HIV & AIDS - QUICK REVIEW

HIV is the human immunodeficiency virus, the virus that causes acquired immunodeficiency syndrome (AIDS). Millions of people worldwide now carry the virus. Many people who are HIV-positive look and feel healthy - only through blood testing can the presence of the virus be detected. Others have varying degrees of illness as a result, the most serious consequence of HIV infection being full-blown AIDS.

HIV INFECTION

HIV is usually passed through sexual or blood-to-blood contact. Infected mothers can pass the virus to their children, either during birth or through breast-feeding. It is not passed through coughing, or sneezing, nor through everyday contact, such as shaking hands.

Most people who are infected with HIV are not aware at first that they have it. While the virus can sometimes cause a flu-like illness two to four weeks after infection, it usually takes two to five years for symptoms - including oral thrush and other mouth lesions, intestinal parasites, prolonged and unexplained fatigue, swollen glands, unexplained fever lasting more than 10 days, excessive night sweating, sore throat, and changes in bowel habits - to appear. Antigen tests can detect HIV infection before symptoms appear, but even frequent HIV testing, usually cannot detect the virus in the first six months of the infection. Once the virus does appear, health care providers check and individual's viral load, or the amount of virus present in the bloodstream. In many cases, though, HIV is discovered only if the infected person develops one or more of the opportunistic infections or cancers associated with AIDS.

HIV affects immune-system cells called T-cells. There are two types, T-helper cells, also known as CD4 cells, help the immune system respond against bacteria, viruses and cancerous cells. T-suppressor cells stifle the immune system's response against these invaders, a process meant to keep the system from becoming overactive. The decrease in T-helper cells and the increase in T-suppressor cells results in and increased susceptibility to infection and cancer.

The key to understanding HIV infection is understanding that T-helper cells are destroyed not only by HIV, but also by the immune system itself. HIV changes the surface of T-helper cells so that they are recognized as foreign matter and eliminated by T-suppressor cells. As long as the immune system's response to the infected T-helper cells is not too strong, there is relatively little damage to the immune system as a whole. In this steady state, the symptoms of AIDS do not appear. Stimulating the immune system without regard to which components of the system will be activated, however, makes HIV worse, not better.

The precise relationship between HIV and the immune system was discovered when health care providers gave tetanus booster, which acts as an immune-system stimulant, to volunteers with HIV. The health care providers expectation was that boosting the immune system would lower the amount of HIV in the volunteer's bloodstream. Instead, the amount of the virus present in the volunteer's blood increased 200 to 400 percent. Even worse, their remaining virus-free blood cells became more susceptible to HIV infection. On the other hand, medical attempts to depress the immune system in order to treat HIV have also failed.

The most effective herbal approach to healing is to deal not with the immune system but with the virus itself. Before beginning or using any herbal or nutritional therapy, be sure to discuss it with a health care provider familiar with these therapies and HIV/AIDS, who can determine if a specific herb or therapy is appropriate for any specific stage of the disease.

AIDS

AIDS (acquired immune deficiency syndrome) is a disease in which the immune system breaks down and fails to protect the body against infection. It develops as the result of infection with the human immunodeficiency virus (HIV). HIV usually is passed through sexual or blood-to-blood contact.

Health care providers diagnose AIDS when a person's total count of T-cells, the longest-living immune cells that orchestrate the immune response, drops to 200 or less (compared with from 1,000 to 1,500 in a healthy person). But health care providers also take note of any decrease in the ratio of T-helper cells to T-suppressor cells. T-helper cells are those cells that medicate the immune system's chemical defenses against bacteria, viruses, and cancerous cells. T-suppressor cells, on the other hand, serve as a check on the T-helper cells' response to infection to prevent the destruction of healthy tissues. Health care providers assay viral load, or the number of viruses present in the bloodstream.

The body's response to the initial HIV infection, and to the resulting loss of immune function, varies from person to person. In some people, the progression from HIV to AIDS follows a recognized pattern. Fifty to 70 percent of infected people experience rashes or mild flulike symptoms 3 to 6 weeks after infection. These symptoms, go away and there are no new signs of disease until there is a secondary infection with a different microorganism. At that point, HIV becomes symptomatic. This stage, once called AIDS-related complex (ARC), is now referred to as early symptomatic AIDS. However, in other people, the even more devastating symptoms seen in full-blown AIDS are the first symptoms of infection.

The decrease in T-helper cells and the increase in T-suppressor cells results in an increased susceptibility to infections and cancer. In 50 percent of all people with AIDS, the first symptoms is Pneumoncystis carinii pneumonia. This is a bacterial infection that takes hold in the body only in the absence of a normal immune defense. Many people develop Kaposi's sarcoma, a connective tissue cancer that causes purplish lesions and that is almost unknown except in people with AIDS. Other infections commonly experienced by people with AIDS include those caused by cytomegalovirus. Epstein-Barr virus, herpes, Salmonella, Staphylococcus (staph), Streptococcus (strep), tuberculosis, and yeasts. In addition to multiple infections, people with AIDS are subject to malnutrition. In time there usually is muscle wasting, persistent loss of body-weight mass, and chronic diarrhea, weakness, and fever.

There are a number of conventional drug treatments for AIDS that greatly extend the symptom-free period and life expectancy. Especially in the case of the AIDS cocktails, it is critical not to use herbs as alternatives to conventional treatment. The AIDS treatment regimen must be followed exactly even with herbal treatment; herbs may make its side effects more bearable and make relapses less frequent. Before using any herbal treatments for AIDS, be sure to discuss them with your health care provider, who can determine whether a specific supplement is appropriate within your overall treatment plan.

Use immune-stimulant herbs, other than Cat's Claw, with caution, and inform your health care provider if using immune-stimulant herbs, if you are on any kind of "cocktail" treatment.





NUTRITIONAL & DIETARY RECOMMENDATIONS

IMPORTANT: Pay special attention to meeting your nutritional needs and requirements, and keep in mind that a higher than normal intake of nutrients will probably be necessary.

  • Increase your intake of fresh fruits and vegetables. Eat a diet consisting of 75 percent raw foods, organically grown if possible (avoid foods that have been treated with pesticides and other sprays), plus lentils, beans, seeds, nuts, and whole grains, including brown rice and millet, and also non-acid forming fruit, such as bananas, all berries, peaches, apples, and melons. Raw foods are particularly important because cooking depletes foods of their vital enzymes.


  • Eat plenty of cruciferous vegetables, such as broccoli, Brussels sprouts, cabbage, and cauliflower. Also, consume yellow and deep-orange vegetables such as carrots, pumpkin, squash, and yams.


  • Consume plenty of live juices. Juicing is extremely beneficial for supplying nutrients. Green drinks made from leafy greens such as kale, spinach, beet greens, and carrot and beet root juice, should be consumed on a daily basis, with garlic and onion added. Kyo-Green from Wakunaga is an excellent green drink product that contains chlorophyll, protein, vitamins, minerals, and enzymes. Take this drink three time daily.   • Daily nutritional "smoothies" made of fresh fruits and vegetables as well as fresh juices are particularly good and beneficial and easy to digest. Supplemental nutrients, oils, powders and extracts can be added to the juice or smoothie for easier intake and digestion.

  • People with AIDS who acquired the virus through sharing needles may be subject to vitamin A deficiencies. The best source of vitamin A for people with AIDS is a diet that includes daily servings of dark-green, yellow, or orange vegetables and fruits.


  • Cooking tomatoes rather than eating them raw has been shown to more than double the effectiveness of the lycopene tomatoes contain. Lycopene, a phytochemical found in tomatoes, has been found to reduce the risk of prostate and lung cancer. The incidence of digestive cancers, such as those of the mouth, esophagus, stomach, colon, intestines, and rectum, may also be lowered with correct dosages of lycopene. A study reported in the December 1995 issue of the Journal of the National Cancer Institute found a 45 percent lower risk of prostate cancer in men who regularly consumed ten or more servings of tomatoes or tomato-based foods per week than in those who did not.


  • Drink steam-distilled water (not tap water) or at least top quality bottled water, and lots of it - 8 or 10 8-ounce glasses daily - to flush out toxins from the body. All cells and organ systems need water. Drink plenty of water even if you are not thirsty. The organs, and especially the brain, become dehydrated long before thirst develops. An adequate water supply can prevent kidney problems. This is especially important if you are taking indinavir sulfate (Crixivan).


  • Eat unripened papaya (including a few of the seeds), fresh pineapple (contains bromelain), and Aspergillus oryzae (a type of fungus) frequently. These foods are good sources of proteolytic enzymes, which are crucial for proper digestion of foods and assimilation of nutrients. Without enzymes, the body cannot be supplied with the energy it needs for its activities. Enzymes can also be taken in supplemental form. These help in digestion in the lower stomach and intestinal tract.


  • Eat onions and garlic, or take garlic in supplement form (see under Nutrients below).


  • Add shiitake, reishi, and maitake mushrooms to the diet, or take them in supplement form (see under Nutrients below).


  • Limit your intake of soybeans and soy products, which contain enzyme inhibitors. Do not completely eliminate them from your diet, however, as they are valuable sources of protein.


  • Eliminate from your diet colas, foods with additives and colorings, junk foods, peanuts, processed foods, saturated fats, salt, sugar and sugar products, white flour, all animal proteins, and anything containing caffeine.


  • Take supplemental fiber daily. Alternate between psyllum husks and freshly ground flaxseeds. Take psyllium with a glass of water and drink it down before it thickens. Note: Always take supplemental fiber separately from other supplements and medications.

  • Exercise caution in your choice of foods so as to avoid exposure to foodborne illness. Food poisoning can be very dangerous for people with HIV or AIDS infection.

  • Determine what food allergies or sensitivities may be present. The best way to do this is to have yourself tested by a health care professional or allergist. It is important to eliminate allergic foods from the diet because they wreak havoc in the body, causing damage to the immune system.

    Avoid allergens that trigger allergies. Allergies increase the speed at which HIV infection becomes AIDS. High levels of immunoglobulin E (IgE), the antigen the body makes in response to allergens, make progression to AIDS roughly four times more likely in any 12 month period. In addition to working with the health care provider to reduce the risk of allergic reactions to the drugs in your treatment schedule, avoid exposure to airborne as well as foodborne allergens.

  • Avoid alcohol, noxious chemicals, and anything else that can damage the liver. Do not smoke, and stay away from secondhand smoke. Avoid recreational drug use. Some drugs, notably cocaine and marijuana, may not affect HIV infection itself but can disable T-cell response to other kinds of infections.


  • Obtain as much fresh air and rest as possible, and moderate amounts of sunshine. Get more exercise. While immune suppression through the use of powerful drugs is harmful to people with HIV, immune suppression through strenuous exercise seems to help. People with HIV who exercise tend to have fewer complications and to progress much more slowly to AIDS. As at a local health club about designing an appropriate exercise regimen.


  • Take vitamin E. This vitamin may be able to slow the progression of HIV infection to AIDS. According to a report in the journal AIDS, those men with the highest blood levels of vitamin E experienced a 34 percent reduction in the risk of disease progression when compared with men who had the lowest blood levels of this vitamin. Studies in both humans and animals have shown that vitamin E supplements in doses up to 800 IU per day can increase the immune response to foreign particles.


  • To boost the body's ability to fight harmful free radicals, take glutamine, grapeseed extract, or Pycnogenol and vitamin C. Among people with very low T-cell counts (under 200), glutamine supplements (200 mg daily) greatly increase survival time. This inexpensive and non-toxic treatment works by supplying an amino acid needed for the production of glutathione (GSH), the body's principle free radical fighter. Vitamin C is a GSH cofactor, or a substance that can make GSH more effective. Take at lease 200 mg daily, or eat fresh fruits and vegetables every day.


  • Avoid factors that deplete the body's store of GSH. Ultra-violet radiation (usually from sunlight), or the consumption of alcoholic beverages or acetaminophen-containing drugs (like Tylenol) can lower GSH levels.


  • Take 150 milligrams of lipoic acid 3 times a day. This free-radical fighter slows the destructive action of T-suppressor cells, and is toxic to HIV.

  • Of all the herbal products available for AIDS, curcumin may be the most versatile for stopping the virus at every stage of infection. It deactivates the gene in HIV that takes the virus out of its dormant state. It contains chemicals that stop the action of integrase, an enzyme the virus needs to take over the DNA of an infected cells and it prevents the production of tumor necrosis factor (TNF), a hormone that signals HIV if there are weakened or injured cells ready to be infected.

    Anyone with HIV or AIDS can make a major contribution to his or her survival and quality of life by getting into an early treatment program, especially a program in which immune enhancement is encouraged. The person with AIDS needs higher amounts of all nutrients than normal because malabsorption is a common problem. Those at risk of becoming infected with HIV or of developing AIDS can also be helped through the Nutritional Program found in the links below. For all nutritional supplements, it is strongly recommended that the use of sublingual and injectable forms, as well as rectal suppositories, if available, for improved absorption. Mullein made into a syrup can be given for palpitations, irregular heartbeat, angina, and other coronary conditions. The oil of this herb can be used to soothe the pain of ear infections. Externally, Mullein can be used to treat hemorrhoids, bruises, frostbite, diarrhea, erysipelas (streptococcus infections) and migraine. Mullein comes in various forms and is an ingredient in other products. For Mullein tea, add 1 to 2 teaspoons of dried leaves and flowers to 1 cup of boiling water. Drink as needed for soothing relief. For children, take a 1/2 handful of dried or cut fresh Mullein leaves and flowers in 1 quart of boiling water. Let sit for 30 minutes and strain. Add 2 tablespoons of dark honey. Drink 1/2 cup of warm tea every 3 or 4 hours. To relieve the pain of ear infections (make sure the eardrum is not punctured), squeeze several drops of Mullein oil (room-temperature) into the ear canal. These creams usually contain 0.025-0.075% capsaicin. In 2002 there was some research done that helped to determine Capsaicin's pain relieving effects. Creams that contain the compound lowered pain in arthritis sufferers' hands by 40% when used four times a day. A study done for pain from long-term shingles found that 77% of the people had reduced pain after using the cream for four months. The study also said that Capsaicin containing ream is less expensive and safer than other painkillers used for the same conditions. Cayenne Pepper comes in various forms and is an ingredient in many products. For taking capsules, the recommended daily dose is 1 capsule 2 to 3 times a day. Adults and children two and above can safely use it topically 3 to 4 times a day. For treating a sore throat, combine the juice of one-half lemon or lime with one full tablespoon of salt. Stir both into one-half cup of lukewarm distilled water. Stir in one-quarter teaspoon of Cayenne pepper. Gargle as needed, but do not swallow.

    AIDS

    A debate has gone back and forth in herbal circles for at least the last ten years in the U.S. about whether echinacea is contraindicated in HIV infection or AIDS. I've been unable to locate any published case reports where echinacea worsened the symptoms of AIDS, even though AIDS patients commonly take echinacea (Kassler et al 1991; Gowan et al 1993). Objections to the use of echinacea in AIDS have been theoretical, not based on empirical evidence.

    T-4 CELL LEVELS

    I was partly responsible for spreading the rumor that echinacea can lower T-4 cell counts, when I stated such a contraindication in an article in East-West magazine in August 1986. My reasoning was based on a just-published article showing that echinacea administration could lower T-4 cell counts, with T-8 cells remaining stable (Gaisbauer and Zimmerman, 1986). Since AIDS is characterized by a decline in T-4:T-8 cell ratios, it seemed reasonable to avoid echinacea. Further examination, however, shows that this trial was done with daily injections for seven days, rather than oral use. It's is now well-known that frequently-repeated injections have this effect, although it has not been noted with oral use. In fact, one clinical trial showed a steady improvement in cell-mediated immunity over a period of ten weeks with oral echinacea administration (Coeugniet and Künast 1986). Another trial showed that oral echinacea was most effective at preventing infections in those with low T-4:T-8 ratios (Schöneberger 1992). There is no evidence of any need to avoid the oral use of echinacea to maintain T-4 cell levels.

    AUTOIMMUNITY & AIDS

    The second theoretical objection to using echinacea in AIDS comes from the theory that AIDS has an autoimmune component. In the 1990s the theory emerged among leading AIDS researchers that the HIV virus initiated a series of events that left parts of the immune system attacking itself. So the objection is that immune stimulation by echinacea could hasten this autoimmune attack. This autoimmune component of AIDS no longer plays a prominent role in the theory of the dynamics of the disease. Current opinion is that vital replication in massive amounts is countered by likewise large formation of T-cells, until gradually, usually over years, the virus wins the battle (Wei 1995). The declining number of T-cells leaves the immune system fatally weakened. This theory is supported by newer drug therapies which have led in the last two years to a decline in AIDS deaths in the U.S. and France, and, if this theory is correct, there is no theoretical reason to avoid echinacea on the basis of fears of stimulating autoimmunity. It may remain prudent to monitor the status of AIDS patients taking echinacea




    However, many AIDS infected individuals do use the herb as a way to boost their immune system and stimulate T-Cell replication (rumor is that it also will stimulate viral replication too, but this may not have any scientific fact behind it to date). If you decide to use it, use only oral administrations of the herb. Do not use injections.
