BASIC INFORMATION
The majority of people with HIV and/or AIDS in the United States are men, but the incidence of AIDS in women is rising nearly six times as fast as that in men. The AIDS epidemic disproportionately affects women who are members of racial and ethnic minorities. According to the U.S. Centers for Disease Control, 1 in 98 black women and 1 in 222 Hispanic women between the ages of 27-39 years of age are infected, compared with only 1 in 1,667 white women. Being a member of a particular racial or ethnic group does not mean a woman is any more susceptible to AIDS, however. Instead, it reflects the fact that members of minority groups are more likely to live in places with a high incidence of HIV infection. In the early years of the epidemic, most women with AIDS contracted the disease through intravenous drug use, but sexual contact has now surpassed intravenous drug use as the leading means of transmission among women.
Most women remain undiagnosed as HIV-positive until after the onset of AIDS or until they give birth to a child with HIV who becomes ill. This delay in diagnosis can have grave consequences for survival, and has contributed to the continuing myth that women's expected survival time is shorter than that for men. Experts say that if women were diagnosed at the same point in the disease as men are, their average length of survival would be essentially the same. Compounding the problems women with HIV face is the fact that the majority of them come from economically disadvantaged backgrounds, and their access to quality health care is often limited. In contrast, the gay men with HIV who were the primary ones affected in the early days of the epidemic were generally more affluent and lived in areas with better medical resources. This factor alone has helped to distort the figures kept by various advocacy groups and medical researchers. Moreover, until relatively recently, the list of opportunistic infections considered in diagnosing AIDS did not include diseases that are unique to women, such as chronic vaginal candidiasis (yeast infection). Thus, even a woman who was HIV-positive and had one or more opportunistic infection might not have qualified for an official diagnosis of AIDS.
Because recurrent vaginal candidiasis is the most common early indicator of HIV infection in women, the FDA mandated in 1992 that manufacturers of certain over-the-counter medications include a new warning label on their products. The warning states that frequent vaginal yeast infections, especially when persistent or recurrent, can be the result of a serious medical condition, such as HIV infection. The label advises women with these symptoms to consult with their health care providers.
Other infections and illnesses in women that should raise concern about possible HIV infection are pelvic inflammatory disease, cervical dysplasia (precancerous changes in the cervix), yeast infections of the mouth and throat, and any sexually transmitted disease, such as genital ulcers and warts, and herpes virus infection.
