DESCRIPTION
Intertrigo is a type of dermatitis or skin inflammation caused by repeated friction of skin rubbing against skin. It can appear anywhere two skin surfaces lie next to each other and rub together, but most often occurs in the skin folds of the groin, inner thighs, and underarms, and between the ribs, underside of the belly and the breasts. This condition is most common in warm climates and during the summer.
SYMPTOMS
Intertrigo appears as an intensely reddish, macerated, glistening rash that may be sore or itchy. Maceration of the skin occurs when it is consistently wet. The skin softens, turns white, and can easily get infected with bacteria or fungi. The edge of the rash extends just beyond the limits of the opposing skin folds. Satellite lesions, small areas of the same rash that are close to the main rash, are characteristic of intertrigo and other Candida skin infections. It usually progresses gradually, beginning as a mild chafing, then slowly, with continued exposure to moisture and friction, develops into a persistent itchy rash. Occasionally, a secondary infection (bacterial or fungal [yeast/Candida albicans]) may develop, causing the formation of pustules and weeping and oozing of the skin, as well as severe itching and pain. Severe intertrigo on the groin or thighs can impede mobility.
CAUSES
Intertrigo is usually yeast infection of skin folds caused by Candida albicans. In areas of the body that have skin touching skin such as the armpits, groin, and under heavy breasts or fat folds, the environment is warm and moist. This is the perfect environment for Candida albicans, a yeast that is normally found on the skin, to overgrow and cause symptoms. Other factors that can cause intertrigo include:
- Hot, humid weather.
- Tight or abrasive underclothing.
- Poor hygiene.
- Inflammatory skin diseases, like psoriasis, occurring in the skin folds.
- The use of topical steroids.
RISK FACTORS
Intertrigo primarily affects overweight women who perspire heavily. People with diabetes also are more disposed than others to developing intertrigo and accompanying secondary infections. Persons who suffer from urinary incontinence are at increased risk of developing intertrigo in the groin area. there are also several skin diseases that can cause an intertrigo to develop (inverse psoriasis, Haily-Haily, pemphigus, bullous pemphigoid, glucagonoma and others.

PREVENTION
Prevention of intertrigo involves reducing skin to skin contact and keeping the area dry. Other factors that may help preventing intertrigo include:
- Weight loss to reduce the size of fat folds.
- Wear loose, soft underclothing.
- Apply antifungal absorbent powder to affected areas daily.
- Wash affected areas with soap and water daily.
Some women will place a clean, folded, dry absorbent, natural fiber cloth under each of their breasts to help absorb extra moisture and prevent skin on skin contact during hot weather days.
TREATMENT
MEDICAL TREATMENT OF INTERTRIGO
DIAGNOSIS
The majority of cases of intertrigo can be diagnosed based on the characteristic appearance. This should not be confused with other fungal infections such as jock itch, ringworm, or athlete's foot. If there is any question about the diagnosis, a KOH test can be performed to detect the presence of yeast.
A KOH test is used to diagnose fungal infections of the skin or nails. The skin or nail is scraped with a scalpel or glass slide causing dead skin cells to fall off onto a glass slide. A few drops of Potassium hydroxide (KOH) are added to the slide and the slide is heated for a short time. The KOH dissolves the material binding the skin cells together releasing the fungus. The slide is then viewed under the microscope looking for fungal elements.
A bacterial culture can help diagnose a secondary bacterial infection. A bacterial culture is used to determine if a rash is caused by a bacteria, the type of bacteria, and what antibiotics the bacteria is sensitive to. A bacterial culture is performed by getting a sample of pus onto a special cotton swab that is later rubbed onto a sterile Petri dish. The petri dish is incubated to allow bacteria from the sample to grow. If after 72 hours, there is no growth on the petri dish, there are no bacteria present. If bacteria are present, a sample is taken for gram stain to determine the type of bacteria. If bacteria do grow, antibiotic disks can be placed in the middle of the growth. If the bacteria around the disk die, the bacteria is sensitive to that antibiotic.
TREATMENT
Yeast (fungal) or bacterial infection can be treated with topical or oral medications. Intertrigo is treated with antifungal creams such as clotrimazole and miconazole.
Oozing may be controlled with moist compresses with Burow's solution 1:20 (1 part Burow's Solution mixed with 20 parts of water) followed by air drying, preferably using a hair dryer with a "cool" setting. Burow's compresses can be applied to the skin folds for 20 to 30 minutes several times a day to promote healing. Burow's Solution is an aluminum acetate topical solution and can be purchased at most chemical supply outlets such as Post Apple Scientific. It is also available online from Amazon.com.
Prescription topical steroid creams such as hydrocortisone can be used, but only for a few weeks. Stronger topical steroid creams will be more rapidly effective, but if used for more than a few days may cause serious skin damage.
Antibiotics may be prescribed by your health care provider if intertrigo is complicated by a fungal (yeast) or bacterial infection. If you must take antibiotics, you should be sure to take some form of acidophilus to replace the necessary "friendly" bacteria in the intestinal tract.
If itching is severe, an antihistamine may be prescribed.
Keeping the area dry and exposed to the air can prevent recurrences. If obese, weight loss is helpful. A wad of soft absorbent cotton, or a band of cotton fabric will help absorb sweat. Antiperspirants will help if excess sweating is part of the problem but should only be tried when it has completely healed. It also helps to wash daily with an antibacterial soap and dust with Zeasorb AF Powder.
Relapses are common with this condition, and revisits periodically to the dermatologist may be needed to adjust treatments in many cases.
HOLISTIC & NUTRITIONAL RECOMMENDATIONS
Good body hygiene is important. Keep the skin surface clean, dry, and free of friction. Shower or bathe immediately following periods of heavy perspiration (sweating), and use a hair dryer on a cool setting instead of rubbing with a towel to dry the skin infected. Then apply baby powder. Do this several times daily, or as often as needed.
Use only natural, chemical-free soaps, deodorants, and other personal care preparations. Most health food stores carry such products. Avoid commercial products, which may contain substances that can further irritate the inflamed areas.
Wear loose-fitting 100 percent cotton clothing that does not rub against the skin. Many intertrigo sufferers find relief by remaining unclothed whenever possible.
Avoid sitting in one position for prolonged periods.
Avoid sugar and other refined carbohydrates. Both bacteria and fungi (yeast) thrive on sugar.
Add fresh garlic, soured products, and acidophilus to the diet.
