TYPES OF HERPES VIRUSES - QUICK OVERVIEW
There are more than 90 viruses that belong to a family of animal viruses called herpes virdae. Of this family of viruses, at least seven are known to be important to humans.
Herpes Varicella-Zoster causes chicken pox and shingles.
Epstein-Barr Virus (EBV) causes infectious mononucleosis.
Cytomegalovirus (CMV), another member of the herpes family, can cause some cardiovascular diseases and eye disorders. It is often is carried without producing any symptoms, but can have devastating consequences for developing fetuses, newborns and for people with compromised, depressed immune systems.
Human Herpes Virus Type 6 (HHV-6) and 7 (HHV-7) are suspected of being capable of triggering autoimmune disorders, including multiple sclerosis, and roseola, a common illness of early childhood.
Human Herpes Virus Type 8 (HHV-8) is very closely related to the Epstein-Barr virus, and may lead to cancer of the bone, chronic fatigue syndrome, Kaposi's sarcoma, and infection of the lymphatic system.
This page primarily discusses the Herpes Simplex Virus Type 1 (HSV-1), which causes cold sores (fever blisters), and Herpes Simplex Virus Type 2 (HSV-2), which causes genital herpes.
Once the herpes virus enters the body, they never leave. They live in nerve cells, where the immune system cannot find them, and they become active from time to time as the immune system is depressed. The virus may lie dormant for long periods of time, until illness, sun-exposure, fatigue, stress, or some other factor causes the virus to break out in open sores again. Fortunately, after a period of time, the virus seems to burn itself out. Outbreaks rarely appear after the age of fifty, with the exception of the herpes zoster virus, which tends to reappear as shingles in older adults.

VARICELLA-ZOSTER VIRUS
Varicella-Zoster Virus (VZV) belong to the Herpes Virus Family. This virus causes two major diseases, chicken-pox (Varicella), an infection that usually occurs in childhood, and shingles (Zoster), that usually occurs later in life. Shingles (Zoster) is a reactivation of an earlier varicella infection.
CHICKENPOX
Chickenpox is a very common and highly contagious disease. The symptoms are usually mild, with a low-grade fever, general fatigue, and a characteristic itchy rash.
The chickenpox virus is transmitted by direct contact with an infected person or through the air when an infected person coughs or sneezes. A person can also contract chickenpox (but not shingles) from direct contact with a person with the shingles rash. Outbreaks are common within families and in schools and day care facilities. Symptoms do not appear until after a person reaches the infectious stage, so it is difficult to isolate those who are contagious. Repeated exposures to the virus during the incubation period can result in worse symptoms. One bout with chickenpox generally affords a lifetime immunity against the illness. Second attacks are possible but rare.
SHINGLES
The VZV is spread by respiratory aerosols or direct contact with skin lesions. If you have already had chickenpox, You cannot get shingles from someone else with shingles. However, if you have not had chickenpox, you can get chickenpox from close contact with the open blisters of someone with shingles. Once the blisters are crusted over, the virus can no longer be spread.
This is because the same virus that causes chickenpox also causes shingles and is considered contagious only to those never having chickenpox. Shingles can only occur after a person has had chickenpox.
Most adults have already contracted chickenpox. Chickenpox is a common childhood disease that causes a fever and a rash that itches maddeningly but rarely does any permanent damage. Once the varicella-zoster virus enters the body and has caused chickenpox, it does not ever go away. It may lie dormant in the spinal cord and nerve ganglia for years until activated, usually by a weakening (temporary or permanent) of the immune system. An estimated 90 percent of people who have had chickenpox are at risk of developing shingles. The trigger for shingles is unknown, but it is not caused by being around someone who also has shingles.
The virus affects the nerve endings in the skin. Shingles can appear anywhere on the body; however, it is most commonly found on the skin of the abdomen underneath the ribs, leading toward the navel. Other commonly affected areas are the vaginal tissues and the inside of the mouth.
When the varicella-zoster virus is activated, the infection spreads to the very ends of the nerves, causing them to send impulses to the brain that are interpreted as severe pain, itching, or burning, and rendering the overlying skin much more sensitive than usual.

FREQUENT SIGNS AND SYMPTOMS
CHICKENPOX
The rash is usually preceded by:
- Fever, usually low-grade between 100°F to 102°F, though in rare cases may be higher.
- Abdominal pain or loss of appetite.
- General feeling of unease and discomfort (malaise) or irritability.
- Sore throat.
- Dry cough.
- Fatigue.
- Mild headache.
- Flu-like symptoms.
Chickenpox first manifests itself as a fever and a headache, usually between 7 and 21 days after exposure to the virus.
The typical rash of chickenpox is made up of groups of small, round, itchy pimple-like blisters surrounded by reddish, inflamed skin. The rash usually begins as one or two lesions most commonly appear on the face, abdomen, or back, quickly spreading throughout the body including the trunk, scalp, face, arms, genitals, and legs. They are filled with fluid and look like water blisters, usually less than a quarter of an inch wide. The fluid leaks from swollen areas of the skin, forming a crust. These eruptions continue in cycles, lasting from 3 days to 1 week. The blisters and crusts are infectious and itchy, and scratching them can lead to bacterial infection and scarring. Once the scabs are gone, the individual is no longer infectious. Adults and older children who contract the infection tend to have more severe cases than do younger children.
The total number of blisters varies greatly from person to person. The rash may be more extensive or severe in children who have skin disorders such as eczema. Over four days, each blister tends to dry out and form a scab, which then falls off between 9 to 13 days later. An infected child should stay home and rest until the symptoms are gone.
Most children contract chickenpox before the age of 10 to 12 years old.
CAUSE
CHICKENPOX
Anyone who has not had chickenpox or the vaccine, is at risk for contracting the virus if exposed to someone with either chickenpox or shingles. The time between exposure to the virus and appearance of symptoms (called an incubation period) is between 10 and 20 days. The virus is spread through sneezing, coughing, and breathing - in other words, when someone with chickenpox sneezes or coughs, there are respiratory droplets with the VZV virus in the air. Then, you can breathe in those infected droplets and, if you have never had chickenpox or the vaccine, get chickenpox yourself.
Another possible way to get chickenpox, is to come into direct or indirect (like the clothes of someone with shingles) contact with discharge from VZV skin lesions.
The virus is contagious from two days before the rash appears until all of the lesions have crusted over.
RISK FACTORS
CHICKENPOX
- Exposure to VZV if you have neither had chicken pox nor received the vaccine.
- Age under 10.
- Late winter and early spring is the most common time that the virus is spread.
Chickenpox, which is caused by the varicella-zoster virus, is highly contagious to people not immune to it. The disease spreads quickly, especially in locations where people are in close contact, such as child care facilities, schools and families. The virus is transmitted by direct contact with the rash or by droplets dispersed into the air by coughing or sneezing.
A person who has chickenpox can transmit the virus for up to 48 hours before the telltale rash appears and remains contagious until all spots crust over. People who have been vaccinated against chickenpox are usually immune to the virus. The same is true of anyone who has had chickenpox in the past. People at risk of contracting chickenpox include anyone who has not been vaccinated or who has never had the disease.
PREVENTIVE MEASURES
CHICKENPOX VACCINE
The varicella virus vaccine (Varivax) has become a routine childhood immunization to prevent chickenpox. It is recommended for children between ages 12 months and 18 months. The vaccine is also recommended for older kids and adults who have never had chickenpox. If you (or your child) receive the vaccine before age 13, then you only need one dose. If you receive the vaccine when you are older than 13 (which may be recommended if you have not yet had chickenpox or received the vaccine), then you should get a second dose 1 to 2 months later.
In 2006, the Centers for Disease Control and Prevention (CDC) recommended a booster shot at 4 to 6 years old for further protection. The CDC also recommends that people 13 years of age and older who have never had chickenpox or received chickenpox vaccine get 2 doses of the vaccine at least 28 days apart.
A person usually has only one episode of chickenpox, but VZV can lie dormant within the body and cause a different type of skin eruption later in life called shingles (or herpes zoster). Getting the chickenpox vaccine significantly lowers your child's chances of getting chickenpox, but he or she may still develop shingles later.
Though the chickenpox vaccine does not guarantee you will not get chickenpox or shingles, it can reduce your chances of complications and reduce the severity of the disease. The vaccine is about 70 to 85 percent effective at preventing mild infection, and more than 95 percent effective in preventing moderate to severe forms of the infection. Healthy children who have had chickenpox do not need the vaccine - they usually have lifelong protection against the illness.
The vaccine is not approved for:
- Pregnant women.
- People with weakened immunity, such as those with HIV or people on immune-suppressing medications.
- People who are allergic to gelatin or the antibiotic neomycin.
Talk to your health care provider if you are unsure about your need for the vaccine. If you are planning on becoming pregnant, consult with your health care provider to make sure you are up to date on your vaccinations before conceiving a child. Be sure to wait the recommended time after receiving a vaccination before you become pregnant.
Parents typically wonder whether vaccines are safe. Since it became available, studies have continued to show the vaccine is safe and effective. Side effects are generally mild and include redness, soreness, swelling and, rarely, small bumps at the site of the shot. However, you may consider alternatives to conventional vaccinations (such as homeopathy) or simply avoiding people who are or may be contagious. Do your research before deciding whether a vaccine is right for you.
Chickenpox is very contagious - most kids with a sibling who's been infected will get it as well, showing symptoms about 2 weeks after the first child does. To help keep the virus from spreading, make sure your children wash their hands frequently, particularly before eating and after using the bathroom. And keep a child with chickenpox away from unvaccinated siblings as much as possible.
EXPECTED OUTCOME
Chickenpox generally resolve spontaneously in those not at high risk for complications. The goal, therefore, is to make you as comfortable as possible and to shorten the length of time that you have the pain and itching associated with chickenpox.
POSSIBLE COMPLICATIONS
Chickenpox usually runs its course in 2 weeks or less in otherwise healthy children. While chickenpox usually goes away on its own with non-serious complications like itching and scarring. Severe and potentially serious complications and sometimes fatal infections may occur, particularly in newborn infants, adults, and people whose immune systems are weakened.
HIGH RISK GROUPS
Chickenpox is normally a mild disease. But it can be serious and can lead to complications, especially in these high-risk groups:
- Newborns and infants whose mothers never had chickenpox or the vaccine.
- Teenagers.
- Adults.
- Pregnant women.
- People whose immune systems are impaired by medication, such as chemotherapy, or another disease.
- People who are taking steroid medications for another disease or condition, such as children with asthma.
- People with the skin inflammation eczema.
Anyone with an impaired immune system, such as a person whose immune system is suppressed by drugs or who has a disorders such as AIDS, can develop potential infections including:
- Encephalitis (a brain infection).
- Myocarditis (an infection of the heart muscle).
- Pneumonia.
- Secondary bacterial skin infection.
CHICKENPOX & PREGNANCY
A newborn baby also can develop complications from chickenpox, especially if infected prenatally or soon after birth. Pregnant women and anyone with immune system problems should not be near a person with chickenpox. If a pregnant woman who has not had chickenpox in the past contracts it (especially in the first 20 weeks of pregnancy), the fetus is at risk for birth defects, such as limb abnormalities, and she is at risk for more health complications than if she had been infected when she was not pregnant, including low fetal birth weight. If she develops chickenpox just before or after the child is born, the newborn is at greater risk for serious health complications and a serious, life-threatening infection. There is no risk to the developing baby if the woman develops shingles during the pregnancy.
If a pregnant woman has had chickenpox before the pregnancy, the baby will be protected from infection for the first few months of life, since the mother's immunity gets passed on to the baby through the placenta and breast milk. If you are pregnant and not immune to chickenpox, talk to your midwife or health care provider about the risks to you and your unborn baby.
Those at risk for severe disease or serious complications - such as newborns whose mothers had chickenpox at the time of delivery, patients with leukemia or immune deficiencies, and children receiving drugs that suppress the immune system - may be given varicella zoster immune globulin after exposure to chickenpox to reduce its severity.
About 3.5 to 4 million people, mostly children, get chickenpox every year. The Centers for Disease Control & Prevention (CDC) report that about 11,000 people are hospitalized and about 100 people die in the United States each year due to complications from the disease.
CHICKENPOX & SHINGLES
Anyone who had chickenpox is at risk of a latent illness called shingles. After a chickenpox infection, some of the varicella-zoster virus may remain in your nerve cells. Many years later, the virus can reactivate and resurface as shingles - a painful band of short-lived blisters. About one in 10 adults who have had chickenpox experiences shingles. The virus is more likely to reappear in older adults and people with weakened immune systems.
Shingles can lead to its own complication - a condition in which the pain of shingles persists long after the blisters disappear. This complication, called postherpetic neuralgia, can be severe.
TREATMENT
CONVENTIONAL MEDICAL TREATMENT
In otherwise healthy children, chickenpox typically requires no medical treatment. Your health care provider may prescribe an antihistamine to relieve itching, but for the most part, the disease is allowed to run its course.
A virus causes chickenpox, so your health care provider will not prescribe antibiotics. However, antibiotics may be required if the sores become infected by bacteria. This is pretty common among children because they often scratch and pick at the blisters. Pneumonia, another complication of chickenpox, may also be treated with antibiotics. Treatment for encephalitis is usually with antiviral drugs. Hospitalization may be necessary.
The antiviral medicine acyclovir may be prescribed for people with chickenpox who are at risk for complications. The drug, which can make the infection less severe, must be given within the first 24 hours after the rash appears. It is most often recommended for those at risk for severe chickenpox such as premature babies, adolescents, and adults. Acyclovir can have significant side effects, so it is only given when necessary. Your health care provider can tell you if the medication is right for your child.
Another drug called immune globulin intravenous (IGIV) may be suggested to lessen the severity of the disease if given within 24 hours after the rash first appears. Other antiviral drugs, such as valacyclovir (Valtrex) and farnciclovir (Famvir), also may lessen the severity of the disease, but have been approved for use only in adults. In some cases, your health care provider may recommend getting the chickenpox vaccine after exposure to the virus. This can possibly prevent the disease or lessen its severity.

DEALING WITH THE DISCOMFORTS OF CHICKENPOX
You can help relieve the itchiness, fever, and discomfort of chickenpox by:
- Using cool wet compresses or giving baths in cool or lukewarm water every 3 to 4 hours for the first few days. It can also help provide a playful diversion for a young child as well as relieve itching. Oatmeal (uncooked) or colloidal oatmeal baths, available at the supermarket, pharmacy, or online. Colloidal oatmeal is a finely ground oatmeal that is made for the tub, not the stove. (Baths do not spread chickenpox.)
- Wash the blisters twice a day with regular soap and water but do not bandage them.
- Apply cool, wet compresses to the blisters to relieve the pain and itch or soak in a cool water bathtub. Use water or water mixed with white vinegar - 1 ounce of vinegar to 32 ounces of water. Apply the compresses three times a day.
- Warm baths with a tablespoon of salt, Epsom salts, or baking soda added can ease some of the discomforts, such as itching and pain, caused by the blisters. After the bath, patting (not rubbing) the body dry gently and keep the lesions dry.
- Putting calamine lotion (dabbing the spots) on itchy areas (but do not use it on the face, especially near the eyes) may provide some relief. Apply an anti-itch cream to the affected area. A non-prescription hydrocortisone cream, containing at least 1 percent hydrocortisone, can temporarily relieve the itch. A non-prescription oral antihistamine, such as diphenhydramine (Benadryl, others), may be helpful if itching is severe.
- Giving your child foods that are cold, soft, and bland because chickenpox in the mouth may make drinking or eating difficult. Avoid feeding your child anything spicy, highly acidic, hard or crunchy, or especially salty, like orange juice or pretzels since these can be irritating to mouth sores.
- Asking your health care provider or pharmacist about pain-relieving creams to apply to sores in the genital area.
- Giving your child acetaminophen (Tylenol, others) or ibuprofen (Advil, Motrin, others) regularly to help relieve pain if your child has mouth blisters and to treat a fever. These will not cure chickenpox, but they can help lower the fever. Never use aspirin to reduce pain or fever in children with chickenpox because aspirin has been associated with the serious disease Reye's syndrome, which can lead to liver failure and even death. Do not try to treat a high fever without consulting with your health care provider.

- As much as possible, discourage infants and children from scratching. Scratching can cause scarring, slow down the healing process and increase the risk that the sores will become infected. This can be difficult for them, so consider putting loose fitting cotton gloves, flannel mittens or socks on your child's hands to prevent scratching during sleep. In addition, trim fingernails and keep them clean to help lessen the effects of and damage from scratching, including broken blisters and infection. If itching is particularly severe or irritating, an antihistamine may need to be prescribed for relief.

CONTACTING YOUR HEALTH CARE PROVIDER
Most chickenpox infections require no special medical treatment. But sometimes, there are problems. Call your health care provider if your child:
- Has fever that lasts for more than 4 days or rises above 102°F. Consult a health care provider immediately if symptoms include dizziness, a fever higher than 103°F, rapid heartbeat, tremors and any of the other symptoms listed below.
- Has a severe cough or trouble breathing.
- Has an area of rash that leaks pus (thick, discolored fluid) or becomes red, warm, swollen, or sore.
- Has a severe headache.
- Is unusually drowsy or has trouble waking up.
- Has trouble looking at bright lights.
- Has difficulty walking.
- Seems confused.
- Seems very ill or is vomiting.
- Has a stiff neck.
Call your health care provider if you think your child has chickenpox, if you have a question, or if you are concerned about a possible complication. Your health care provider can guide you in watching for complications and in choosing medication to relieve itching. When taking your child to the health care provider, let the office know in advance that your child might have chickenpox. It is important to ensure that other children and any pregnant women in the office are not exposed - for some of them, a chickenpox infection could cause severe complications.
RECOMMENDATIONS
Get plenty of rest. Children should be kept home from school or daycare and in bed while sick. Keep infected child away from elderly people, newborn babies, and pregnant women who have not had chickenpox.
Stay out of bright sunlight and keep interior areas dimly lit. Do not expose the patient to bright lights until he or she is completely healed.
Apply vitamin E and vitamin A, alternately, directly on the sores. Or try using L-Lysine Cream from a health food store.

Try using essential oils. Bergamot Oil, Calophyllum Oil (related to St. John's Wort), Eucalyptus Oil, Geranium Oil, Goldenseal Oil, Lemon Oil can be used singly or in combination. An experienced homeopath assesses all of these factors when determining the most appropriate treatment for each individual.
Homeopathic remedies to alleviate chickenpox symptoms may include:
- Antimonium Crudum. For irritable children who are extremely sensitive to touch and may have a thick white coating on the tongue.
- Antimonium Tartaricum. For large, slowly appearing pox lesions.
- Mercurius. For large, pus-filled pox which may ooze; this remedy is most appropriate for individuals who sweat profusely and may have enlarged lymph nodes.

Be sure to call ahead for an appointment, to avoid waiting and possibly infecting others in a crowded waiting room.
Also, be sure to let your health care provider know if any of these complications occur:
- The rash spreads to one or both eyes.
- The rash gets very red, warm or tender, indicating a possible secondary bacterial skin infection.
- The rash is accompanied by dizziness, disorientation, rapid heartbeat, shortness of breath, tremors, loss of muscle coordination, worsening cough, vomiting, stiff neck or a fever higher than 103°F.
