DESCRIPTION

Anxiety disorder is a far more common problem than was once thought. It can affect people in their teenage years through middle age and later. Anxiety disorder appears to affect twice as many women as men, though there may not actually be that wide a disparity between the sexes. Psychologists believe that men are far less prone to report or even acknowledge having a problem of this nature.

TYPES OF ANXIETY DISORDERS

There are many types of anxiety disorders that include panic disorder, obsessive compulsive disorder, post traumatic stress syndrome, social anxiety disorder, specific phobias, and generalized anxiety disorder. Anxiety is a normal human emotion that everyone experiences at times. Many people feel anxious, or nervous, when faced with a problem at work, or before taking a test or making an important decision. Anxiety disorders, however, are different. They can cause such distress that it interferes with a person's ability to lead a normal life. An anxiety disorder is a serious mental illness. For people with anxiety disorders, worry and fear are constant and overwhelming, and can be crippling.

There are several recognized anxiety disorders, including:
  • Panic Disorder: People with this condition have feelings of terror that strike suddenly and repeatedly with no warning. Other symptoms of a panic attack include sweating, chest pain, palpitations (irregular heartbeats) and a feeling of choking, which may make the person feel like he or she is having a heart attack or "going crazy."


  • Obsessive-Compulsive Disorder (OCD): People with OCD are plagued by constant thoughts or fears that cause them to perform certain rituals or routines. The disturbing thoughts are called obsessions, and the rituals are called compulsions. An example is a person with an unreasonable fear of germs who constantly washes his or her hands.


  • Post-Traumatic Stress Disorder (PTSD): PTSD is a condition that can develop following a traumatic and/or terrifying event, such as a sexual or physical assault, the unexpected death of a loved one, or a natural disaster. People with PTSD often have lasting and frightening thoughts and memories of the event, and tend to be emotionally numb.


  • Social Anxiety Disorder: Also called social phobia, social anxiety disorder involves overwhelming worry and self-consciousness about everyday social situations. The worry often centers on a fear of being judged by others, or behaving in a way that might cause embarrassment or lead to ridicule.


  • Specific Phobias: A specific phobia is an intense fear of a specific object or situation, such as snakes, heights or flying. The level of fear usually is inappropriate to the situation and may cause the person to avoid common, everyday situations.


  • Generalized Anxiety Disorder: This disorder involves excessive, unrealistic worry and tension, even if there is little or nothing to provoke the anxiety.





SIGNS & SYMPTOMS

Anxiety disorder can be either acute of chronic.

ACUTE ANXIETY DISORDER - THE PANIC ATTACK

Acute anxiety disorder manifests itself in episodes commonly known as panic attacks. A panic attack is an instance in which the body's natural "flight or fight" reaction occurs at the wrong time. This is a complex, involuntary physiological response in which the body prepares itself to deal with an emergency situation. Stress causes the body to produce more adrenal hormones, especially adrenaline. the increased production of adrenaline causes the body to step up its metabolism of proteins, fats, and carbohydrates to quickly produce energy for the body to use. In addition, the muscles tense, and heartbeat and breathing become more rapid. Even the composition of the blood changes slightly, making it more prone to clotting.

PANIC DISORDERS

In the face of a threat such as an assault, an accident, or a natural disaster, this type of reaction is perfectly normal and helpful for survival. At other times, the symptoms caused by a surge of adrenaline can be distressing and frightening.

Panic disorders consist of separate and intense periods of fear or feelings of doom developing over a very short time frame, usually 10 minutes, and associated with at least 4 of the systems associated with panic attacks.

A person having a panic attack often is overwhelmed by a sense of impending disaster or death, which makes it impossible to think clearly. Other feelings that can accompany a panic attack include:
  • Sense of choking.
  • Shortness of breath, breathing difficulties.
  • A smothering, claustrophobic sensation.
  • Heart palpitations.
  • Chest pain.
  • Dizziness. Feeling weak or faint.
  • Hot flashes and/or chills.
  • Trembling.
  • Numbness or tingling sensations in the extremities.
  • Sweating.
  • Nausea.
  • Feeling a loss of control.
  • A feeling of unreality.
  • A distorted perception of the passage of time.

Eventually the disorder can have other cumulative effects, such as generalized aches and pains, muscular twitching and stiffness, depression, insomnia, nightmares and early waking, decreased libido, and abnormal feelings of tension with no accompanying inability to relax. Women may experience changes in the menstrual cycle and increased premenstrual symptoms.

Panic attacks are usually abrupt and intense. They can occur at any time of day or night, lasting from several seconds up to half an hour. To the panic sufferer, it often feels as though they are much longer. A person having a panic attack often believes that he or she is experiencing a heart attack or stroke. The attacks themselves are very unpredictable; some people experience one every few weeks, while others may have several a day.

CHRONIC ANXIETY DISORDER

Chronic anxiety is a milder, more generalized form of this disorder. Many sufferers feel a vague sense of anxiety much of the time, but the intensity of the feeling does not reach the levels of those in an actual panic attack. They may feel chronically uneasy, especially in the presence of other people, and tend to startle easily. Headaches and chronic fatigue are common among people with this form of the disorder.

Generalized anxiety disorder can begin at any age, but the onset typically occurs in one's 20s or 30s. Some people with chronic anxiety disorder also suffer from occasional panic attacks.

Generalized anxiety disorder is excessive and unrealistic worry over a period of at least 6 months associated with 3 of the following:
  • Restlessness.
  • Easy fatigue.
  • Difficulty concentrating.
  • Irritability or explosive anger.
  • Muscle tension.
  • Sleep disturbances.
  • Personality changes such as becoming less social.

PHOBIC DISORDERS

Phobic disorders are intense, persistent, and recurrent fear of certain objects (such as snakes, spiders, blood) or situations (such as heights, speaking in front of a group, public places). These exposures may trigger a panic attack.

STRESS DISORDERS

Stress disorders consists of anxiety caused by the exposure to either death or near-death circumstances such as fires, floods, earthquakes, shootings, automobile accidents, or wars, for example. This is also known as post-traumatic stress disorder. The traumatic event is re-experienced in thoughts and dreams. Common behaviors include the following:
  • Avoiding activities, places, or people associated with the triggering event.
  • Difficulty concentrating.
  • Difficulty sleeping.
  • Being hyper-vigilant (you closely watch your surroundings).
  • Feeling a general sense of doom and gloom with diminished emotions such as loving feelings or aspirations for the future.

Symptoms such as chest pain, shortness of breath, palpitations, dizziness, fainting, and weakness generally should not be attributed to anxiety and require evaluation by a health care provider.





CAUSES & TRIGGERS

The exact cause of anxiety disorders is unknown; but anxiety disorders. Like other forms of mental illness, anxiety disorders are not the result of personal weakness, a character flaw or poor upbringing. As scientists continue their research on mental illness, it is becoming clear that many of these disorders are caused by a combination of factors, including changes in the brain and environmental stresses.

Like certain illnesses, such as diabetes, anxiety disorders may be caused by chemical imbalances in the body. Studies have shown that severe or long-lasting stress can change the balance of chemicals in the brain that control mood. Other studies have shown that people with certain anxiety disorders have changes in certain brain structures that control memory or mood. In addition, studies have shown that anxiety disorders run in families, which means that they can be inherited from one or both parents, like hair or eye color. Moreover, certain environmental factors, such as a trauma or significant event, may trigger an anxiety disorder in people who have an inherited susceptibility to developing the disorder.

Panic attacks are often triggered by stress (conscious or unconscious) or certain emotions, but may also occur in response to certain foods, drugs, or illness. Food allergies and hypoglycemia are both common in people with this disorder, and can promote panic attacks. An attack may follow ingestion or overindulgence in caffeine-based stimulants such as tea or coffee. Some attacks occur with no apparent cause. The unpredictability of the attacks makes them even more distressing.

Many people with acute anxiety disorder become fearful of being alone and of visiting public places because they fear having a panic attack. Of course this only adds to the level of anxiety and leads to their lives being abnormally restricted. Many psychologists believe that at least in some cases, panic attacks are self-induced; that is, the fear of a panic attack is the very thing that brings one about.

For years, panic attacks were dismissed as a psychosomatic phenomenon. However, repeated studies have shown that this disorder has a real, physical basis. Experts believe that panic attacks are caused principally by a malfunction in brain chemistry, wherein the brain sends and receives false "emergency signals". Researchers have recently identified a genetic factor that appears to influence anxiety in women. Combining DNA analysis, brain activity recordings, and psychological testing, investigators at the National Institute on Alcohol Abuse (NIAAA) found that both Caucasian and Native American women with the same gene variant had similar electroencephalograms (EEGs), recordings of the brain's electrical activity that indicated tendencies toward an anxious temperament. This study was reported in the journal Psychiatric Genetics.

COMT is the acronym for a major enzyme responsible for the metabolism of certain neurotransmitters (chemical messengers of the nervous system), including norepinephrine, which affects anxiety. Other studies have confirmed a genetic link that might explain why more women than men experience anxiety disorders. In general, women have been found to have lower COMT levels than men. If the gene that encodes the COMT enzyme is faulty, the metabolism of these neurotransmitters is affected. Hyperactivity in certain areas of the brain causes the release of norepinephrine, which cases the pulse, blood pressure, and breathing to become more rapid, producing the classic symptoms of panic attack.

According to the Mayo Clinic researchers, between 10 and 20 percent of Americans will have a panic attack at some time in their lives. Panic attacks are now recognized as a potentially disabling but treatable condition.

Anxiety disorder may be hereditary to some extent, as it seems to run in families. Some cases may be linked to a relatively harmless abnormality of heart function called mitral valve prolapse. Anxiety disorder manifests itself in different ways, but health care providers agree that conflict, whether internal or interpersonal, promotes a state of anxiety.





PREVENTION

Anxiety disorders cannot be prevented; however, there are some things you can do to control or lessen symptoms:
  • Stop or reduce your consumption of products that contain caffeine, such as coffee, tea, cola and chocolate.


  • Ask your health care provider, pharmacist, or qualified herbalist before taking any over-the-counter medicines or herbal remedies. Many contain chemicals that can increase anxiety symptoms.


  • Seek counseling and support after a traumatic or disturbing experience.




DIAGNOSIS & TREATMENT


CONVENTIONAL MEDICAL DIAGNOSIS

Anxiety disorders affect about 19 million adult Americans. Most anxiety disorders begin in childhood, adolescence and early adulthood. They occur slightly more often in women than in men, and occur with equal frequency in whites, African-Americans, and Hispanics.

If symptoms are present, the health care provider will begin an evaluation by asking you questions about your medical history and performing a physical examination. Although there are no laboratory tests to specifically diagnose anxiety disorders, the health care provider may use various tests to look for physical illness as the cause of the symptoms.

If no physical illness is found, you may be referred to a psychiatrist or psychologist, mental health professionals who are specially trained to diagnose and treat mental illnesses. Psychiatrists and psychologists use specially designed interview and assessment tools to evaluate a person for an anxiety disorder.

The health care provider bases his or her diagnosis on the patient's report of the intensity and duration of symptoms - including any problems with daily functioning caused by the symptoms - and the health care provider's observation of the patient's attitude and behavior. The practitioner then determines if the patient's symptoms and degree of dysfunction indicate a specific anxiety disorder.

DEPRESSION, STRESS, ANXIETY DISORDER & BIPOLAR DISORDER

The symptoms of depression, anxiety disorder and bipolar disorder have similarities, but require different treatments. The sad, hopeless feeling that just can't go on, affecting your job, your life, and it seems like depression, but it could be something more - many people with depression also experience some degree of anxiety - anxiety that goes beyond the typical tension we experience when we face life's challenges. For people with an anxiety disorder, the overwhelming worry and fear is constant - with obsessive thoughts, feelings of panic, trouble sleeping, heart palpitations, cold or sweaty hands. Very often, we find that people have more than one condition - both depression and anxiety disorder. It is often very hard to find patients who are depressed who do not also have anxiety. It is equally hard to find people with anxiety who do not have some depression.

MOOD DISORDERS SIMILAR TO DEPRESSION

Indeed, sadness, depression, and anxiety are often triggered by life events and the symptoms are not easily separated out. Frequently financial, relationship, and family problems can trigger anxiety and sadness, so we consider these feelings to be normal. They are not normal when the feelings are extremely intense, when they impair everyday functioning, affect quality of life. When all that is happening, it becomes difficult to solve the very problems that started the depression. In addition to anxiety and depression, there might be something else going on such as bipolar disorder. This is a condition that involves shifts in a person's mood from severe depression to manic phases. A person with bipolar disorder will experience soaring highs, restlessness, difficulty concentrating, racing thoughts, impulsive decisions, reckless behavior, and poor judgment. In many cases there is normal mood in between the phases. Anxiety is nearly always part of bipolar disorder. Not only is it difficult to distinguish between anxiety and mania, it is sometimes impossible.

ANTIDEPRESSANT MEDICATIONS

Because of the difficulty in diagnosing these mood disorders, it is important to talk to your health care provider candidly about what you have been feeling. It it also crucial that your health care provider take time to ask enough questions to help with proper diagnosis and treatment. Many people go to a general practitioner first. They have been feeling depressed and think they may need an antidepressant. But if that practitioner is very busy, he or she can not do much evaluation. Under those circumstances, an antidepressant is often prescribed - yet that may or may not be the right choice. Antidepressants are used to treat both anxiety disorders and depression. However, someone with bipolar disorder needs a different set of drugs such as a mood stabilizer and anti-manic medication. There are several types of mood-stabilizing drugs, which include medications like lithium and anti-convulsive drugs such as Depakote or Lamictal. The danger in giving an antidepressant to someone with bipolar disorder is that it could trigger a manic episode. Manic episodes can be dangerous, because you have very poor judgment, tend to use more drugs, drive recklessly, spend a lot of money, have much more sex - and have it completely unprotected. There's a higher risk of high-risk behaviors because there is poor judgment.

Treating a mood disorder takes time. Psychiatrists say it is most important to recognize that something is wrong. Then see a doctor regularly. It is a good sign if your health care provider wants to see you more than once before prescribing something. It is better if your health care provider wants to see you regularly, rather than prescribe the medication and tell you to check back with him or her in six months.

Because bipolar disorder is condition that is ongoing but not always evident, the National Institute of Mental Health recommends long-term preventive treatment. The institute notes that a combination of medication and psychotherapy works best to keep the disorder under control over time. With most people, things are more complicated than they seem on the first visit and almost always, there is something more going on, and a health care provider just can not know all that in one visit. It is erroneous if they think they can and if the patient expects to be "fixed" after one visit to their health care provider. It is important to get treated for any mood disorder, because it can affect your own quality of life as well as the people around you. When you are depressed for any reason you simply do not care about yourself. You do not care about those around you. And often, you do not have the motivation to get help because you feel hopeless. You might think there is no way to solve your problems. But that is not true if you can find the proper health care provider that can treat your depression and so you will be better able to find solutions specific to your problems.

When treating depression, several medication options are available. Some of the most commonly used include the selective serotonin reuptake inhibitors (SSRIs) such as Prozac, Zoloft, and Paxil. Other options include older medications such as the tricyclic antidepressants and monoamine oxidase inhibitors (MAOIs), which may be less tolerated and cause more side effects. Other medications include Wellbutrin, trazodone (Desyrel), Effexor, and Remeron.

Your health care provider can determine which medication is right for you. Remember that medications usually take 4 to 8 weeks to become fully effective. And if one medication does not work, there are many others to try before giving up.

In some cases, a combination of antidepressants may be necessary. Sometimes an antidepressant combined with a different type of medication, such as a mood stabilizer (such as lithium) or anti-anxiety drug, is the most effective treatment.

Side effects vary, depending on what type of antidepressant medication you are taking, and may improve once your body adjusts to the medication.

If you decide to stop taking your antidepressants, it is important that you gradually reduce the dose over a period of several weeks. Quitting antidepressants abruptly can cause withdrawal symptoms. It is important to discuss quitting (or changing) medications with your health care provider first.

Antidepressants, particularly the SSRIs, may also be effective in treating many types of anxiety disorders. Other anti-anxiety medications include the benzodiazepines, such as Valium, Ativan and . These drugs do carry a risk of addiction so they are not as desirable for long term use. Other possible side effects include drowsiness, poor concentration and irritability.

CONVENTIONAL MEDICAL TREATMENT

Fortunately, much progress has been made in the last two decades in the treatment of people with mental illnesses, including anxiety disorders. Although the exact treatment approach depends on the type of disorder, one or a combination of the following therapies may be used for most anxiety disorders:
  • Medication: Medicines used to reduce the symptoms of anxiety disorders include anti-depressants and anxiety-reducing medications.


  • Psychotherapy: Psychotherapy (a type of counseling) addresses the emotional response to mental illness. It is a process in which trained mental health professionals help people by talking through strategies for understanding and dealing with their disorder.


  • Cognitive-Behavioral Therapy: People suffering from anxiety disorders often participate in this type of psychotherapy in which the person learns to recognize and change thought patterns and behaviors that lead to troublesome feelings.


  • Dietary & Lifestyle Changes.


  • Relaxation Therapy.

  • A number of different drugs are used to block panic attacks. Their use must be carefully monitored by a health care provider. The effectiveness of any given drug varies from individual to individual, and all drugs used for this disorder can cause unpleasant side effects. Alprazolam (Xanax), one of the drugs most commonly prescribed for this illness, has varying effectiveness and can cause drowsiness and lightheadedness. It can also be very addictive. The risk of dependence and its severity seem to be higher if this drug is taken in relatively high doses (more than 4 milligrams per day) and for more than 8 weeks.


  • Recreational drugs such as marijuana can cause anxiety attacks.
