease.

TYPES OF MEMORY LOSS

Dementia A condition of declining mental abilities, especially memory.
Vascular Demential loss of brain function due to a series of small strokes.
Alzheimer's Disease The type of dementia known as Alzheimer's disease was first described in 1907 by Alois Alzheimer.
Mild Cognitive Impairment (MCI) A general term most commonly used to describe a subtle but measurable memory disorder.





CAUSES OF MEMORY LOSS

Information is stored in different parts of your memory. Information stored in the short-term memory may include the name of a person you met moments ago. Information stored in the recent memory may include what you ate for breakfast. Information stored in the remote memory (called long-term memory) includes things that you stored in your memory years ago, such as memories of childhood.

When you are in your 20s, you begin to lose brain cells a few at a time. Your body also starts to make less of the chemicals your brain cells need to work. The older you are, the more these changes can affect your memory. Aging may affect memory by changing the way the brain stores information and by making it harder to recall stored information. Your short-term and remote memories are not usually affected by aging. But your recent memory may be affected. For example, you may forget names of people you have met recently. These are normal changes.

It is generally believed that advancing age brings an increasing likelihood of developing memory loss. Some mild memory problems can occur in normal older adults, but sometimes memory problems can be a sign of temporary confusion, called delirium, or more persistent memory loss, called dementia. It is difficult to predict whether a person who has mild problems with memory will go on to develop more severe memory loss, because dementia becomes more common as people age.

Problems with memory can have many causes, including medication side effects, strokes, head injury, infections, depression, thyroid disease and vitamin B-12 deficiency, so it is important to be aware of problems with memory and identify the causes that can be treated or prevented. Often caregivers are the first to notice that a family member is having problems with memory.

AGE ASSOCIATED MEMORY IMPAIRMENT

The mildest form of this malady is called Age-Associated Memory Impairment (AAMI). AAMI is characterized by one's perception of his or her own memory loss. An estimated 40 percent of Americans over the age of 65 experience AAMI. However, not all memory loss is attributable to aging. Occasional memory lapses, such as misplacing the car keys or forgetting something at the grocery store, are a natural part of life at virtually any age, and are not likely to precede serious memory loss. In fact, with proper diet, nutrition, and memory use (exercising the brain cells), the memory should remain sharp and active well into one's 90s or beyond.

INSUFFICIENT NUTRIENTS

One reason many people suffer from memory loss is an insufficient supply of necessary nutrients (especially the B vitamins and amino acids) to the brain. The life of the body is in the blood. It literally feeds and nourishes every cell within our bodies. The brain is surrounded by a protective envelope known as the blood-brain barrier, which allows only certain substances to pass from the bloodstream into the brain. If the blood is "thick" with cholesterol and triglycerides, the amount of nutrient-rich blood that can pass through the blood-brain barrier decreases. Over time, this can result in the brain becoming malnourished.

INSUFFICIENT OXYGEN

Sustaining an adequate oxygen-rich flow of blood to the brain, as well as a steady supply of nutrients to all brain cells, are important factors that determine our brain's efficiency in performing its vital tasks. This preempts cognitive issues such as memory loss and muddled thinking.

INSUFFICIENT NEUROTRANSMITTERS

In addition, the functioning of the brain depends upon substance called neurotransmitters. Neurotransmitters are brain chemicals that act as electrical switches in the brain and, through the functioning of the nervous system, are ultimately responsible for all the functions of the body. If the brain does not have an adequate supply of neurotransmitters, or the nutrients from which to make them, it begins to develop the biochemical equivalent of a power failure or a short circuit. If your mind goes blank when you are trying to recall a specific fact or piece of information, or it begins to plug into some other, irrelevant memory instead, it is likely that such a "short circuit" has occurred.

FREE RADICAL EXPOSURE There are numerous other factors involved in the deterioration of memory. One of the most important is probably exposure to free radicals, which can wreak enormous damage to the memory if unchecked.

SUBSTANCE ABUSE

Alcoholics and drug addicts often suffer a great deal from memory loss. Alcoholics are notorious for "blackouts" - huge memory gaps that occur even though they are conscious.

CONTRIBUTING ILLNESSES

Allergies, candidiasis, stress, thyroid disorders, and poor circulation to the brain may also be contributing factors. Hypoglycemia (low blood sugar) can play a role in memory loss as well, because to function properly, the brain requires that the level of glucose in the blood fall within a very specific narrow range. Wide swings in blood sugar levels affect brain function and memory.

SLEEP APNEA

The part of the brain that stores memory appears to shrink in people with sleep apnea, adding further evidence that the sleep and breathing disorder is a serious health threat. The findings, from brain scan studies conducted by researchers at the University of California, Los Angeles, show for the first time that sleep apnea is associated with tissue loss in brain regions that store memory. And while the thinking and focus problems of sleep apnea patients often are attributed to sleep deprivation, the scans show something far more insidious is occurring.

"Our findings demonstrate that impaired breathing during sleep can lead to a serious brain injury that disrupts memory and thinking," said principal investigator Ronald Harper, Professor of Neurobiology at the David Geffen School of Medicine at U.C.L.A. The data appear in the June 27 issue of the journal Neuroscience Letters.

Sleep apnea occurs when the muscles in the throat, soft palate and tongue relax during sleep. They sag and narrow the airway and the tongue slides to the back of the mouth, blocking the windpipe and cutting off oxygen to the lungs. The sleeper gasps for air, wakes up briefly and falls back to sleep in a cycle that repeats itself hundreds of times per night. The result is loud snoring and chronic daytime fatigue. The disorder also is linked to a higher risk of stroke, heart disease and diabetes. An estimated 20 million Americans have sleep apnea. The study focused on structures on the underside of the brain called mammillary bodies, so named because they resemble small breasts. The researchers used magnetic resonance imaging to scan the brains of 43 sleep apnea patients. Compared to images of 66 control subjects, the brains of the sleep apnea patients had mammillary bodies that were nearly 20 percent smaller, particularly on the left side. The structures also are known to shrink in patients who have other forms of memory loss related to alcoholism or Alzheimer's disease.

The researchers do not know why the sleep disorder affects brain tissue but theorize that it's related to repeated drops in oxygen. During an apnea episode, the brain's blood vessels constrict, starving its tissue of oxygen and causing cells to die. The inflammatory process, also linked with heart disease and stroke, further damages the tissue. "The reduced size of the mammillary bodies suggests that they have suffered a harmful event resulting in sizable cell loss," Dr. Harper said. "The fact that patients" memory problems continue despite treatment for their sleep disorder implies a long-lasting brain injury." The data show the importance of early diagnosis and treatment of sleep apnea. Unfortunately, the most effective treatment is a continuous positive airway pressure, or CPAP, machine that many patients find unwieldy and uncomfortable. In a future study, the U.C.L.A. researchers will explore whether vitamin B-1 supplements might help restore memory in sleep apnea patients by moving glucose into cells and preventing cell death.

ALZHEIMER'S DISEASE

Alzheimer's disease starts by changing the recent memory. At first, a person with Alzheimer's disease will remember even small details of his or her distant past but not be able to remember recent events or conversations. Over time, the disease affects all parts of the memory.

A memory problem is serious when it affects your daily living. If you sometimes forget names, you are probably okay. But you may have a more serious problem if you have trouble remembering how to do things you have done many times before, getting to a place you have been to often, or doing things that use steps, like following a recipe.

Another difference between normal memory problems and dementia is that normal memory loss does not get much worse over time. Dementia gets much worse over several months to several years. It may be hard to figure out on your own if you have a serious problem. Talk to your family health care provider about any concerns you have. Your health care provider may be able to help you if your memory problems are caused by a medicine you are taking or by depression.

  • Hypertension. A large, preliminary study in 1998 found associations between hypertension and deterioration in mental function.

    THINGS TO HELP YOU REMEMBER
    • Keep lists.
    • Follow a routine.
    • Make associations (connect things in your mind), such as using landmarks to help you find places.
    • Keep a detailed calendar.
    • Put important items, such as your keys, in the same place every time.
    • Repeat names when you meet new people.
    • Do things that keep your mind and body busy.
    • Run through the ABC's in your head to help you think of words you are having trouble remembering. "Hearing" the first letter of a word may jog your memory.

    LIFESTYLE FOR IMPROVING MEMORY & CONCENTRATION

  • Exercise. Exercise has beneficial effects on and cognitive functioning and well-being in older people. Exercisers show significant improvements in reaction time, memory span, and measures of well-being when compared with non-exercisers. Going for walks may be enough to modify the usual age-related decline in reaction time. Women with higher levels of physical activity were less likely to develop cognitive decline. For every one mile walked per week, a woman can reduce her risk of significant loss of mental function by 13 percent.


  • Linguistic Ability. Low linguistic ability in early life was a strong predictor of poor cognitive function and Alzheimer's disease in late life.

  • DIAGNOSIS

    If you have trouble with your memory, you should discuss the problem with your health care provider. Tell the health care provider the specific problems, when they occur, any recent changes in your home environment, and all the medicines you are taking, including non-prescription and herbal or "natural" remedies; also let him or her know if you drink alcohol. Tell your health care provider about when the problems first started, and whether the problems have been the same, worse, or better over time. Any fever, chills, pain, weight loss or gain, or recent changes in other medical conditions should be discussed with your health care provider. Any problems with depressed mood should be discussed, including problems with your appetite or sleep disturbances. Be sure to tell your health care provider if you have had any falls or blackout spells or if you are no longer able to care for yourself or socialize or do errands like you have in the past.

    After discussing these issues and performing a physical examination, your health care provider may perform a screening memory test that includes basic questions regarding memory. The result of the screening test may be normal, borderline, or abnormal. If the memory test is normal or low-normal, the health care provider may suggest that you keep track of any memory problems over the next few months and repeat the test after six months or so. Or the health care provider may suggest another type of memory testing called neuropsychological testing. This type of testing is similar to the memory screen but includes many more questions and tasks. The purpose of this test is to find out if the problems with memory are not severe enough to show up on the screening test, but are apparent in the more in-depth test. If the neuropsychological testing is normal, then the problems with memory are likely the very mild memory problems that may occur with aging. If the test is borderline, the results can help your health care provider decide if additional testing might be important to look for a cause of memory loss.

    If the memory test is abnormal, your health care provider will talk with the person and caregiver, if involved, about evaluating possible causes of memory loss. Longer-term memory loss may be caused by a number of conditions including Alzheimer's Disease and vascular disease in the brain (small strokes in the brain), and less commonly, by thyroid disease, a low vitamin B-12 level, or inadequately treated syphilis. In addition to the history of the memory problems and the physical examination, blood tests, and sometimes imaging studies of the brain, such as CT (computerized tomographic) or MRI (magnetic resonance imaging) are necessary to detect most of these illnesses.

    If no other explanation for the memory loss is found, the most common cause for memory loss is Alzheimer's Disease. With the tests currently available, the only way to know for certain that a person has Alzheimer's Disease is by taking a small sample of the brain, called a biopsy. This test is very rarely done, so Alzheimer's Disease can only be presumed in most people. Researchers are trying to develop new tests to diagnose Alzheimer's Disease, but so far no tests are reliable enough to use for diagnosis.

    If Alzheimer's Disease is the most likely diagnosis, you and your family members may benefit from information provided by organizations about what to expect with Alzheimer's Disease and about the support services that are available. No treatments that we know of will cure Alzheimer's Disease. One treatment may help some of the symptoms of memory loss, but the drugs, called acetylcholinesterase inhibitors (tacrine [Cognex], donepezil, [Aricept]) have common side effects of nausea and diarrhea, and one drug, tacrine can cause reversible liver damage. These drugs can improve memory equal to about four to six months of memory decline, but memory will nonetheless continue to decline after the drug is started, and if the drug is stopped, the memory loss will become as severe as if the drug had never been started.

    Many researchers are studying ways to detect, treat and prevent Alzheimer's Disease.     DIETARY & LIFESTYLE RECOMMENDATIONS

    OVERVIEW

    To maintain good health and address normal age-related forgetfulness and absentmindedness, it is important to eat sufficient amounts of fresh fruits, vegetables, and whole grain foods. It is also beneficial to get sufficient fresh air and sunshine, as well as exercise on a regular basis. Also avoid smoking, excessive alcohol intake, and other drugs.

    Natural memory tonics have been used in traditional medicine for thousands of years to support the healthy functioning of the brain and nervous system, helping to maintain normal and efficient concentration and memory functioning. In more recent times, research has confirmed this traditional wisdom.

    DIET & NUTRITION FOR IMPROVING MEMORY & CONCENTRATION

  • A health diet is important. A healthy diet is associated with better cognitive performance in the elderly. Eat a diet high in raw foods. A diet high in fruits, vegetables, and fiber was found to be associated with less age-related brain-function impairment. The greatest benefit was found to be associated with high fiber consumption. Consume the following often: Brewer's Yeast, brown rice, farm eggs, fish, legumes, millet, nuts, soybeans, tofu, wheat germ, and whole grains.


  • High intake of monounsaturated fatty acids (e.g., olive oil) has been associated with protection against age-related cognitive decline.


  • Diets high in antioxidant-rich foods, such as spinach and strawberries (and most other fruits and vegetables), may be beneficial in slowing age-related cognitive decline. Eating more blueberries and spinach and other foods containing flavonoids high in antioxidants may aid in memory retention, according to some researchers.


  • Combine complex carbohydrates with foods containing 10 percent protein and 10 percent essential fats.


  • Make sure you are getting the correct amounts of amino acids, antioxidants, B vitamins, choline, coenzyme Q-10, and pregnenolone, either from the diet or through supplementation.


  • Avoid dairy and wheat products (except for wheat germ) for one month. If there is no memory improvement, slowly add these foods back into your diet.


  • Avoid refined sugars. These types of sugars "turn off" the brain.


  • IMPORTANT NUTRIENTS FOR BRAIN FUNCTION

  • Vitamin C is an antioxidant that reduces free radical damage in the body. Several studies with elderly subjects indicate that individuals who ingest higher amounts of vitamin C have better cognitive function and memory performance, which suggests that vitamin C may protect against cognitive decline.


  • Vitamin E is another antioxidant nutrient that reportedly protects against subsequent development of dementia and poor cognitive functioning, probably due to its protection against free radical damage to the blood vessel system in the brain. Antioxidant effects of Vitamin E are effective in slowing age-related cognitive decline. The amount of vitamin E in the blood is associated with memory performance. A study of patients with moderately advanced Alzheimer's disease indicated that vitamin E may slow functional deterioration (functional abilities last longer).


  • Among people aged 65 and older, higher beta carotene levels in the blood have been associated with better memory performance.


  • Several studies suggest that acetyl-L-carnitine delays onset of age-related cognitive decline and improves overall cognitive function in the elderly. ALC protects against brain degeneration, helps with energy production in mitochondria of cells, and removes toxins from the mitochondria. In one study, acetyl-L-carnitine was given to elderly people with mild cognitive impairment. After 45 days, significant improvements in cognitive function (especially memory) were observed. Another large trial of acetyl-L-carnitine for mild cognitive impairment in the elderly found that supplementation significantly improved memory, mood, and responses to stress. The favorable effects persisted at least 30 days after treatment was discontinued.


  • Phosphatidyl Serine (PS) is the most abundant phospholipid in the brain, and it plays critical roles in maintaining both the structure and functionality brain cells. PS encourages the regrowth of damaged nerve networks. Low levels of phosphatidyl serine in the brain are associated with impaired mental function and depression in the elderly. Supplementation with PS consistently benefits memory, learning, concentration, word choice, and other measurable cognition parameters, as well as mood and the capacity to cope with stress. Numerous studies have documented phosphatidyl serine's ability to improve memory, learning, concentration, word recall, and mood in middle-aged and elderly subjects with dementia or age-related cognitive decline.


  • Vitamin B-6 (pyridoxine) deficiency is common among people over age 65. A study of healthy men, aged 70 to 79 years, showed that supplementation with pyridoxine for 3 months improved memory performance, especially long-term memory.


  • Vitamin B-12 supplementation may improve cognitive function in elderly people who have been diagnosed with a B-12 deficiency. Cognitive impairment is an important manifestation of vitamin B-12 deficiency. Cognitive decline due to low levels of vitamin B-12 is a greater problem in elderly individuals since cobalamin deficiency increases with advancing age. Supplementation with vitamin B-12 showed improvements in cognitive function among elderly people with vitamin B-12 deficiency and cognitive decline, even in people without obvious signs of B-12 deficiency.


  • Nicotinic Acid supplementation results in improvement of sensory register and short-term memory, and long-term memory.


  • Melatonin is a hormone secreted by the pineal gland in the brain. Melatonin is partially responsible for regulating sleep-wake cycles. Cognitive function is linked to adequate sleep and normal sleep-wake cycles. Melatonin supplementation benefits cognitive function, possibly due to its ability to prevent sleep disruptions. A study of elderly patients with mild cognitive impairment showed that melatonin significantly improved sleep, mood, and memory.


  • Short-term testosterone supplementation improved cognitive function in healthy older men.


  • Docosahexaenoic Acid (DHA) is an omega-3 fatty acid that is essential for proper growth and functional development of the brain in infants, and DHA is also required for maintenance of normal brain function in adults. Adequate levels of DHA in the diet improves learning ability, whereas deficiencies of DHA are associated with deficits in learning. Also, in adults, decreases in DHA in the brain are associated with cognitive decline during aging.

  • Bee pollen may also be helpful. Caution: Bee pollen may cause an allergic reaction in some people. Start with a very small amount. Discontinue use if a rash, wheezing, discomfort or other symptoms appear. If you have known allergies to pollen, avoid it completely unless taken under the close supervision of a health care provider or allergist.

  • Make sure to focus on things that you may wish to remember, and be sure to give them meaning. Often, we blame our inability to recall something on a failing memory when the problem is that we did not really pay attention in the first place.


  • Keep yourself mentally active by using your brain on a daily basis. Pursue brain exercising activities such as reading, doing crossword and other types of puzzles, surfing the internet, or playing a mentally challenging game. Research has shown that the more you put your memory to use, the more vital it will be.

  • Keep yourself physically active (but do not overdo it). This increases blood flow to the brain. One study of older women indicated that those who engaged in moderate exercise, such as walking at least 6 hours per week, had a 20 percent decrease in risk of cognitive impairment as compared with those who were inactive. They also demonstrated cognitive functioning of someone 3 years younger, on average, than their actual age. Exercise reduces stress and elevates mood by releasing endorphins in the brain that make you feel good, which also has a positive effect on your immune system. In addition, T-lymphocyte production is stimulated by exercise. Walking is a good form of exercise. Taking Tai Chi or Yoga provides exercise, stretching, and movement as well as focusing and calming techniques for the mind and spirit.
