DESCRIPTION
Cold sores, or fever blisters (also known as oral herpes) are a painful infection caused by herpes simplex virus I (HSV Type 1), which is related to, but different from the virus that causes genital herpes. They may show up anywhere on your body but are most likely to appear on your gums, the outside of your mouth and lips, your nose, cheeks, or fingers. They first appear 3 to 10 days after the initial exposure (sometimes as late as 20 days) and may last up to 3 weeks. Once the virus enters your body, it will lie dormant until triggered. It may emerge years later at or near the original site of entry. The virus remains permanently in the body and moves into the nervous system, usually close to the lips. Repeated outbreaks may be triggered by fever, certain foods, a cold or other viral infections, exposure to sun and wind, stress, menstruation, high levels of amino acid arginine, allergies, or depression of the immune system. These sores are very contagious and very common. Studies have shown that between 30 and 60 percent of children carry the virus by the age of 10.
Cold sores are transmitted by such forms of contact as kissing an infected person or sharing eating utensils, towels, or razors. Genital herpes is usually caused by herpes simplex virus Type 2. A person with a cold sore who performs oral sex on another person can give that person genital lesions with HSV-1.
SYMPTOMS
The first sign of a developing cold sore is local tenderness with a small bump.
The bump then turns into a blister, and there may be more tenderness in the area. The fluid-filled blisters or red, painful sores are found on or near the mouth, or on fingers.
Swollen, sensitive gums of a deep red color.
A fever, flu-like symptoms, and swollen lymph nodes in the neck often accompanying the first attack; recurrent sores usually don't produce these symptoms.
Tingling and itching in the area of a previous outbreak - symptoms of a recurrence.
The adjacent lymph nodes may become swollen and tender.
In some cases, pus oozes from the blisters, a yellow crust develops and eventually sloughs off, revealing new skin underneath. Cold sores may make eating difficult.
There is less discomfort with recurrences of cold sores than with the initial outbreak.
The sores usually last 7 to 10 days.
6 STAGES OF COLD SORE DEVELOPMENT
There are said to be 6 stages of the development of the average cold sore.
STAGE 1: First comes stage one. In this stage, no sore is visible but there is a feeling of prickling or itchiness around the affected area.
STAGE 2: In stage two, swelling may start and the area may feel slightly red.
STAGE 3: Stage three brings with it the first signs of a blister or blisters.
STAGE 4: Stage four is the most painful, and generally comes on the fourth day. A soft crust or ulcer forms, and from this the cold sore moves into the next stage.
STAGE 5: In stage five is when the hard crust forms on the ulcer. This stage usually lasts for 4 days.
STAGE 6: The sixth and last stage, coming on or around the tenth day, leaves the sore red and slightly swollen, but without the hard scab.
An outbreak of cold sores may last more or less than 10 days, depending on its severity.
PREVENTION
Wash your hands after touching a cold sore.
Don't rub your eyes after touching your cold sores, you could develop an ocular herpes infection, which may lead to blindness if left untreated.
Don't touch your genitals after touching your cold sore; you could develop genital herpes.
Replace your toothbrush.
Eat foods high in lysine, or take supplements (see below).
Don't kiss someone who has a cold sore or use the same utensils, towels, or razors.
Use sunscreen to the face and lips before prolonged exposure to the sun.
Reduce stress in your life as much as possible.
Eat a well balanced diet and keep your immune system healthy and working well.
COMPLICATIONS & RISKS
Ninety percent of all people get at least one cold sore in their lives. This first occurrence is often the worst. Some children who are affected may become seriously ill. After the first infection, many people develop antibodies and never have another cold sore. About 40% of American adults, however, have repeated cold sores.
Although cold sores generally are not serious, the infection may be life-threatening for anyone who has AIDS or whose immune system is depressed by other disorders or medications.
The infection from a cold sore may cause blindness if it spreads to the eye; herpes is a frequent cause of infectious blindness.
TREATMENT
Herpes simplex is a viral disease affecting the mucous membranes and the skin. It can be categorized by two types: Type I begins with a tingling feeling in the face, cheeks, nose, eyes or lips, and results in cold sores, skin eruptions and, if you have already had the chicken pox virus, painful shingles. Herpes simplex Type II is the sexually-transmitted type, with first indications of the disease being a tingling sensation in the genitals. The affected area then itches, burns and develops a blister, which will usually heal over a period of about two weeks. Both types of herpes simplex are highly infectious and are usually transmitted by contact. Often a person with Type I got the disease as a child but showed no symptoms. Later, the virus might unexpectedly appear when the immune system is weakened or there is a skin irritation. Treating the herpes simplex virus with herbal compresses and essential oils can both speed the healing process and prevent scarring. Consult with your health care provider if you have sexually transmitted herpes or shingles.
WHAT YOU CAN DO: Begin taking antiviral herbs and formulas that boost the body's immune system the minute you suspect an infection. You can usually prevent a larger blister from forming if you begin treatment as soon as you notice tingling. Applying teas or tinctures will also help speed the healing process. Although blisters heal on their own after several weeks, remember that they are infectious, even when they have healed and a crust has formed.
HERBS
Red Clover, Echinacea, Goldenseal, and Pau D'Arco can be used for cold sores. Echinacea stimulates the immune system and may also aid in preventing an outbreak. Caution: do not take goldenseal on a daily basis for more than one week at a time, and do not use it during pregnancy. If you have a history of cardiovascular disease, diabetes, or glaucoma, use it only under a health care provider's supervision.
Lemon Balm contains a high concentration of polyphenols and appears to minimize herpes outbreaks. It can be applied topically in cream form. Or brew lemon balm tea and apply it directly to the sore. Taken internally, lemon balm tea has antiviral qualities.
Daily Ritual - Lemon Balm Tincture has proven to be a good treatment for Type I and Type II herpes blisters. Lemon Balm's constituents, along with Citronella Oil and Citral, work to hamper the multiplication of the herpes-simplex virus and soothe infected skin. Make a tincture from 1/3 ounce of Lemon Balm leaves and 3-1/2 ounces of vodka or rum. Dab a bit of the tincture on the blisters several times a day.
Olive Leaf Extract is good as a natural antibiotic for viral infections.
Grapefruit Seed Extract will mobilize bodily defenses and help make the herpes virus harmless. Mix 1 part of the extract with 9 parts of water. Dampen cotton swabs with the mixture and dab any blisters you have several times a day to hasten the immune response of your body.
Boosting the immune system whenever your immune system has been weakened, such as when you have a cold or you are under unusual emotional or physical stress, for example, when you are more likely to contract a viral infection, such as herpes simplex. If you exercise in fresh hair and take vitamins, you can help boost your immune system. Consider taking vitamin C, zinc and beta-carotene, which will support the immune response and help fight viruses. For acute or chronic infections, St. John's Wort, Echinacea, Licorice or Astragalus will support the body's defenses and help speed the healing process.
Essential oils for disinfection. Some essential oils can be more beneficial than others in treating a herpes infection because of their strong antiseptic properties. Tea Tree Oil is especially effective and will help stabilize the immune system and act as a disinfectant as well. Other oils that relieve inflammation and discourage viral multiplication are Bergamot, Lemon Balm and Chamomile essential oils. Tea Tree Oil may be used undiluted on your skin, while the other oils should be mixed with a carrier oil, such as Sweet Almond Oil or Olive Oil.

HOLISTIC RECOMMENDATIONS & CONSIDERATIONS
Eat plenty of raw vegetables, as well as yogurt and other soured products.
If cold sore outbreaks occur often, check for low thyroid function.
You cannot cure a cold sore, but you can alleviate the pain it causes by avoiding spicy or acidic foods, applying ice, and using over-the-counter remedies. Wrap an ice cube in a handkerchief and press it on the affected region. Keep pressing it against the area for as long as you can, taking occasional breaks, so your skin does not get overly or painfully cold. Look for medicines that contain numbing agents such as phenol and emollients to reduce cracking and soften scabs.
If you are prone to allergies, you most likely have a malfunctioning immune system and may be susceptible to cold sores.
Strong sunlight can cause an outbreak of herpes blisters. Be sure to protect your lips with an ointment that has high sun-protection factor.
Stress weakens the immune system and encourages herpes. Exercise such as yoga or walking helps alleviate stress. Relaxation and visualization techniques (imagining the virus being destroyed by healing white blood cells, for example) may reduce the seriousness of cold sore outbreaks.
Eat chocolate, dairy products, cereals, almonds, and chicken in moderation, as they suppress an acid that retards the growth of the virus.
Keep your toothbrush dry when not using it, and get a new one every 4 weeks. Wet toothbrushes offer optimal conditions for the herpes virus.
Lysine is a naturally occurring amino acid that inhibits the ability of a herpes virus to reproduce itself. Take several grams per day for 1 month at the onset of an outbreak. Those who are plagued by repeat eruptions may take it regularly.
Zinc is a trace mineral that speeds up the healing process by drying out blisters and killing the virus. Use zinc paste or zinc powder from a pharmacy. You can make a zinc solution by dissolving 4 grams of zinc sulfate in 1/2 cup of boiled, and then cooled water. Dampen a linen cloth or cotton ball with the solution and apply to blisters. OTHER HELPFUL SUGGESTIONS
Cimetidine (an inexpensive OTC drug with immune enhancing properties) has proven to be equal to or superior to expensive antiviral drugs in treating herpes simplex and herpes zoster. Cimetidine is an acid blocker used for ulcers and heart burn, and goes by the trade name Tagamet.
Coconut oil lyses lipid coated viruses including the herpes family. Coconut oil is anti-viral, anti-bacterial and anti-fungal in the originating plant, in our gut and in our blood. Reducing seed oil consumption and using coconut milk or coconut oil as the dietary staple is reversing viral loads in HIV, eliminating all types of herpes virus and reducing or preventing other viral diseases including the yearly flu and measles. The antiviral effect seems to be against lipid-enveloped viruses and is due to the presence of lauric acid in coconut oil. Monolaurin is a derivative of coconut that has been shown to have anti-fungal and anti-viral properties. A therapeutic dose of monolaurin is generally 1800 mg to 2400 mg per day.

Olive Leaf Extract - The main constituent of olive leaf is the phytochemical oleuropein, which has broken down to elenolic acid, which have a powerful anti-bacterial effect, and has the ability to interfere with critical amino acid production essential for viruses. To quote one study, "The amount of lysine required to control herpes varied from case to case but a typical dose to maintain remission was 500 mg daily and active herpes required 1 to 6gm between meals to induce healing." Drs. Kagan, Griffith and Norins at the UCLA School of Medicine found of 45 patients receiving L-Lysine for herpes, only two failed to respond (a 96% success rate). The patients were receiving about 1500 mg L-Lysine daily. Lysine supplements (as opposed to foods high in this nutrient) can play an important role in staving off and reducing the severity of herpes-related cold sores. Results of a six-month trial involving more than 50 people indicate that lysine is far more effective than a placebo in preventing cold sores. Participants given a placebo had more than twice as many such infections as those taking lysine. Moreover, the herpes sores that did develop in the lysine group tended to be milder, and to heal faster, than the outbreaks in the placebo group. Lysine supplements may even prevent HSV outbreaks in chronic sufferers. General advice is to take 1,000 mg L-lysine three times a day with meals for flare-ups. If you are subject to recurrent outbreaks of cold sores, continue on a maintenance dosage of 1,000 mg per day. Not recommended: L-Arginine. Arginine promotes viral replication of herpes. NUTRITIONAL SUPPLEMENTS
Unless otherwise specified, the dosages recommended here are for adults. For children between the ages of 12 and 17, reduce the dose to 3/4 the recommended amount. For children between 6 and 12, use 1/2 the recommended dose, and for children under the age of 6, use 1/4 the recommended amount.
MEDICATIONS
Denavir (penciclovir) cream is clinically proven to be effective in treatment of oral cold sores (herpes cold sores or fever blisters) and reduces the duration of the infection by one day, on average.
The drug acyclovir (Zovirax), in oral (capsule or liquid) or topical (ointment) form, is sometimes prescribed for cold sores.
A new over-the-counter drug called docosanol (Abreva) has been approved by the FDA to reduce healing time and duration of symptoms of cold sores. The medication contains 10 percent docosanol, an antiviral agent. This is a topical medication that is rubbed into the sore five times a day for adults and children over the age of 12. Children younger than 12 should use this medication under a health care provider's supervision.
