DESCRIPTION
Diverticulitis is a condition in which the mucous membranes lining the colon become inflamed, resulting in the formation of small, pouch-like areas called diverticula usually in the intestines. The diverticula appear as sac-like, pea- or grape-sized protrusion in the intestinal wall that are perforated and become infected and inflamed. The pouches can develop anywhere on the digestive tract, but they most commonly form at the end of the descending and sigmoid colons, and the also occur frequently on the first section of the small intestine (although they rarely cause problems there). Once diverticula develop, they do not go away. The diverticula themselves cause no symptoms, but if waste matter becomes trapped in them, they can become infected or inflamed, causing fever, chills, and pain.
CAUSES
Diverticulitis typically forms when an individual is constipated. Eating a low-fiber diet, as is typical in industrialized countries such as the United States, may contribute to the development of diverticulitis. Without sufficient fiber to soften and add bulk, stools are harder to pass. Greatly increased pressure is required to force small portions of hard, dry stool through the bowel. This rise in pressure can cause pouches to form at weak points in the wall of the colon. Once diverticula develop, they do not go away.
Because the walls of the large intestine often weaken as a person ages, this is a condition affecting older rather than younger people. It usually strikes people between the ages of 50 and 90. It affects millions of American, but many people do not even know they have the condition because they either experience no symptoms or accept their symptoms as simple indigestion.
Exactly why is not known, but it is known that smoking and stress make symptoms worse. This is a classic example of a stress-related disorder.
Poor eating habits compound the problem. A poor diet, a family history of the disease, gallbladder disease, obesity, and coronary artery disease all increase the chances of developing diverticulitis.
FREQUENT SIGNS & SYMPTOMS
It usually strikes people between the ages of 50 and 90 years. It affects millions of Americans, but many people do not even know they have the condition because they either experience no symptoms or accept their symptoms as simple indigestion.
Diverticulitis may be acute or chronic. The acute form can manifest itself with one or more severe attacks of infection and inflammation. In chronic diverticulitis, inflammation and infection may subside, but they may never clear up completely.
The inflammation of diverticulitis can eventually result in a bowel obstruction, which is indicated by constipation, thin stools, diarrhea, abdominal distension (bloating), and abdominal pain cramping and tenderness on the left side of the abdomen that is relieved by passing gas or a bowel movement). If the obstruction persists, abdominal pain and tenderness will increase, and you may experience nausea and vomiting, constipation or diarrhea and an almost continual need to eliminate.
It may involve anything from a small abscess in one or more of the pouches to abscesses forming around the infected diverticula. If these abscesses go through the intestinal wall, a massive infection in the lining of the abdominal cavity (peritonitis) which can develop. If diverticulum ruptures with perforation of the bowel, the intestinal contents flow into the abdomen. Developing peritonitis is a potentially fatal infection that requires immediate treatment.
The symptoms are similar to those of appendicitis, except the location of the pain may differ. If left untreated, diverticulitis can lead to serious complications requiring extensive surgery. Sometimes scarring can occur, leading to a stricture or blockage of the intestine from the infection. An infected diverticulum can also reach an adjoining organ and form a connection, or fistula, between them. This most frequently occurs between the neighboring kidneys. Another potential complication of diverticulitis is severe internal and colonic bleeding.
- Cramping, sometimes severe abdominal pain that is usually worse on the left side.
- Tenderness on the left side of the abdomen that is relieved by passing gas or a bowel movement. Pain in the lower left quadrant of the abdomen that increases when the area is touched.
- Bloating.
- Constipation, thin stools, or diarrhea.
- Nausea.
- An almost constant need to eliminate.
- Chills or fever.
Symptoms that may accompany diverticulitis include:
- Rectal bleeding, blood in the stool. Diverticula on rare occasions bleed bright red blood and may appear in your stool.
TREATMENT
Aging and heredity are primary factors in the development of diverticulitis and diverticulosis, but diet also plays a role. Eating a lot of low-fiber, refined foods can greatly increase the risk. Indeed, in Western societies, an estimated 10% of people over 50 eventually develop diverticulosis, the figure reaches 50% in people over 60.
Though it has not been proven, it is though that if you are often constipated and usually strain at bowel movements, you may create enough pressure in the intestinal walls to begin the development of diverticular pouches. If the diverticula then become filled with fecal material of with undigested food, they are vulnerable to bacterial infection, leading to the inflammation of diverticulitis.
DIAGNOSTIC TESTS
If you think you have either diverticulosis or diverticulitis, your health care provider will probably have tests done on both your blood and stool to help diagnose possible infection. Because cancer, stomach ulcers, irritable bowel syndrome, and diverticulitis can share the same symptoms, your health care provider will need to thoroughly examine your intestines.
Barium Enema: A barium enema is a procedure in which the colon is filled with liquid barium and allows an X-ray to reveal an outline of the inside of the intestines and pouches in the colon wall, narrowing of the colon, or other abnormalities.

Sigmoidoscopy: To help confirm a diagnosis of diverticulosis, you may also have the insides of your intestines examined with a flexible lighted tube in a procedure called sigmoidoscopy. This procedure if performed by inserting a lighted tube into the rectum to give a health care provider a closer look at the lower colon.
Biopsy: Sometimes, if necessary, a bit of a tissue sample may also be removed for a biopsy, to test for cancerous growth.
Colonoscopy: To see into other parts of the colon, a colonoscopy must be performed. This is similar to a sigmoidoscopy, but allows a view of the entire colon.
CT Scan; If you have an acute case of diverticulitis, both barium enemas and sigmoidoscopies can injure your intestine, so your health care provider may choose instead a CT scan, which can help confirm the diagnosis.
TREATMENT
Diverticulosis can be treated at home, but if you have diverticulitis, you need to see your health care provider to make sure you recover completely and to avoid possible life-threatening complications. Conventional medicine treats diverticulitis using diet modifications, antibiotics, and possibly surgery.
If you have a mild case of diverticulosis, your health care provider may have you eat a high-fiber diet to make sure your bowels move regularly and to reduce your odds of getting diverticulitis. If you do go on to develop diverticulitis but have only a mild infection, you may need just bed rest, stool softeners, a liquid diet, antibiotics to fight the inflammation, and possibly antispasmodic drugs.
If you have had a perforation or develop a more severe infection, however, you will probably be hospitalized so you can receive intravenous antibiotics. You may also be fed intravenously to give your colon time to recuperate. In some cases, your health care provider may want to drain infected abscesses and give your intestinal tract a rest by performing a temporary colostomy. A colostomy creates an opening (called a stoma) so your intestine will empty into a bag that is attached to the front of your abdomen. Depending on the success of your recovery, this procedure may be reversed during a second operation.
If you have several attacks of acute diverticulitis, your health care provider may want to remove the affected section of the intestine when you are free of symptoms. You may also need surgery if intravenous therapy does not effectively treat your acute attack of diverticulitis. Whatever your treatment, your chances for a full recovery are very good if you receive prompt medical attention.
PREVENTION
The best preventive action you can take against diverticulitis, of course, is to keep from developing the small sacs called diverticula. You have a good chance of doing that simply by modifying your diet and lifestyle. Eat whole-grain breads, oatmeal, bran cereals, fibrous fresh fruits and vegetables, or try an over-the-counter preparation containing psyllium (Plantago psyllium) to increase the bulk in your diet. You can also try ground psyllium seed: Once a day, add 1 teaspoon over any cold liquid and drink within a few minutes of preparing, before the mixture gels. Make sure you drink plenty of fluids (at least eight 8-ounce glasses of water a day) if you increase your intake of fiber, and take care to add fiber gradually; a sudden switch to a high-fiber diet can cause your abdomen to become distended and create an uncomfortable amount of intestinal gas. Adjust your diet to remove foods that are difficult for you to digest.
Try over-the-counter stool softeners to fight constipation. If you have the urge to move your bowels, don't delay or ignore it. Regular exercise can help the muscles in your intestine retain their tone, which encourages regular bowel movements. Do not use suppositories for constipation, because your system may become addicted to them. Prunes, prune juice, and psyllium seed are all good natural laxatives. Specially formulated teas to fight constipation are available in health food stores, but some may be very strong, so use them only as directed; also avoid products containing senna (Cassia senna), which is an especially strong herbal laxative and can be addicting.

Follow a low-fat diet since fat slows down the passage of food through the intestine.
The key to controlling this disorder is to consume an adequate amount of fiber and lots of quality water. You need at least 30 grams of fiber each day. You may prefer to supplement your diet with a bulk product and/or a stool softener that contains methylcellulose or psyllium, since these do not promote as much gas formation in the colon as other sources of fiber, especially wheat bran. Eat a low carbohydrate diet with high levels of protein from vegetable sources and fish. Do not eat grains, seeds, or nuts, except for well-cooked brown rice. Also eliminate from the diet dairy products, red meat, sugar products, fried foods, spices, and processed foods.
Eat plenty of green leafy vegetables. These are good sources of vitamin K. Obtaining this vitamin through diet is especially important for people with intestinal disorders.

During an acute attack of diverticulitis, your health care provider may recommend low-fiber diet temporarily. Once the inflammation clears you may slowly switch back to a high-fiber diet. Eat low-bulk foods (broths and low-fiber breads) while diverticula are still inflamed and sensitive, and make the following foods a significant part of your diet: cooked vegetables, cooked fruits, and apples - all of which will be soothing to the inflamed intestine.
