COLD SORES DESCRIPTION
Cold sores, or fever blisters (also known as oral herpes) are a painful infection caused by herpes simplex virus I (HSV Type 1), which is related to, but different from the virus that causes genital herpes. They may show up anywhere on your body but are most likely to appear on your gums, the outside of your mouth and lips, your nose, cheeks, or fingers. They first appear 3 to 10 days after the initial exposure (sometimes as late as 20 days) and may last up to 3 weeks. Once the virus enters your body, it will lie dormant until triggered. It may emerge years later at or near the original site of entry. The virus remains permanently in the body and moves into the nervous system, usually close to the lips. Repeated outbreaks may be triggered by fever, certain foods, a cold or other viral infections, exposure to sun and wind, stress, menstruation, high levels of amino acid arginine, allergies, or depression of the immune system. These sores are very contagious and very common. Studies have shown that between 30 to 60 percent of children carry the virus by the age of 10.
Cold sores are transmitted by such forms of contact as kissing an infected person or sharing eating utensils, towels, or razors. Genital herpes is usually caused by herpes simplex virus Type 2. A person with a cold sore who performs oral sex on another person can give that person genital lesions with HSV-1.
COLD SORES FREQUENT SIGNS & SYMPTOMS
The first sign of a developing cold sore is local tenderness with a small bump. The bump then turns into a blister, and there may be more tenderness in the area. The fluid-filled blisters or red, painful sores are found on or near the mouth, or on fingers.
Swollen, sensitive gums of a deep red color.
A fever, flu-like symptoms, and swollen lymph nodes in the neck often accompanying the first attack; recurrent sores usually do not produce these symptoms.
Tingling and itching in the area of a previous outbreak. Symptoms of a recurrence.
The adjacent lymph nodes may become swollen and tender.
In some cases, pus oozes from the blisters, a yellow crust develops and eventually sloughs off, revealing new skin underneath. Cold sores may make eating difficult.
There is less discomfort with recurrences of cold sores than with the initial outbreak.
The sores usually last 7 to 10 days.
6 STAGES OF COLD SORE DEVELOPMENT
There are said to be 6 stages of the development of the average cold sore.
STAGE 1: First comes stage one. In this stage, no sore is visible but there is a feeling of prickling or itchiness around the affected area.
STAGE 2: In stage two, swelling may start and the area may feel slightly red.
STAGE 3: Stage three brings with it the first signs of a blister or blisters.
STAGE 4: Stage four is the most painful, and generally comes on the fourth day. A soft crust or ulcer forms, and from this the cold sore moves into the next stage.
STAGE 5: In stage five is when the hard crust forms on the ulcer. This stage usually lasts for 4 days.
STAGE 6: The sixth and last stage, coming on or around the tenth day, leaves the sore red and slightly swollen, but without the hard scab.
An outbreak of cold sores may last more or less than 10 days, depending on its severity.
COLD SORES CAUSES
Herpes simplex is a viral disease affecting the mucous membranes and the skin. It can be categorized by two types: Type-I begins with a tingling feeling in the face, cheeks, nose, eyes or lips, and results in cold sores, skin eruptions and, if you have already had the chicken pox virus, painful shingles. Herpes simplex Type-II is the sexually-transmitted type, with first indications of the disease being a tingling sensation in the genitals. The affected area then itches, burns and develops a blister, which will usually heal over a period of about two weeks. Both types of herpes simplex are highly infectious and are usually transmitted by contact. Often a person with Type-I got the disease as a child but showed no symptoms. Later, the virus might unexpectedly appear when the immune system is weakened or there is a skin irritation. Treating the herpes simplex virus with herbal compresses and essential oils can both speed the healing process and prevent scarring. Consult with your health care provider if you have sexually transmitted herpes or shingles.
Wash your hands after touching a cold sore. Do not your eyes after touching your cold sores, you could develop an ocular herpes infection, which may lead to blindness if left untreated. Do not touch your genitals after touching your cold sore; you could develop genital herpes. Replace your toothbrush. Eat foods high in Lysine, or take Supplements (see below). Do not kiss someone who has a cold sore or use the same utensils, towels, or razors. Use sunscreen to the face and lips before prolonged exposure to the sun. Reduce stress in your life as much as possible. Eat a well balanced diet and keep your immune system healthy and working well.
COMPLICATIONS & RISKS
Ninety percent of all people get at least one cold sore in their lives. This first occurrence is often the worst. Some children who are affected may become seriously ill. After the first infection, many people develop antibodies and never have another cold sore. About 40-percent of American adults, however, have repeated cold sores.
Although cold sores generally are not serious, the infection may be life-threatening for anyone who has AIDS or whose immune system is depressed by other disorders or medications.
The infection from a cold sore may cause blindness if it spreads to the eye; herpes is a frequent cause of infectious blindness.
COLD SORES DIAGNOSIS
The diagnosis of cold sores is usually based on the appearance of the lesions. Usually, no laboratory test is needed because most sores that look like cold sores are cold sores. Occasionally, mouth sores known as canker sores may be mistaken for cold sores. However, canker sores occur inside the mouth whereas recurrent cold sores usually occur on the lips.
If there is a question about the diagnosis, a variety of tests are available, including viral culture and polymerase chain reaction (PCR). To do these tests, a swab is rubbed over an active blister. Swabbing the blister in an attempt to culture the virus in the lab works best in the first 24 to 48 hours before the blister has crusted over. There is also a way to test for herpes DNA in a swab that has been rubbed on the sore. This type of testing is called a "polymerase chain reaction" or PCR for short. PCR testing is very good at detecting herpes virus, but it is not as readily available as culture.
Blood tests for antibodies are not very useful, because finding antibodies to herpes just means that the body has been exposed to this virus at some point in the past. It does not tell if the current sore is due to herpes.
If the diagnosis is in doubt, the best approach is to encourage the person to see a health practitioner at the first sign of a sore. That will allow the clinician to see active lesions that can be tested by culture or PCR. If a delay is anticipated in being seen, a mobile phone or camera can be used to take pictures of the lesion to show to the health practitioner.
COLD SORES CONVENTIONAL MEDICAL TREATMENT
There are several medications to reduce the duration or symptoms of cold sores. Some are available without a prescription (over the counter), and others require a prescription from a health care practitioner. Some are topical (meaning that they are creams or ointments rubbed directly on the sore), and others are taken in pill form.
Denavir (penciclovir) cream is clinically proven to be effective in treatment of oral cold sores (herpes cold sores or fever blisters) and reduces the duration of the infection by one day, on average.
The drug acyclovir (Zovirax), in oral (capsule or liquid) or topical (ointment) form, is sometimes prescribed for cold sores.
An over-the-counter drug called docosanol (Abreva) has been approved by the FDA to reduce healing time and duration of symptoms of cold sores. The medication contains 10 percent docosanol, an antiviral agent. This is a topical medication that is rubbed into the sore five times a day for adults and children over the age of 12. Children younger than 12 should use this medication under a health care provider's supervision.
OVER-THE-COUNTER (OTC) TOPICAL MEDICATIONS
Most topical OTC products provide symptomatic relief only. This means that they make people feel better but they do not decrease healing time. Using topical anesthetics that contain benzocaine (5 to 20-percent), lidocaine (0.5 to 4-percent), tetracaine (2-percent) or dibucaine (0.25 to 1-percent) will help relieve burning, itching, and pain. Examples are Lipactin gel and Zilactin. It is important to keep in mind that these topical agents have a short duration of action, usually only lasting 20 to 30 minutes. Skin protectants, such as allantoin, petrolatum, and dimethicone-containing products, help keep the lesion moist and prevent cracking of the lesion.
Sunscreen-containing lip balms may also help to prevent additional outbreaks if the sun is a precipitating factor.
For additional pain relief, using ibuprofen (Advil) or acetaminophen (Tylenol) may be beneficial. Do not apply any topical steroids, such as hydrocortisone, to the lesions.
Docosanol 10-percent cream (Abreva) is the only over-the-counter product that has been shown to decrease healing time when applied at the first sign of recurrence (for example, a tingling sensation). Docosanol is applied five times per day until the lesion is healed. Possible side effects include rash and itching at the site of application.
PRESCRIPTION-STRENGTH TOPICAL MEDICATIONS
Treatment with topical acyclovir (Zovirax 5-percent cream) or penciclovir (Denavir 1-percent cream) will reduce healing time by approximately half a day and decrease pain associated with the lesion. Topical treatment is limited in its effectiveness because it has poor penetration into the site of replication of the virus and therefore is restricted in its healing ability. Acyclovir cream should be applied five times per day for four days, and penciclovir cream should be applied every two hours while awake for four days.
The current FDA-approved medications used in the treatment of herpes simplex virus in adults are acyclovir (Zovirax), valacyclovir (Valtrex), and Famciclovir (Famvir). These oral medications have been shown to decrease the duration of the outbreak, especially when started during the "prodrome" (symptom onset before the actual condition becomes fully evident). The medications are generally well tolerated with few side effects. Headache, nausea, and diarrhea may occur in some people. For simple, recurrent cold sores in adults, valacyclovir is given as 2 grams orally every 12 hours for one day, and famciclovir is given as 1,500 milligrams orally for one dose. Acyclovir is given as 400 mg orally five times per day for five days.
Pregnant women and nursing mothers should contact their health care providers or pharmacists prior to using any medication. Famciclovir and valacyclovir are not FDA approved for use in children under 12 years of age with cold sores.
Begin taking antiviral herbs and formulas that boost the body's immune system the minute you suspect an infection. You can usually prevent a larger blister from forming if you begin treatment as soon as you notice tingling. Applying teas or tinctures will also help speed the healing process. Although blisters heal on their own after several weeks, remember that they are infectious, even when they have healed and a crust has formed.
Red Clover, Echinacea, Goldenseal, and Pau D'Arco can be used for cold sores. Echinacea stimulates the immune system and may also aid in preventing an outbreak. Caution: do not take Goldenseal on a daily basis for more than one week at a time, and do not use it during pregnancy. If you have a history of cardiovascular disease, diabetes, or glaucoma, use it only under a health care provider's supervision.
Olive Leaf Extract is good as a natural antibiotic for viral infections.
Lemon Balm contains a high concentration of polyphenols and appears to minimize herpes outbreaks. It can be applied topically in cream form. Or brew lemon balm tea and apply it directly to the sore. Taken internally, lemon balm tea has antiviral qualities.
Daily Ritual - Lemon Balm Tincture has proven to be a good treatment for Type I and Type II herpes blisters. Lemon Balm's constituents, along with Citronella Oil and Citral, work to hamper the multiplication of the herpes-simplex virus and soothe infected skin. Make a tincture from 1/3 ounce of Lemon Balm leaves and 3.5 ounces of vodka or rum. Dab a bit of the tincture on the blisters several times a day.
Grapefruit Seed Extract will mobilize bodily defenses and help make the herpes virus harmless. Mix 1 part of the extract with 9 parts of water. Dampen cotton swabs with the mixture and dab any blisters you have several times a day to hasten the immune response of your body.
Boosting the immune system whenever your immune system has been weakened, such as when you have a cold or you are under unusual emotional or physical stress, for example, when you are more likely to contract a viral infection, such as herpes simplex. If you exercise in fresh hair and take vitamins, you can help boost your immune system. Consider taking Vitamin C, Zinc and Beta-Carotene, which will support the immune response and help fight viruses. For acute or chronic infections, St. John's Wort, Echinacea, Licorice or Astragalus will support the body's defenses and help speed the healing process.
RED CLOVER: In Europe Red Clover is widely cultivated and used as a tonic and a salad herb. Herbalists have long prized Red Clover for it's traditional use as a blood purifier and it's isoflavones that offer great medicinal potential.
ECHINACEA: Echinacea (Echinacea Augustofolia, Purpurea, Pallida) is an excellent herb for all kinds of viral & bacterial infections, strengthens the immune system against pathogenic infection by stimulating phagocytosis & T-Cell formation, blood cleansing properties, acne, bronchitis, colds & flu, congestion, psoriasis, tonsillitis, wounds, ear infections and stimulates the immune system.
GOLDENSEAL: Goldenseal (hydrastis canadensis), a perennial wild native American herb, is also called Yellow Root. Native Americans used Goldenseal both internally and externally and also derived a dye from the root. Golden Seal is a very important traditional herb used to treat infections. Golden Seal was listed from 1860 to 1926 in the U.S. Pharmacopoeia. It was primarily used for inflammations of the mucous membranes.
PAU D'ARCO: Pau D'Arco and it's constituents have demonstrated antiviral properties against various viruses including Herpes I and II, influenza, poliovirus, and vesicular stomatitis virus. It's anti-parasitic actions against various parasites including malaria, Schistosoma, and Trypansoma have been clinically validated. Bark extracts of Pau D'Arco have demonstrated anti-inflammatory activity and have been shown to be successful against a wide range of inflammations.
Essential oils for disinfection. Some essential oils can be more beneficial than others in treating a herpes infection because of their strong antiseptic properties. Tea Tree Oil is especially effective and will help stabilize the immune system and act as a disinfectant as well. Other oils that relieve inflammation and discourage viral multiplication are Bergamot, Lemon Balm and Chamomile essential oils. Tea Tree Oil may be used undiluted on your skin, while the other oils should be mixed with a carrier oil, such as Sweet Almond Oil or Olive Oil.
DIET & NUTRITION RECOMMENDATIONS
Eat plenty of raw vegetables, as well as yogurt and other soured products.
Eat chocolate, dairy products, cereals, almonds, and chicken in moderation, as they suppress an acid that retards the growth of the virus.
Lysine is a naturally occurring amino acid that inhibits the ability of a herpes virus to reproduce itself. Take several grams per day for 1 month at the onset of an outbreak. Those who are plagued by repeat eruptions may take it regularly. Lysine supplements and diets with a high lysine/arginine ratio inhibit herpes virus replication. The amino acid lysine often controls herpes. Supplementation with free-form lysine has shown to be beneficial in controlling herpes along with a diet high in lysine and low in arginine. It has been found that foods high in I-Arginine may cause herpes outbreaks. Increased levels of lysine over arginine suppress viral replication and inhibit cytopathogenicity of herpes simplex virus. L-Lysine appears to be an effective agent for reduction of the occurrence, severity and healing time for recurrent HSV infection. Several health care providers have reported that if lysine use reduces herpes outbreaks, an immunological imbalance is present. Treatments aimed at immune system improvement have been effective in eliminating or reducing recurrence. Foods high in lysine and low in arginine include most fruits and vegetables, fish, chicken, beef, lamb, milk, cheese, beans, brewer's yeast and mung bean sprouts. Gelatin, chocolate, carob, coconut, oats, whole-wheat and white flour, peanuts, soybeans, and wheat germ have more arginine than lysine and should thus be avoided. In one study, the amount of lysine required to control herpes varied from case to case but a typical dose to maintain remission was 500 mg daily and active herpes required 1 to 6 grams between meals to induce healing. A study at the UCLA School of Medicine found of 45 patients receiving L-Lysine for herpes, only two failed to respond (a 96-percent success rate). The patients were receiving about 1,500 mg L-Lysine daily. Lysine supplements (as opposed to foods high in this nutrient) can play an important role in staving off and reducing the severity of herpes-related cold sores. Results of a six-month trial involving more than 50 people indicate that lysine is far more effective than a placebo in preventing cold sores. Participants given a placebo had more than twice as many such infections as those taking lysine. Moreover, the herpes sores that did develop in the lysine group tended to be milder, and to heal faster, than the outbreaks in the placebo group. Lysine supplements may even prevent HSV outbreaks in chronic sufferers. General advice is to take 1,000 mg L-lysine three times a day with meals for flare-ups. If you are subject to recurrent outbreaks of cold sores, continue on a maintenance dosage of 1,000 mg per day. The amino acid L-Arginine is not recommended. Arginine promotes viral replication of herpes. In some people, changing the dietary ratio of lysine to arginine reduces the frequency and intensity of outbreaks.
Coconut Oil lyses lipid coated viruses including the herpes family. Coconut oil is anti-viral, anti-bacterial and anti-fungal in the originating plant, in our gut and in our blood. Reducing seed oil consumption and using coconut milk or coconut oil as the dietary staple is reversing viral loads in HIV, eliminating all types of herpes virus and reducing or preventing other viral diseases including the yearly flu and measles. The antiviral effect seems to be against lipid-enveloped viruses and is due to the presence of lauric acid in coconut oil. Monolaurin is a derivative of coconut that has been shown to have anti-fungal and anti-viral properties. A therapeutic dose of monolaurin is generally 1800 mg to 2400 mg per day.
Vitamin A, Vitamin C & Zinc in combination can significantly enhance the function of your immune system. Vitamin C, Zinc, Thymus Extracts, TMG (Tri-methyl-glycine), Monolaurin from Coconut, and Olive Leaf Extract have all been used with some success as reported by various health care providers.

Olive Leaf Extract. Olive Leaf (Olea europaea) herb has long been used for general well being and has been the focus of research since the 1800's. Many people throughout the USA are using BHT to successfully keep the lesions dormant, with no sores as long as they consume BHT regularly. Some people develop sores once every 3 to 4 months opposed to higher frequency when not using BHT. The majority of those who use lysine regularly along with 750 to 1,000 mg of BHT daily keep the sores from ever occurring. The only human antiviral test that has been conducted with BHT was for herpes, the outcome of which was successful.
Indole-3-Carbinol (I3C) & Resveratrol, according to in vitro research, are 99-percent effective in preventing the herpes virus from replicating in human cells. Researchers are finding a tremendous link between increased intake of broccoli, cabbage and Brussels sprouts and the reduction in severity and frequency of herpes simplex. Cimetidine (an inexpensive OTC drug with immune enhancing properties) has proven to be equal to or superior to expensive antiviral drugs in treating herpes simplex and herpes zoster. Cimetidine is an acid blocker used for ulcers and heart burn, and goes by the trade name Tagamet.
If cold sore outbreaks occur often, check for low thyroid function.
You cannot cure a cold sore, but you can alleviate the pain it causes by avoiding spicy or acidic foods, applying ice, and using over-the-counter remedies. Wrap an ice cube in a handkerchief and press it on the affected region. Keep pressing it against the area for as long as you can, taking occasional breaks, so your skin does not get overly or painfully cold. Look for medicines that contain numbing agents such as phenol and emollients to reduce cracking and soften scabs.
If you are prone to allergies, you most likely have a malfunctioning immune system and may be susceptible to cold sores.
Strong sunlight can cause an outbreak of herpes blisters. Be sure to protect your lips with an ointment that has high sun-protection factor.
Stress weakens the immune system and encourages herpes. Exercise such as yoga or walking helps alleviate stress. Relaxation and visualization techniques (imagining the virus being destroyed by healing white blood cells, for example) may reduce the seriousness of cold sore outbreaks.
Keep your toothbrush dry when not using it, and get a new one every 4 weeks. Wet toothbrushes offer optimal conditions for the herpes virus.
Zinc is a trace mineral that speeds up the healing process by drying out blisters and killing the virus. Use zinc paste or zinc powder from a pharmacy. You can make a zinc solution by dissolving 4 grams of zinc sulfate in 1/2 cup of boiled, and then cooled water. Dampen a linen cloth or cotton ball with the solution and apply to blisters.
Aerobic Exercise is a healthy recommendation. A healthy immune system may be important in controlling the virus, so proper nutrition, exercise and rest is recommended.
Personal Hygiene Changes may be necessary. During an outbreak, keep the infected area as clean and dry as possible. This will help your natural healing processes. Some health care providers recommend warm showers in order to cleanse the infected area. Afterwards, towel dry gently, or dry the area with a hair dryer on a low or cool setting. Most creams and lotions do no good and may even irritate.
Ozone / Oxidative Therapy may be considered. Dr. R. Mattassi of Italy in his 1982 study found that 24 out of 27 people with Herpes simplex stayed in remission with up to a 3 year follow-up. He also showed that all lesions went into remission after a maximum of 5 ozone injections.

Unless otherwise specified, the following recommended doses are for adults over the age of 18. For children between the ages of 12 and 17, reduce the dose to 3/4 the recommended amount. For children between the ages of 6 and 12 years old, reduce the dose to 1/2 the recommended amount. For children under 6 years old, use 1/4 the recommended amount.
