    DIABETIC RETINOPATHY DESCRIPTION

    DIABETIC EYE DISEASE OVERVIEW

    Diabetic eye disease comprises a group of eye conditions that affect people with diabetes. These conditions include diabetic retinopathy, diabetic macular edema (DME), cataract, and glaucoma. All forms of diabetic eye disease have the potential to cause severe vision loss and blindness.

    CONTROLLING DIABETES

    Diabetes control is done by taking medications as prescribed, staying physically active, and maintaining a healthy diet. This can prevent or delay vision loss. EARLY DETECTION

    Because diabetic retinopathy often goes unnoticed until vision loss occurs, people with diabetes should get a comprehensive dilated eye exam at least once a year. Early detection, timely treatment, and appropriate follow-up care of diabetic eye disease can protect against vision loss. Diabetic retinopathy can be treated with several therapies, used alone or in combination.

    Diabetic eye disease can affect many parts of the eye, including the retina, macula, lens and the optic nerve. Diabetic eye disease is a group of eye conditions that can affect people with diabetes. Diabetic eye disease also includes cataract and glaucoma.
    • Diabetes can cause a disorder known as diabetic retinopathy. Retinopathy involves changes affects blood vessels in the light-sensitive tissue called the retina that lines the back of the eye. Changes to retinal tiny capillaries that nourish the retina can cause them to leak fluid or bleed, damaging the rod and cone cells. New capillaries then begin to form in the injured area, and these also interfere with and distorting vision. It is the most common cause of vision loss among people with diabetes and the leading cause of vision impairment and blindness among working-age adults. Diabetic retinopathy affects about 7 million Americans and causes blindness among about 7,000 each year. Unfortunately, there are few warning signs; the condition usually causes no symptoms until it is relatively advanced.

    • Diabetic macular edema (DME). A consequence of diabetic retinopathy, DME is swelling in an area of the retina called the macula.

    • Cataract is a clouding of the eye's lens. Adults with diabetes are 2 to 5 times more likely than those without diabetes to develop cataract. Cataract also tends to develop at an earlier age in people with diabetes.

    • Glaucoma is a group of diseases that damage the eye's optic nerve. The optic nerve is the bundle of nerve fibers that connects the eye to the brain. Some types of glaucoma are associated with elevated pressure inside the eye. In adults, diabetes nearly doubles the risk of glaucoma.
    All forms of diabetic eye disease have the potential to cause severe vision loss and blindness.

    DIABETIC RETINOPATHY FREQUENT SIGNS & SYMPTOMS

    People with diabetic retinopathy can experience a variety of symptoms. Diabetic retinopathy generally develops slowly over a period of months as ongoing high blood sugar levels damage the blood vessels of the retina. In early stages of diabetic retinopathy, there may be no noticeable symptoms. However, changes in the retina and the blood vessels of the retina may be seen during an eye examination. Because symptoms may not be apparent until late stages of diabetic retinopathy, regular eye examinations are vital to catching the condition in its early stage.

    Ongoing high blood sugar levels damage the blood vessels of the retina and leads to leaking or bleeding of the blood vessels or the growth of abnormal blood vessels. These processes can prevent light from reaching the retina. This leads to the typical symptoms of diabetic retinopathy, such as changes in vision, including blurred vision. There also may be eye pain. Other symptoms include seeing spots and a sudden loss of vision. Symptoms of diabetic retinopathy can also come and go.

    EARLY STAGES

    The early stages of diabetic retinopathy usually have no symptoms. The disease often progresses unnoticed until it affects vision. Bleeding from abnormal retinal blood vessels can cause the appearance of "floating" spots. These spots sometimes clear on their own. But without prompt treatment, bleeding often recurs, increasing the risk of permanent vision loss. If DME occurs, it can cause blurred vision.

    ADVANCED STAGES

    In more advanced stages of diabetic neuropathy, serious complications can result, including permanent vision loss, retinal detachment and blindness.

    OTHER COMPLICATIONS OF DIABETES

    There are other complications of diabetes that can commonly accompany diabetic retinopathy. These include diabetic neuropathy and peripheral vascular disease.





    DIABETIC RETINOPATHY CAUSES

    Chronically high blood sugar from diabetes is associated with damage to the tiny blood vessels in the retina, leading to diabetic retinopathy. The retina detects light and converts it to signals sent through the optic nerve to the brain. Diabetic retinopathy can cause blood vessels in the retina to leak fluid or hemorrhage (bleed), distorting vision. In its most advanced stage, new abnormal blood vessels proliferate (increase in number) on the surface of the retina, which can lead to scarring and cell loss in the retina.

    FOUR STAGES OF DIABETIC RETINOPATHY

    Diabetic retinopathy may progress through four stages:
    • Mild nonproliferative retinopathy. Small areas of balloon-like swelling in the retina’s tiny blood vessels, called microaneurysms, occur at this earliest stage of the disease. These microaneurysms may leak fluid into the retina.

    • Moderate nonproliferative retinopathy. As the disease progresses, blood vessels that nourish the retina may swell and distort. They may also lose their ability to transport blood. Both conditions cause characteristic changes to the appearance of the retina and may contribute to DME.

    • Severe nonproliferative retinopathy. Many more blood vessels are blocked, depriving blood supply to areas of the retina. These areas secrete growth factors that signal the retina to grow new blood vessels.

    • Proliferative diabetic retinopathy (PDR). At this advanced stage, growth factors secreted by the retina trigger the proliferation of new blood vessels, which grow along the inside surface of the retina and into the vitreous gel, the fluid that fills the eye. The new blood vessels are fragile, which makes them more likely to leak and bleed. Accompanying scar tissue can contract and cause retinal detachment. This is the pulling away of the retina from underlying tissue, like wallpaper peeling away from a wall. Retinal detachment can lead to permanent vision loss.

    DIABETIC MACULAR EDEMA (DME)

    DME is the build-up of fluid (edema) in a region of the retina called the macula. The macula is important for the sharp, straight-ahead vision that is used for reading, recognizing faces, and driving. DME is the most common cause of vision loss among people with diabetic retinopathy. About half of all people with diabetic retinopathy will develop DME. Although it is more likely to occur as diabetic retinopathy worsens, DME can happen at any stage of the disease.

    DIABETIC RETINOPATHY RISK FACTORS

    People with all types of diabetes (type 1, type 2, and gestational) are at risk for diabetic retinopathy. Risk increases the longer a person has diabetes. Between 40 and 45 percent of Americans diagnosed with diabetes have some stage of diabetic retinopathy, although only about half are aware of it. Women who develop or have diabetes during pregnancy may have rapid onset or worsening of diabetic retinopathy.





    DIABETIC RETINOPATHY MEDICAL DIAGNOSIS

    MEDICAL HISTORY

    Diagnosing diabetic retinopathy begins with taking a thorough personal and family medical history, including symptoms.

    PHYSICAL & EYE EXAM

    Completing a physical examination, including an extensive eye examination will be required. Diagnosis of diabetic retinopathy is generally made by an eye specialist called an ophthalmologist.

    An eye examination includes testing visual acuity or sharpness of vision, checking the sharpness of peripheral vision, and testing the pressure inside the eye. The outer eye is examined using an instrument called a slit lamp, and the inner eye is examined using an instrument called an ophthalmoscope. During the exam with an ophthalmoscope, the pupils of the eye are dilated (opened up) with a drop of medication so that the ophthalmologist can best see any changes in the retina that are signs of diabetic retinopathy such as leaking blood vessels, or abnormal new blood vessels.

    EYE TESTS

    Diabetic retinopathy and DME are detected during a comprehensive dilated eye exam that includes:
    • Visual acuity testing. This eye chart test measures a person's ability to see at various distances.
    • Tonometry. This test measures pressure inside the eye.
    • Pupil dilation. Drops placed on the eye's surface dilate (widen) the pupil, allowing a practitioner to examine the retina and optic nerve.
    • Optical coherence tomography (OCT). This technique is similar to ultrasound but uses light waves instead of sound waves to capture images of tissues inside the body. OCT provides detailed images of tissues that can be penetrated by light, such as the eye.
    A comprehensive dilated eye exam allows the practitioner to check the retina for:
    • Changes to blood vessels.
    • Leaking blood vessels or warning signs of leaky blood vessels, such as fatty deposits.
    • Swelling of the macula (DME).
    • Changes in the lens.
    • Damage to nerve tissue.
  • If DME or severe diabetic retinopathy is suspected, a fluorescein angiogram may be used to look for damaged or leaky blood vessels. In this test, a fluorescent dye is injected into the bloodstream, often into an arm vein. Pictures of the retinal blood vessels are taken as the dye reaches the eye.





  • DIABETIC RETINOPATHY CONVENTIONAL MEDICAL TREATMENT

    If you have diabetes, make sure to have an annual eye examination to detect the onset of retinopathy. Vision lost to diabetic retinopathy is sometimes irreversible. However, early detection and treatment can reduce the risk of blindness by 95 percent. Because diabetic retinopathy often lacks early symptoms, people with diabetes should get a comprehensive dilated eye exam at least once a year. People with diabetic retinopathy may need eye exams more frequently. Women with diabetes who become pregnant should have a comprehensive dilated eye exam as soon as possible. Additional exams during pregnancy may be needed. If the disorder is detected in time, laser surgery to seal leaking blood vessels can help stem vision loss.

    Studies such as the Diabetes Control and Complications Trial (DCCT) have shown that controlling diabetes slows the onset and worsening of diabetic retinopathy. DCCT study participants who kept their blood glucose level as close to normal as possible were significantly less likely than those without optimal glucose control to develop diabetic retinopathy, as well as kidney and nerve diseases. Other trials have shown that controlling elevated blood pressure and cholesterol can reduce the risk of vision loss among people with diabetes.

    Treatment for diabetic retinopathy is often delayed until it starts to progress to PDR, or when DME occurs. Comprehensive dilated eye exams are needed more frequently as diabetic retinopathy becomes more severe. People with severe nonproliferative diabetic retinopathy have a high risk of developing PDR and may need a comprehensive dilated eye exam as often as every 2 to 4 months.

    DME THERAPIES

    DME can be treated with several therapies that may be used alone or in combination.

  • Anti-VEGF Injection Therapy. Anti-VEGF drugs are injected into the vitreous gel to block a protein called vascular endothelial growth factor (VEGF), which can stimulate abnormal blood vessels to grow and leak fluid. Blocking VEGF can reverse abnormal blood vessel growth and decrease fluid in the retina. Available anti-VEGF drugs include Avastin (bevacizumab), Lucentis (ranibizumab), and Eylea (aflibercept). Lucentis and Eylea are approved by the U.S. Food and Drug Administration (FDA) for treating DME. Avastin was approved by the FDA to treat cancer, but is commonly used to treat eye conditions, including DME.

  • The National Eye Institute-sponsored Diabetic Retinopathy Clinical Research Network compared Avastin, Lucentis, and Eylea in a clinical trial. The study found all three drugs to be safe and effective for treating most people with DME. Patients who started the trial with 20/40 or better vision experienced similar improvements in vision no matter which of the three drugs they were given. However, patients who started the trial with 20/50 or worse vision had greater improvements in vision with Eylea.

    Most people require monthly anti-VEGF injections for the first six months of treatment. Thereafter, injections are needed less often: typically three to four during the second six months of treatment, about four during the second year of treatment, two in the third year, one in the fourth year, and none in the fifth year. Dilated eye exams may be needed less often as the disease stabilizes.

    Avastin, Lucentis, and Eylea vary in cost and in how often they need to be injected, so patients may wish to discuss these issues with an eye care professional.

  • Focal/Grid Macular Laser Surgery. In focal/grid macular laser surgery, a few to hundreds of small laser burns are made to leaking blood vessels in areas of edema near the center of the macula. Laser burns for DME slow the leakage of fluid, reducing swelling in the retina. The procedure is usually completed in one session, but some people may need more than one treatment. Focal/grid laser is sometimes applied before anti-VEGF injections, sometimes on the same day or a few days after an anti-VEGF injection, and sometimes only when DME fails to improve adequately after six months of anti-VEGF therapy.

  • Corticosteroids. Corticosteroids, either injected or implanted into the eye, may be used alone or in combination with other drugs or laser surgery to treat DME. The Ozurdex (dexamethasone) implant is for short-term use, while the Iluvien (fluocinolone acetonide) implant is longer lasting. Both are biodegradable and release a sustained dose of corticosteroids to suppress DME. Corticosteroid use in the eye increases the risk of cataract and glaucoma. DME patients who use corticosteroids should be monitored for increased pressure in the eye and glaucoma.

  • PROLIFERATIVE DIABETIC RETINOPATHY (PDR) TREATMENT

    For decades, PDR has been treated with scatter laser surgery, sometimes called panretinal laser surgery or panretinal photocoagulation. Treatment involves making 1,000 to 2,000 tiny laser burns in areas of the retina away from the macula. These laser burns are intended to cause abnormal blood vessels to shrink. Although treatment can be completed in one session, two or more sessions are sometimes required.

    While it can preserve central vision, scatter laser surgery may cause some loss of side (peripheral), color, and night vision. Scatter laser surgery works best before new, fragile blood vessels have started to bleed. Recent studies have shown that anti-VEGF treatment not only is effective for treating DME, but is also effective for slowing progression of diabetic retinopathy, including PDR, so anti-VEGF is increasingly used as a first-line treatment for PDR.

    TREATMENT RESULTS WITHOUT IMPROVED VISION

    An eye care professional can help locate and make referrals to low vision and rehabilitation services and suggest devices that may help make the most of remaining vision. Many community organizations and agencies offer information about low vision counseling, training, and other special services for people with visual impairment. A nearby school of medicine or optometry also may provide low vision and rehabilitation services.

    DIABETIC EYE DISEASE RESEARCH

    The National Eye Institute (NEI) is conducting and supporting research that seeks better ways to detect, treat, and prevent vision loss in people with diabetes. This research is being conducted in labs and clinical centers across the country.

  • The Diabetic Retinopathy Clinical Research Network (DRCR.net) conducts large multi-center trials to test new therapies for diabetic eye disease, and to compare different therapies. The network formed in 2002 and comprises more than 350 physicians practicing at more than 140 clinical sites across the country. Many of the sites are private practice eye clinics, enabling the network to quickly bring innovative treatments from research into community practice.

  • One study reported that insulin-dependent (type I) diabetics who controlled their blood sugar levels tightly were able to slow the pace of retinopathy by about 60-percent.

  • Researchers at the National Eye Institute induced a condition resembling diabetic retinopathy in dogs and then treated the animals with an experimental drug called sorbinil, which suppresses the action of an enzyme that converts excess sugar in the blood into an alcohol that seems to damage the retinal blood vessels. In this study, the sorbinil treatment totally blocked the progression of retinopathy.

  • NEI-funded scientists are also seeking ways to detect diabetic retinopathy at earlier stages. For example, researchers are harnessing a technology called adaptive optics (AO) to improve imaging techniques such as OCT. AO was developed by astronomers seeking to improve the resolution of their telescopes by filtering out distortions in the atmosphere. In the clinic, diagnostic devices that use AO may improve the detection of subtle changes in retinal tissue and blood vessels.

Homemade bean soup is always nice, especially during the winter months when it is cold and miserable outdoors.

  • Bhumy Amalaki (Phyllanthus Niruri) is employed for numerous conditions including blennorrhagia, colic, diabetes, dysentery, fever, flu, tumors, jaundice, vaginitis, and dyspepsia.

  • Black Seed with Bitter Melon is a high potency diabetic supplement blend. Bitter melon is one of the few agents which has the potential to bolster a flagging pancreas. It has been recommended by the Department of Health of the Philippines as one of the best herbal medicines for it's ability to help with liver problems, Diabetes and HIV. It is an excellent natural remedy for the common cold.

  • Bitter Melon (Momordica Charantia), Gudmar (Gymnema sylvestre), and Gulvel/Guduchi (Tinospora Cordifo) are herbal remedies used in Ayurvedic medicine to regulate blood sugar levels.

  • Other herbs that may be beneficial for diabetes include Bilberry, Buchu, Goldenseal, and Uva Ursi.

  • Bilberry leaf isused in connection with vascular and blood disorders and shows positive effects when treating varicose veins, thrombosis, and angina. Bilberry extract is high in bioflavonoid complex anthocyanosides and supports healthy vision. Bilberry is useful for maintaining a healthy digestive system, is an anti-inflammatory and an antioxidant.

  • Buchu leaves act as a diuretic and antiseptic, and today is used in treating inflammation of the urethra, blood in the urine, bladder infections, and other chronic urinary tract disorders. Buchu herb has traditionally been claimed to treat a wide variety of disorders, some of which include stomach complaints and urinary tract infections. Buchu leaves act as a diuretic and antisepic, and today is used in treating inflammation of the urethra, blood in the urine, bladder infections, and other chronic urinary trat infections.

  • Goldenseal is an antibacterial and antiseptic. Use especially when mucus turns thick and discolored. Use short term only with other herbs. Caution Do not take Goldenseal on a daily basis for more than one week at a time, and do not use it during pregnancy. If you have a history of cardiovascular disease, diabetes, or glaucoma, use it only under a health care provider's supervision.

  • Uva Ursi, also known as bearberry, has a specific affinity for the genito-urinary organs, especially for urinary tract infections, cystitis, nephritis, urethritis, pyelitis, hematuria (bloody urine), yeast infections, vulvitis. Uva Ursi leaves were officially a drug for urinary antiseptic in the United State Pharmacopoeia (U.S.P.) and the National Formulary N.F. from 1820 until 1950.

  • Cat's Claw Bark, Uncaria tomentosa, was reputed to be a super drug in the Andean region of South America. It has a long history of indigenous use for arthritis and rheumatism as well as other types of inflammation associated with various stomach disorders and ulcers where it was clinically shown to be effective. Cat's Claw extract, also known as Una de Gato is derived from the bark of plants wild-harvested in Peru and Brazil.

  • Cinnamon Bark (Cinnamonum Verum) experiments conducted by the USDA have shown that this - the variety we call "True Cinnamon" can lower blood sugar by mimicking insulin, activating insulin receptors and working with insulin in the cells to reduce blood sugar by up to 20-percent.

  • Chlorella Green Algae is helpful as a support for long-term debilitating systemic conditions which lower energy and resistance; such as diabetes, arthritis, high blood pressure, allergies, and infections.

  • Cedar Berries are excellent nourishment for the pancreas.

  • Dandelion Root (Taraxacum Officinale) is known as a blood purifier used for liver and kidney disorders. Dandelion root also contains nutritive salts to build up the blood. Dandelion root protects the liver, which converts nutrients into glucose and improves liver function and the metabolism of fats. Note: If you suffer from gallbladder problems, avoid large quantities of dandelion.

  • Elderberry juice is a medically proven strong anti-viral and immune system enhancer, elderberry is recommended as part of a health maintenance program.

  • Fenugreek Seeds have been shown to reduce cholesterol and blood sugar levels. Fenugreek seed is used around the world as a culinary spice and food that is soothing to the stomach. Fenugreek seed's steroidal saponins account for many of its beneficial effects, particularly the inhibition of cholesterol absorption and synthesis.

  • Fenugreek Seeds, Blackberry and Blueberry leaves have a beneficial effect on pancreatic function. Mix in equal parts to make a tea blend. Use 1 teaspoon of the mixture for each cup of water. Steep for 30 minutes and then strain. Drink as much as 2 quarts daily.

  • Flaxseed & Super Lignan Flax Seed Oil is a rich, vegetarian source of omega-3 and omega-6 essential fatty acids beneficial for many ailments. Flax Seeds are an important source of polyunsaturated fatty acids, including Omega-3, plus Magnesium, Zinc and dietary fiber. Flax oil from flax seeds is the richest known source of linolenic acid.

  • Take Garlic capsules and/or eat fresh Garlic in salad dressings or prepare a Garlic tincture. For the tincture, peel 8 to 10 whole bulbs of garlic and place in a blender with enough vodka or brandy to cover. Puree and transfer to a glass jar. Ensure that the garlic is fully covered by adding additional vodka, as needed. Let stand in a dark, warm place for about 2 weeks, shaking it daily. Filter out the garlic pieces and take 1 teaspoon of the tincture 3 to 4 times daily. In clinical studies garlic has been shown to lower serum cholesterol and help maintain circulatory efficiency. The greatest effect resulted from garlic products with standardized amounts of allicin release in doses of 5,000 mcg per day. The Europeans use garlic as an all-around treatment for preventing atherosclerosis, the cause of heart disease and strokes.

  • Ginseng tea is believed to lower the blood sugar level. Caution: Do not use this herb if you have high blood pressure. Red Panax Ginseng is often used to increase energy.

  • Ginkgo Biloba herbal extract standardized to 24-percent Ginkgo flavone glycosides and 6-percent terpene lactones, supported by Gotu Kola whole herb, supports circulation to the brain as well as the extremities. This is achieved by maintaining healthy blood vessel tone and reducing blood viscosity. Additionally, Ginkgo biloba is a proven free radical scavenger and antioxidant.

  • Glucomannan (Amorphophallus konjac fiber) is a 100-percent dietary fiber source obtained from the root of the konjac plant.

  • Guarana is a creeping shrub native to the Amazon (and particularly the regions of Manaus and Parintins). In the lushness of the Brazilian Amazon where it originates, it often grows to 12 meters high. The fruit is small, round, bright-red in color, and grows in clusters. As it ripens, the fruit splits and a black seed emerges - giving it the appearance of an "eye" about which Indians tell legends.

  • Gymnema extract is a traditional Ayurvedic medicine and is referred to as destroyer of sugar because chewing the leaves blocks the taste of sweetness.   • Opinions may differ as to the optimal ratio of dietary carbohydrates, proteins and fats for prevention and treatment of diabetes. However, it is safe to say that carbohydrates trigger the release of insulin. As more carbohydrates are consumed, more insulin is produced. The current epidemic of obesity and diabetes in the United States indicates that we are asking our bodies to burn the wrong fuel - refined carbohydrates in this case. It has been suggested by Dr. Gerald Reaven of Stanford University that a diet consisting of 45 percent complex carbohydrates, 40 percent "good" (polyunsaturated) fats, and 15 percent protein will benefit individuals with insulin resistance, the precursor to diabetes. It is estimated that as many as 70 to 80 million Americans may be affected by insulin resistance syndrome. Medications such as metformin (Glucophage) and rosiglitazone (Avandia) are prescribed to combat insulin resistance.

  • Eat a low-fat, high-fiber diet including plenty of raw fruits and vegetables as well as fresh vegetable juices. This reduces the need for insulin and also lowers the level of fats in the blood. Fiber helps to reduce blood sugar surges. For snacks, eat Oat or Rice Bran crackers with nut butter or cheese. Legumes, root vegetables, and whole grains are also good.   • The types of carbohydrates consumed are at least as important as the total carbohydrate loading. High-glycemic foods such as white rice, white flour products, pasta, starchy vegetables, and many processed foods are quickly converted into blood sugar during digestion, causing insulin to go up. Carbohydrates found in lower glycemic foods such as asparagus, broccoli, cabbage, green beans, and low-starch vegetables and fruits are converted into blood sugar more slowly, which only gradually raises insulin levels. Avoiding "white foods" might be best.

  • Supplement your diet with Spirulina. Spirulina helps stabilize blood sugar levels. Other foods that help normalize blood sugar include berries, Brewers Yeast, dairy products (especially cheese), egg yolks, fish, Garlic, Kelp, sauerkraut, soybeans, and vegetables.

DHEA may be used for immune system dysfunction, heart disease, diabetes, obesity, osteoporosis, arthritis, anti-aging and auto-immune diseases.

  • Avoid saturated fats, trans-fats, hydrogenated or partially hydrogenated oils, and simple sugars (except when necessary to balance an insulin reaction). While total fat intake does not seem to change the risk of getting diabetes, the trans-saturated fats and hydrogenated oils found in most fast foods can greatly increase the risk. Beneficial fats and oils include Extra Virgin Olive Oil, Salmon Fish Oil, Almond Oil, Avocado Oil, nuts and seed oils such as Sesame Seed Oil, Flaxseed Oil, Sunflower Oil, and Pumpkin Seed Oil.

  • Type 2 diabetes can be controlled by diet and exercise alone, but oral medications or injections of insulin can be added if regulating the diet does not work. Obesity is a major factor in type 2 diabetes, and a weight reduction program is often all that is required to control it.

  • It was once thought that people with diabetes should avoid sweetened foods. For weight control, this is still the case. However, research has shown that sugar - a simple carbohydrate - does not cause the greatest increase in blood glucose. Eating baked potatoes or some breakfast cereals causes a greater rise in blood sugar. Indeed carrots raise blood sugar more than ice cream does. It is on this principle that many high-protein, low-carbohydrate diets are based. It is essential, therefore, that people with diabetes measure their intake of both simple and complex carbohydrates - not simply those coming from sugar.

  • Omega 3 Fish Oil EFA research suggests a correlation between consuming more omega-3 fatty acids from Fish and better emotional health. Do not take fish oil capsules or supplements containing large amounts of para-aminobenzoic acid (PABA), and avoid salt and white flour products. Consumption of these products results in an elevation of blood sugar.

  • Eat more carbohydrates or reduce your insulin dosage before exercise. Exercise produces an insulin-like effect in the body. Discuss with your health care provider about the right approach for you.

  • A higher Magnesium intake lowers the risk of getting type 2 diabetes. The journal Diabetes Care published a study in which overweight women who consumed large amount of magnesium were 22 percent less likely to develop type 2 diabetes than women who consumed lower amounts. A diet rich in nuts, whole grains, and vegetables that are high in magnesium may be beneficial.

  • Do not take supplements containing amino acid Cysteine. It has the ability to break down the bonds of the hormone insulin and interferes with absorption of insulin by the cells.

  • Do not take extremely large doses of Vitamins B-1 (thiamine) and Vitamin C. Excessive amounts may inactivate insulin. These vitamins may, however, be taken in normal amounts. See Nutrient Table for recommendations.

  • If symptoms of hyperglycemia develop, go to the emergency room of the nearest hospital or medical center. This is a potentially dangerous situation. Intravenous administration of proper fluids, electrolytes, and insulin may be required.

  • Avoid taking large amounts of Vitamin B-3 (Niacin). However, small amounts (50 to 100 milligrams daily), taken by mouth, may be beneficial. In one study, large amounts of niacin raised blood sugar levels in people with non-insulin-dependent diabetes by as much as 16 percent. Over time, this could cause dependence on insulin or medication. Niacin can also cause the level of uric acid in the blood to rise, indicating probable kidney dysfunction and an increased risk of gout. However, niacinamide, a form of niacin, shows down the destruction and enhances the regeneration of the insulin-producing beta cells in the pancreas, and therefore may be helpful for those with type I diabetes.

  • Alpha Lipoic Acid is central to recycling other antioxidants like Vitamin E, Vitamin C, and GSH glutathione. By "sparing" their destruction, Alpha Lipoate extends their antioxidant power.   • CHROMIUM PICOLINATE: A COMPLEMENTARY NUTRITIONAL THERAPY FOR DIABETES


    Chromium Picolinate is a nutritional supplement that can help control diabetes. As the name implies, it is a combination of two different substances: chromium and picolinate. Chromium is a mineral that helps to increase the efficiency of insulin, the hormone that controls blood glucose (blood sugar) levels; picolinate is an amino acid derivative that allows the body to use chromium much more readily.

    The shape of individual insulin molecules is important to the hormone's effectiveness. If the molecules maintain their proper shape, the insulin can effectively transport glucose into the cells, where it is needed. Without chromium, insulin molecules become misshapen, and can no longer serve as an effective transportation system, glucose builds up in the bloodstream, starting a chain reaction that eventually leads to diabetes.

    Scientists have long known that chromium is a vital nutrient, but not until chromium was combined with picolinate was a truly effective means of providing supplemental chromium developed. In the body, chromium takes the form of an ion, a particle with an electrical charge. This charge is repelled by the body's cells, making it difficult for chromium to enter the cells. Picolinate is a chelator, a substance that can bind with an ion and neutralize its charge. Once chromium and picolinate join together, the chromium's repellant charge is done away with. The body's cells then are able to accept the chromium.

    The effectiveness of chromium picolinate has been tested on individuals with a number of different health problems, including diabetes. Research shows that most people with diabetes experience a decline in blood glucose levels after they start taking daily chromium picolinate supplements. As a result, it is believed that chromium picolinate may be able to help many people with diabetes (especially type II diabetes) to control their blood sugar levels. This in turn would allow them to cut back on their intake of insulin and other drugs, likely resulting in fewer side effects. Furthermore, since improving the action of insulin also helps the body to use fat as a fuel, chromium picolinate can help reduce obesity. This means that it may enable some people with type II diabetes to lose enough weight to stop taking drugs entirely.

    However, it is not recommended that people with diabetes simply go out and buy chromium picolinate supplements and start taking them. Anyone who has diabetes, especially type I diabetes, must exercise caution with this supplement. Its effects on insulin requirements are very real. Blood sugar levels must be monitored carefully and the appropriate dosages of insulin and/or other drugs adjusted as needed in response. Otherwise, a potentially dangerous insulin reaction may occur as a result of too little glucose in the blood.

    Anyone interested in using chromium picolinate for the treatment of diabetes should first seek the advice of a qualified health-care provider, preferably a nutritionally oriented practitioner who has experience in this area.


  • If you have a child with diabetes, be sure his or her teacher knows how to respond to the warning signs of hypoglycemia and hyperglycemia.

  • If symptoms of hypoglycemia develop, immediately consume fruit juice, soda pop, or anything else that contains sugar. If that fails to help within 20 minutes, repeat this regimen. If the second treatment fails, or if you cannot ingest food, seek immediate medical attention and/or administer a glucagon injection. Anyone who has insulin-dependent diabetes should always carry a glucagon kit and know how to use it.

  • Avoid tobacco in any form. It constricts the blood vessels and inhibits circulation. Keep your feet clean, dry, and warm, and wear only white cotton socks and well-fitting shoes. Lack of oxygen (because of poor circulation) and peripheral nerve damage (with loss of pain sensation) are major factors in the development of diabetic foot ulcers. Try to avoid injury, and take measures to improve the circulation in the feet and legs. See Circulatory Problems.

  • Alcohol now and then may help. A growing body of evidence links moderate alcohol consumption with reduced risks of heart disease. The same may be true for type 2 diabetes. Moderate amounts of alcohol - a drink a day for men, a drink every other day for women - increases the efficiency of insulin at getting glucose inside cells. And some studies indicate that moderate alcohol consumption decreases the risk of type 2 diabetes. If you already drink alcohol, the key is to keep your consumption in the moderate range. If you do not drink alcohol, there is no need to start - you can get the same benefits by losing weight, exercising more, and changing your eating patterns.
