DESCRIPTION

When part of an organ bulges out of the area where it is not supposed to be, it is called a hernia. Hiatal hernia is a condition in which the stomach pushes or herniates, upward through an opening in the diaphragm into the thorax (chest cavity). Both children and adults may have a hiatal hernia, but it is more common in adults.

Usually a result of a congenital abnormality or trauma, this condition is associated with gastroesophageal reflux, in which the muscle that encircles the juncture of the stomach and the esophagus fails to keep food and acid from coming back up from the stomach into the esophagus as it should. If that happens, the tissues in the esophagus become irritated, causing heartburn and, some times, a coughing up of bloody mucus.





TYPES OF HIATAL HERNIA

The abdomen and chest are separated by a sheet-like muscle called the diaphragm. The esophagus goes through an opening (the hiatus) in the diaphragm to connect to the stomach. The upper stomach bulges through this opening to create a hiatal hernia. There are 3 types of hiatal hernia: sliding, paraesophageal, and mixed. The most common type is the sliding hiatal hernia and is generally not a serious condition. Paraesophageal and mixed hiatal hernias may progress and are more serious.

SLIDING HIATAL HERNIA

In a sliding hiatal hernia, part of the stomach moves through the diaphragm so that it is positioned outside of the abdomen and in the chest. The lower esophageal sphincter (LES) often moves up above its normal location in the opening of the diaphragm.

Most people with a sliding hiatal hernia have no symptoms, and it is often diagnosed when a person is being evaluated for other health concerns. However, if the LES moves above the diaphragm, it may not close well and stomach acid and juices may back into the esophagus (acid reflux). This results in an uncomfortable feeling of burning, warmth, heat, or pain behind the breastbone. This may occur after eating, soon after lying down, or when bending forward, and may come and go. You may also have a sour taste in your mouth. This group of symptoms is commonly known as heartburn or reflux.

PARAESOPHAGEAL HERNIA

In a paraesophageal hernia, the stomach bulges up through the opening in the diaphragm (hiatus) alongside the esophagus (upside-down stomach). The LES remains in its normal location inside the opening of the diaphragm. This type of hernia most commonly occurs when there is a large opening in the diaphragm next to the esophagus.

The stomach and, rarely, other abdominal organs (such as the intestine, spleen, and colon) may also bulge into the chest in a paraesophageal hernia.

MIXED HERNIA

In a mixed hiatal hernia, the LES is above the diaphragm as in a sliding hiatal hernia, and the stomach is alongside the esophagus as in a paraesophageal hiatal hernia.

An estimated 50 percent of people over forty years of age have hiatal hernias. However, many people are unaware of the condition. Small hernias rarely cause any trouble. It is the larger hernias that are most often linked to reflux problems. Ulcers often accompany a hiatal hernia. The acid reflux may lead to ulceration of the esophagus. Ulcers can also occur in the duodenum (the top of the small intestine) or the stomach.





SIGNS & SYMPTOMS

Most people with a sliding hiatal hernia have no symptoms, and it is often diagnosed when a person is being evaluated for other health concerns. However, if the LES moves above the diaphragm, it may not close well, and stomach acid and juices may back into the esophagus (acid reflux). This results in an uncomfortable feeling of burning, warmth, heat, or pain behind the breastbone (sternum). This may occur after eating, soon after lying down, or when bending forward, and may come and go. You may also have a sour or bitter taste in your mouth and belching. This group of symptoms is commonly known as heartburn or reflux. Coughing up of bloody mucus may occur when the esophagus become irritated.

Other symptoms may include:
  • Fullness in the upper abdomen after a meal.
  • Difficulty swallowing (dysphagia) or a sensation of a lump in the throat (globus sensation).
  • Regurgitation (not reflux) of stomach juices and nausea.
  • A hoarse voice.

If symptoms are continual or severe, you may have gastroesophageal reflux disease (GERD). Treatment for GERD usually requires stronger prescription medications, and surgery may be an option.

CAUSES

  • A congenital abnormality, trauma, or surgery may cause hiatal hernia.


  • A hiatal hernia may develop or worsen as a person ages (over 40 years).


  • Dietary differences have been implicated since it is very common in the west and rare in Asia and Africa.


  • Genetics may play a role in the causation. Clustering of hiatal hernia among family members encompassing several generations has been described.


  • Other theories implicate the weakening of the ligaments attaching the stomach to various adjacent structures. This weakened system permits the stomach to be pushed up into the chest. Such a situation may occur due to pregnancy, obesity, and aging, etc.


  • Some experts believe that the hiatal hernia occurs because the esophagus is shortened, thus pulling the stomach into the chest. The shortening occurs because of malfunctioning of nerves in the lower part of the esophagus.





  • POTENTIAL PROBLEMS

    Just because you have a hiatal hernia does not mean that you have a problem at hand. In fact, they don't cause any problem in most people. In many patients however, acid reflux may worsen.

    Patients with hiatal hernia tend to have greater acid induced damage to the esophagus. The sliding of stomach in and out of abdomen may cause friction at the neck of the hernia leading to ulcers. Giant hernias may cause anemia, bleeding and chest pain. Paraesophageal hernias are prone to strangulation.




    TREATMENT


    DIAGNOSIS & TESTS

    A hiatal hernia often is diagnosed when a person is being evaluated for another health concern. The diagnosis is usually made during an upper GI x-ray or upper endoscopy. If you go to your health care provider with a feeling of burning, warmth, heat, or pain behind the breastbone (often known as heartburn), your health care provider will probably ask you questions about your symptoms and may want to perform other tests if any of the following is true:
    • You have had heartburn symptoms for a long time.
    • Your symptoms do not get better with medication.
    • Your symptoms promptly come back after medication is stopped.
    • Other serious signs or symptoms are present, such as weight loss, vomiting of blood, or difficulty swallowing.




    CONVENTIONAL MEDICAL TREATMENT

    Treatment for people who have mild symptoms begins with making lifestyle changes and, if necessary, taking non-prescription antacids or acid reducers. If symptoms are continual or severe, you may have gastroesophageal reflux disease (GERD). Treatment for GERD, paraesophageal or mixed hiatal hernia, usually requires stronger prescription medications, and surgery may be necessary to avoid potential complications.

    GASTROESOPHAGEAL REFLUX DISEASE (GERD)

    Gastroesophageal reflux disease (GERD) is the abnormal backflow (reflux) of food, stomach acid, and other digestive juices into the esophagus. This backflow occurs when the valve between the lower end of the esophagus and the stomach (the lower esophageal sphincter) does not close tightly enough or relaxes at the wrong times.

    The main symptom of GERD is heartburn. In GERD, the reflux (and heartburn) lasts longer and occurs more often than ordinary heartburn. Over time, acid reflux may cause irritation and inflammation of the esophagus (esophagitis), sores (ulcers) in the esophagus, and narrowing of the esophagus (stricture).

    Home treatment with antacids and non-prescription acid controllers can often ease the symptoms of mild GERD and may help prevent it in some situations. It may help to eat small, frequent meals and avoid food for at least 2 hours before lying down. It also helps to avoid smoking and foods that aggravate your heartburn.

    Prescription medications may be needed for more severe or frequent symptoms.

    A person may have a hiatal hernia or GERD or both. A person with a hiatal hernia may not always have GERD, and many people with GERD do not have a hiatal hernia. If a person has heartburn, certain tests, such as an upper gastrointestinal series or endoscopy, can help determine if the person has a hiatal hernia and/or GERD.

    The need for surgery for hiatal hernia is rare. It is indicated in cases of persistent ulcers and bleeding due to the hernia; very large or symptomatic sliding type hernias and; paraesophageal hernias. The surgeon pulls down the hernia into the abdomen and hooks it up to adjacent structures so the stomach won't escape again into the chest. Usually an anti-reflux procedure is also performed.

    HOME TREATMENT

    If you have a sliding hiatal hernia, no treatment generally is necessary. If you have occasional or mild symptoms - a feeling of burning, warmth, heat, or pain behind the breastbone (commonly known as heartburn) - you can make the following lifestyle changes:
    • If you smoke or chew tobacco, stop. Nicotine from tobacco relaxes the valve between the esophagus and stomach (lower esophageal sphincter, or LES). This can allow stomach acid and juices to back up (reflux) into the esophagus, which cause the symptoms of heartburn.


    • Change your eating habits. This may make heartburn less likely to occur. Take your spouse or partner along with you when you go to discuss diet habits with your health care provider. It will be easier to make changes in your diet if your family understands what you need to do and why.


    • Lose weight if you are overweight. Being overweight puts additional pressure on your stomach and increases the likelihood that heartburn will occur. Even losing a few pounds can help.


    • Avoid or reduce pressure on your stomach to make heartburn less likely to occur.


    • Raise the head of your bed 6 inches (15.24 cm) to 8 inches (20.32 cm) by putting blocks underneath your bed frame or placing a foam wedge under the head of your mattress. This will help keep stomach acid from flowing into your esophagus when you are sleeping. Using extra pillows does not work, because they cause you to bend at your waist. This squeezes your stomach, making heartburn worse.

  • Along with making lifestyle changes, non-prescription medications may control mild or occasional heartburn. These medications include acid reducers and antacids.





  • NUTRITION, HOLISTIC RECOMMENDATIONS, HERBS & SUPPLEMENTS


    NUTRITION AND LIFESTYLE RECOMMENDATIONS

  • At the first sign of heartburn, drink one or two large glasses of water. This often relieves heartburn by washing the acid out of the esophagus.


  • Even if you are not thirsty, drink a large glass of water every three hours during the daytime.


  • Eat several small meals daily. Eat 6 small meals instead of 3 big meals. This keeps the stomach from getting too full. Eat slowly. Don't lie down for 2 to 3 hours after eating. Do not drink anything 1 to 2 hours before going to bed.


  • Include extra fiber in the diet.


  • Do not consume spicy foods, and do not take enzyme supplements that contain hydrochloric acid (HCl).


  • Avoid fats/fatty foods and fried foods. They delay digestion and prolong the stomach's emptying time. Also avoid caffeine beverages (coffee, tea, colas, cocoa), citrus fruits and other foods and drinks that seem to increase heartburn.


  • Avoid alcohol and smoking. Smoking often causes the stomach to make more acid.


  • Avoid heavy lifting and bending. Give the stomach at least two hours of empty before lifting or other exertion. Bend from the knees, not the waist, to avoid abdominal pressure. Avoid straining when lifting things.


  • Do not wear clothing that is tight around the waist, stomach, or chest.


  • Avoid becoming constipated and straining during bowel movements or when urinating.


  • Do not eat within three hours of bedtime. If you are regularly bothered by indigestion during the night, try raising the bed of your bed six to ten inches.


  • Do not lie down during the day if you are bothered by heartburn. Sitting or standing helps to keep the stomach acids in the stomach.


  • Because heartburn causes symptoms similar to those of heart disease, see a health care provider if you have more than occasional bouts to rule out the possibility of heart problems.

  • Allergenic foods often magnify the symptoms and prolong healing. Symptoms of hiatal hernia often clear up when allergenic foods are avoided. See Allergies for more detailed information.


  • Gastroesophageal reflux can also be caused by other factors as well as hiatal hernia, such as smoking or eating too close to bedtime. Obesity can also contribute to the problem.





  • HERBS

  • Aloe Vera promotes healing of the intestinal lining. Drink 1/4 cup of aloe vera juice in the morning and again at night. It can be added to juice or herbal tea if you desire.


  • Other beneficial herbs include Fenugreek, Goldenseal, Marshmallow Root, Red Clover, and Slippery Elm. Caution: Do not take Goldenseal internally on a daily basis for more than one week at a time, do not use during pregnancy, and use it with caution if you are allergic to ragweed.

  • You have symptoms of heartburn that worsen or do not improve with treatment and lifestyle changes.


  • You have severe upper abdominal pain.


  • You develop a high temperature.

    Seek Medical Care Immediately If...

  • You vomit blood or have recurrent vomiting.

  • You have chest pain that is crushing or squeezing, feels like a heavy weight on the chest, or occurs with any other symptoms of a heart attack. Note: Symptoms of a heart attack and symptoms of heartburn may feel the same. It is important to consider all your other symptoms to determine if you have a more serious problem.


  • You have signs of shock. Shock is a life-threatening condition. Immediate medical care can make the difference between life and death.
    • Early signs of shock (most of which will be present) include:
      • Lightheadedness or a feeling that you are about to pass out.
      • Restlessness, confusion, or signs of fear.
      • Shallow, rapid breathing.
      • Moist, cool skin or possibly profuse sweating.
      • Weakness.
      • Thirst, nausea, or vomiting.

      Also, a person in shock usually has an abnormal increase in heart rate and an abnormal decrease in blood pressure.

      Late signs of shock include:
      • Changes in the person's level of consciousness, such as sudden unresponsiveness, faintness, loss of consciousness, or behavior changes.
      • A pale face or blue lips or earlobes.

      Shock may occur in response to a sudden illness or injury. When the body loses too much blood or fluids, the circulatory system cannot get enough blood to the vital organs, and shock results.

  • If the person has chest pain, after calling 911 or other emergency services:
    • If the person becomes unconscious, see Rescue Breathing and Cardiopulmonary Resuscitation (CPR) in the topic Dealing With Emergencies.


    • If emergency medical personnel (such as paramedics) are not going to arrive soon and the person is awake, alert, not vomiting, and not allergic to aspirin, have the person chew and swallow 1 adult aspirin (325 mg). Be sure to tell emergency personnel that aspirin has been taken.


    • Life-threatening symptoms occur, including rapid, irregular heartbeat; chest pain; or loss of consciousness. Call for assistance immediately (usually 911)... THIS IS AN EMERGENCY!! Quick treatment to open the blocked artery is essential to lessen the amount of damage. At the first signs of a heart attack, call for emergency treatment. The best time to treat a heart attack is within one to two hours of the first onset of symptoms. Waiting longer than that increases the damage to your heart and reduces your chance of survival. Keep in mind that chest discomfort can be described in many ways. It can occur in the chest or the arms, back, or jaw. If you have symptoms, take notice. These are your heart disease warning signs. Seek medical care immediately.


    • If any unexpected or unusual symptoms appear. Therapies, supplements, medications or other treatments may produce side effects in some individuals.
